J. A. Glass pointed out that both TIF and SIF were higher than last year!s record. It appears that we have broken the upward trend which occurred after we broke the previous all time high record for manhours worked without a lost time injury. Although a definite improvement has been noted, we have a long way to go to even meet our last year1 s record performance.
The line organization through personal contact made an impact whixHi resulted in the turn-around noted above, however, the interest and support of the line supervision at every level is needed to make the search for "New Avenues to Safety Awareness" program work. The members of the board were reminded that the NASA program was a plant-wide effort and should not be left entirely to the Unit/ Craft representatives and their Contact Supervisors. The success of the program depends upon sustained effort and involvement of people at every level throughout the plant.
REPORT OF SAFETY ADVISORY COMMITTEE (SAC) -- McCullough
A. Biennial Loss Prevention Review Procedure
The committee reviewed the proposal for revised Biennial Loss Prevention Reviews and listed several items that needed clarification and recommended preparation of a P-Bulletin to describe the appropriate procedure. The procedure will be submitted to the PMSB for explanation and approval.
B. Combustible Liquid Storage
The Safety Advisory Committee reviewed recommendations made by G. W. Daues and J. E. Fox and concurred with them. The committee directed the Safety and Housekeeping Action Committee to prepare a bulletin to cover drum storage and other flammable liquids.

C. Ownership and Responsibility for Fires and Spills Outside Battery Limits
The committee recommended that the fire crew should handle all fires and spills regardless of the nature of the spill, whether it is hazardous or super-hazardous and that staffing and training must be stepped up and emphasized for this group to insure theircapabi1ities in this area. They also made a recommendation to integrate super-hazardous material emergency procedures into regular procedures through bulletins, alarm code modifications, and training. This subject will be discussed at the next PMSB meeting to establish the nature and extent of the problems in this area and to clarify responsibility and appropriate actions.

CONTROL OF PERSONNEL PROTECTIVE EQUIPMENT -- Glass
We are seeing an increasing number of unapproved cotton gloves that do not meet plant standards for proper protective equipment. Supervision at all levels should be reminded to comply with the P-Bulletin regarding the purchase and use of all kinds of personal protective devices. It is not the intent of this bulletin to exclude innovation; however, if anyone has a question about approval of desired items, please have them contact Tony Ozarchuk in the LP&S department.
FOLLOW-UP ON SWAPS AREA (STYRENE) STUDY -- Glass
In November of last year, the Safety department made a detailed inspection of the salt water pump pit and condensate stripping area of the styrene plant, and issued a detailed report of hazards identified. A recent report hy the styrene plant supervisors indicated that 26 up-grading steps, were completed and 9 others were still in progress. This report exemplifies the fine record of accomplishment and a very thorough analysis of the needs of this department to identify and eliminate hazards in old operating units that had been modified many times on

a contingency basis over many years. The report shows dedication to sound principles of thorough investigation and elimination of hazardous conditions within operating areas.
REVIEW OF RECENT INCIDENTS
A. A report of the painting contractor lost-time accident was given by Earl Moore for R. C. Martini.
B. Earl Moore also reported on the explosion in center support clarifier.
C. Norm Marsh reported on the near-miss accident in Phthalic Anhydride.
AWARDS RECEIVED;
National Safety Council's Award of Honor for safety performance in 1975.
Manufacturing Chemists Association "Certificate of Achievement" in recognition of outstanding safety record attained by operating without a disabling Injury in 1975.
Letters of congratulations from Mr. Hanley to P. E. Brubaker were received along with the awards. Copies of the letter and the awards will be placed in the display case and copies of the letters were distributed to all members of the formal plant-wide safety organization.
EMERGENCY PROCEDURE DRILL AND ALARMS
The board requested that a formal presentation be prepared for a review of the emergency procedure plan. J. A. Glass and J. F. Carlisle will arrange this meeting.
PIPE RACK SIGNS
Bill Latimer has issued a request for the annual inspection of the pipe rack signs in accordance with P-Bulletin P-0403. The labeling of materials is also a requirement of OSHA regulations. The correction or additions to the identification signs are to be returned to Latimer by 7/14/76.
