Average) REPORT OF SAFETY ADVISORY COMMITTEE (SAC) -- DAUES (a) Review of addition to P-0100 proposed by SHAC The addition proposed on expansion of the safety organization was accepted by SAC with a minor revision. (The approval with the minor revision was given to R. L. Stuart`immediately after the meeting. (b) Emergency Communications System The committee presented a proposal for a single console wireless system purchased to be installed at a location SC 002638 STG 240l95 LAM003845 Minutes of PMSB Meeting - 9/21/77 Page 2 near the main gate. This system would: 1) Be capable of equivalent emergency communications to those we now have in the emergency communi cations center, 2) Be remotely located for backup, 3) Be independent of wire lines, and 4) Allow for future wireless improvements in the ECR. The estimated cost of this system is 120,000. The PMSB reviewed some possible alternatives bo this proposal including consideration of the front gate house, west gate or mobile unit locations. (c) MCI Transportation Accident Policy Texas City materials handling supervisors have reviewed and approved the "Legal Considerations and General Guidelines" sections of the MCI policy being put together by Cole Downing. Copies of the two sections (mentioned above) had been sent to SAC members for review. (d) Heim Radio Communications The ham radio at the gate house has been checked and made operable. Jim Cavender has accepted the assignment of heading up the operation of this equipment for those emergencies which require its use. He has also found three other qualified employees who have volunteered to assist with the radio during extended emergencies such as hurricanes. At present it appears that our radio is obsolete and consideration should be given to upgrading or replace ment. In addition our ability to use this radio system to contact our Houston office is limited to emergency messages only via a radio-phone system at the CD headquarters north of Dickinson. Delays waiting for Houston phone lines are very likely with the current system. Therefore, SAC feels we ought to consider installing a radio system in our Houston office. In addition we should coordinate our radio communications with Chocolate Bayou. George Bostick has accepted the responsibility of coordinating our policy and actions on the radio system and will work with Jim Cavender. Glass has agreed to arrange a test for switch engine horn warning at each intersection. This will require informing evening shift personnel (via Dial 5 and bulletin boards) of the test to avoid confusion with horn warning systems. This test will be arranged sometime after housekeeping inspec tion has been completed. (f) Future Policy Considerations 1) We have been asked to form a task force to study and make recommendations to overcome problems involving ground faults. (JAG/GWD 9/7/77) 2) A policy decision on guarding smooth rotating shafts has been requested by E. P. Bailey. Pertinent information is being circulated to SAC members. 1 3) A suggestion for color coding of locks for lockout use has been received. REPORT OF SAFETY HOUSEKEEPING ACTION COMMITTEE (SHAC) -- STUART Final schedules and plans for annual housekeeping inspection were reviewed. 4. DECONTAMINATED ITEMS SENT TO SALVAGE DEPT. -- DANNER A memo discussing the problem was distributed prior to this meeting with plans for reviewing it at a subsequent PMSB meeting. SCAFFOLDING PROBLEMS The Board discussed the need for careful review of scaffolding throughout the plant. Some scaffolds are being poorly constructed by contractors or modified after being properly erected.
