formed on Mr. Paul G r e i s c h a r on I I - I O -65 were
not e n t i r e l y s a t i s f a c t o r y due to lack of patie nt c o o p e r a t i o n . However, s i n c e t h e pulmonary com pliance is increased, sign ific a n t re stric tiv e disease is ruled out; in fact, the increased compliance and small transpuImonary pressure a t maximal i n s p i r a t i o n s u g g e s t s t h a t th er e i s loss of " e l a s t i c r e c o i l . " This is compatible with a c l i n i c a l d ia g n o sis of ob stru ctive pulmonary emphysema.

Since m y last examination in June of this year the patient has had some further difficulty. On July 4th he developed an illness which was subsequently diagnosed as a slight stroke. The patient had a sudden onset of severe weakness of the right leg, weakness of the right side of the face and trouble with his speech. He was hospitalized at Concord Community Hospital. Within a few days his face and speech problems subsided completely. He continued having some weakness in the right leg for a longer period of time; however, after about a month this also cleared up entirely. At the present time he has no symptomatic residuals from this episode. He states he has had considerable difficulty with high blood pressure since this episode. He has been on six different medications for his high blood pressure and he states he has been unable to tolerate any of these because of symptofhs of dizziness, insomnia and headache. His left leg at the present time feels warmer than the right leg and he has a sensation like an electric shock going through the left leg.
Insofar as his pulmonary problems are concerned he feels that his shortness of breath is somewhat worse than it was in June, continuing the gradual progression which he has noted previously. He has an occasional morning cough, but nothing of any severity. He has an occasional pain in the left lower chest. At the present time he becomes out of breath on walking about one block on level ground and one short flight of stairs. He becomes.out of breath quite promptly if he tries to climb any hill or incline. He worked for 13 days in June. Otherwise he has not done any work since m y last examination.
Physical Examination:
Eyes, Ears, Nose, Throat - No abnormalities detected..
Lymph Nodes - No significant enlarged lymph nodes are felt.
Chest - The lungs are clear to percussion, and auscultation. There are a few
crackling rales heard posteriorly at the left base. The breath sounds are somewhat
diminished in intensity.

Physical Examination CONTINUED: Heart - Not enlarged, rhythm is regular, no murmurs are heard. Extremities - There is no edema. There is probable early clubbing of the fingers and toes. Reflexes are generally hyperactive, but equaL Bab inski sign is negative bilaterally.
X-Ray Examination of the Chest: There is no significant change when compared with the previous film of June 15, 1965. The x-rays show, as before, changes consistent with pulmonary emphysema and, in addition, a suggestion of fine fibrosis in the middle portion of both lungs, more evident on the left. It is felt that the principal x-ray abnormality is that which would ordinarly be associated with pulmonary emphysema, but that there is evidence of abnormality consistent with asbestosis, although not entirely diagnostic of such.
The patient was again referred to the Cardiovascular Research Institute at the University of California for further pulmonary function studies. Such studies were performed on October 7, 1965. Copies of their report are enclosed. I would disagree to some extent with the interpretation of these results in that in m y opinion the figures obtained do show evidence of significant obstructive airway disease consistent with emphysema. Because of this apparent difficulty in interpreting their results, it was felt that a further procedure was indicated, namely what is known as a pressure-volume curve of the lung which is frequently able to distinguish between diseases such as emphysema and diseases of pulmonary fibrosis such as asbestosis. This study was performed on November 11, 1965. A copy of their report and conclusion is enclosed. It is noted that this study demonstrated -abnormalities which would be expected to be found in emphysema and did not demonstrate abnormalities which would be expected to be found in a restrictive type of pulmonary abnormality. Asbestosis would be expected to produce a restrictive type of abnormality.
In m y opinion these further studies confirm the previous impressions that this patient is suffering mainly from pulmonary emphysema. However, there is a good possibility that he does have some degree of Asbestosis in addition to his pulmonary emphysema. In m y opinion, if he does have any Asbestosis it is of a relatively mild degree and that the majority of his shortness of breath and disability is produced by the emphysema and not by the Asbestosis, although there is a possibility that some of his present disability is produced by Asbestosis. Unfortunately I do not know of any way of putting the relative amounts of his

disability due to these different diseases into any precise percentage other than to state as above that I do feel the emphysema produces the majority of his shortness of breath, but that the possibility that some of his shortness of breath is produced by Asbestos is cannot be ruled out.

139118 Mr. Paul J. Greischar REPORT OF RE-EXAMINATION
Since m y last examination in June of this year the patient has had some further difficulty. On July 4th he developed an illness which was subsequently diagnosed as a slight stroke. The patient had a sudden onset of severe weakness of the right leg, weakness of the right side of the face and trouble with his speech. He was hospitalized at Concord Community Hospital. Within a few days his face and speech problems subsided completely. He continued having some weakness in the right leg for a longer period of time; however, after about a month this also cleared up entirely. At the present time he has no symptomatic residuals from this episode. He states he has had considerable difficulty with high blood pressure since this episode. He has been on six different medications for his high blood pressure and he states he has been unable to tolerate any of these because of symptoms of dizziness, insomnia and headache. His left leg at the present time feels warmer than the right leg and he has a sensation like an electric shock going through the left leg.
Insofar as his pulmonary problems are concerned he feels that his shortness of breath is somewhat worse than it was in June, continuing the gradual progression which he has noted previously. He has an occasional morning cough, but nothing of any severity. He has an occasional pain in the left lower chest. At the present time he becomes out of breath on walking about one block on level ground and one short flight of stairs. He becomes.out of breath quite promptly if he tries to climb any hill or incline. He worked for 13 days in June. Otherwise he has not done any work since m y last examination.
Physical Examination:

Eyes, Ears, Nose, Throat - No abnormalities detected..
Lymph Nodes - No significant enlarged lymph nodes are felt.
Chest - The lungs are clear to percussion and auscultation. There are a few
crackling rales heard posteriorly at the left base. The breath sounds are somewhat
diminished in intensity.

Physical Examination CONTINUED: Heart - Not enlarged, rhythm is regular, no murmurs are heard. Extremities - There is no edema. There is probable early clubbing of the fingers and toes. Reflexes are generally hyperactive, but equaL Babinski sign is negative bilaterally.
X-Ray Examination of the Chest: There is no significant change when compared with the previous film of June 15, 1965. The x-rays show, as before, changes consistent with pulmonary emphysema and, in addition, a suggestion of fine fibrosis in the middle portion of both lungs, more evident on the left. It is felt that the principal x-ray abnormality is that which would ordinarly be associated with pulmonary emphysema, but that there is evidence of abnormality consistent with asbestosis, although not entirely diagnostic of such.
The patient was again referred to the Cardiovascular Research Institute at the University of California for further pulmonary function studies. Such studies were performed on October 7, 1965. Copies of their report are enclosed. I would disagree to some extent with the interpretation of these results in that in m y opinion the figures obtained cb show evidence of significant obstructive airway disease consistent with emphysema. Because of this apparent difficulty in interpreting their results, it was felt that a further procedure was indicated, namely what is known as a pressure-volume curve of the lung which is frequently able to distinguish between diseases such as emphysema and diseases of pulmonary fibrosis such as asbestosis. This study was performed on November 11, 1965. A copy of their report and conclusion is enclosed. I t is noted that this study demonstrated abnormalities which would be expected to be found in emphysema and did not demonstrate abnormalities which would be expected to be found in a restrictive type of pulmonary abnormality. Asbestosis would be expected to produce a restrictive type of abnormality.
In m y opinion these further studies confirm the previous impressions that this patient is suffering mainly from pulmonary emphysema. However, there is a good possibility that he does have some degree of Asbestosis in addition to his pulmonary emphysema. In m y opinion, if he does have any Asbestosis it is of a relatively mild degree and that the majority of his shortness of breath and disability is produced by the emphysema and not by the-Asbestosis, although there is a possibility that some of his present disability is produced by Asbestosis. Unfortunately I do not know of any way of putting the relative amounts of his

disability due to these different diseases into any precise percentage other than to state as above that I do feel the emphysema produces the majority of his shortness of breath, but that the possibility that some of his shortness of breath is produced by Asbestosis cannot be ruled out.
