Section I - INTRODUCTION PURPOSE OF THE INSPECTION EPA Region 6 inspectors Kerri McCabe (lead) and Uma Lad arrived at the Plummer, JD Operating Co., LLC / Parham-IPCO #1 Tank Battery ("facility") located from Interstate 49 S, N Fouke Rd, left on Miller CR 24, right on Miller CR 41, and left on lease road at 33.27099, -93.91321 in Fouke, Miller County, Arkansas; at 1445 on 01/12/2026 for an unannounced inspection. No facility representatives were present at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection on the Plummer, JD Operating Co., LLC / Parham-IPCO #1 Tank Battery facility, which contained three (3) tank batteries with four (4) tanks for crude oil and saltwater separation and storage, one (1) saltwater disposal (SWD) injection well, and one (1) active production well.

Section II - OBSERVATIONS I observed the facility signage and an overview of the facility; see Appendix 1 (Photographs Log), Photo No. 1 and No. 2. For additional GPS location and photo metadata, see Appendix 1. I observed an active production well at the facility, see Photo No. 3. I observed a separator inside a tank battery, see Photo No. 4. I observed the interior of the oil/saltwater storage tank battery (front view), see Photo No. 5. I observed the loading lines for the oil/saltwater storage tank battery without catchment buckets, see Photo No. 6. I observed the SWD injection well and pump inside a tank battery, see Photo No. 7. Section III - AREAS OF CONCERN No closing conference was held due to no personnel with Plummer, JD Operating Co., LLC being present during the inspection. The area of concern at the time of the inspection was:
Loading lines outside the tank battery without secondary containment, 40 CFR 112.7(h)(1) (Photo No. 6)
