That a p p lic a n t became tem porarily t o t a ll y d isab led on May 20, 195?, which temporary t o t a l d i s a b i li t y continues to the p resen t time with no c e s s a tio n date a v a ila b le or fo re s e e a b le . That a p p lic a n t su stain ed and incurred s u b s ta n tia l medi c a l treatm ent to cure and r e lie v e from the e f f e c t s ' o f h is in d u s t r ia l
in ju r y which need continues with no c e s s a tio n date a v a ila b le or f o r e seeable.
