All of the essential facts are, of course, missing but the significant point, as I am sure you will agree, is that no precautions of any sort were taken, for protecting the men engaged in the tank cleaning operation, and this apparently was due to the fact that the Superintendent of the Depot had only recently been appointed and was unaware that special precautions were required. This is a typical example weof what have discussed together on many previous occasion?, namely, the difficulty of ensuring that the men actually doing the job are familiar with the regulations.
