I presented my credentials to Richard Deck and informed him that this was an EPA RCRA inspection. The table above presents all the opening and closing conference inspection participants. New Power Plant has gone online - generating coal ash, waste lamps managed as universal waste, and used turbine oil. Atkinson Central Power Plant has been decommissioned. Notified under Subpart P for waste pharms - Student Health Counseling Center, Veterinary clinic. Facility opted into Subpart P; however, they do not generate any waste pharmaceuticals, either hazardous waste or non-hazardous waste, that would go back to a reverse distributor for credit. Mr. Riley joined the opening conference at 10:18 a.m. I asked for a list of all the hazardous waste and universal waste accumulation areas and was provided with this information during the inspection which is also included as attachments to this report. During the inspection I was accompanied by Messer's. Deck and Riley. During the inspection I looked at the facility's processes, in addition to hazardous waste management practices, generation points, and accumulation areas. I looked for wastes that facility representatives had not yet identified or designated as hazardous. I also observed the facility's universal waste and used oil management. At the time of the inspection, the tote was not marked with a hazard indicator. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the clear plastic tote that contained waste aerosols had been re-labeled with the words "Universal Waste-Aerosol Cans." Observation_Image-202109151052235223346221.jpg Building: Atkinson Power Plant/ Maintenance Shop Contains CBI: No SAA Observation #: JJ1-OB-018 Date: 09/14/2021 While inspecting the satellite accumulation area (SAA) near the maintenance shop, I saw a 55-gallon container with an attached aerosol can popper and filter. I asked the operator, Mr. Dean Ojala, Jr., CT3 Maintenance Mechanic Lead, about the container and he said they puncture aerosols into the container. At the time of the inspection, the container was approximately 1/6 full. The container was not labeled with the words "Hazardous Waste" and had no hazard indicator. UAF recycles the punctured aerosol cans as scrap metal. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the 55-gallon container had been labeled with the words labeled with the words "Hazardous Waste" and a hazard indicator. Neither container was labeled with the words "Used Oil." Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the two 55-gallon containers had been labeled with the words "Used Oil." One box was closed and had no labeling and the other box was labeled as universal waste lamps but not closed. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the two cardboard boxes that contained waste fluorescent lamps were taken back to the Bldg. 799 - Hazardous Materials Handling Facility (HMF), where they were repackaged and labeled with the words "Universal Waste-Lamps." I looked at the labeling on one of the containers and saw that it was labeled as containing Iso-Propyl Alcohol. The containers were in a bermed concrete containment area that held an accumulation of liquid (potentially rainwater). I could see rust around the bottoms of the containers. According to Mr. Deck, five of the containers held Isopropanol, one held Methanol, and one held Ethanol. Mr. Deck told me that the containers held product that was used in various labs across campus. According to Mr. Deck, the containers held product. According to the labels on the containers, they held North Slope Crude Oil and were marked with DOT Class 3 hazard indicators. The exterior of each cabinet was marked with a hazard indicator. Mr. Deck confirmed the contents in the cabinets consisted of both product and hazardous waste. The products and wastes were separated in each cabinet by either being placed on separate shelves or on opposite ends of the same shelf. I looked at the wastes inside the cabinets and saw that all the hazardous wastes were labeled with the words "Hazardous Waste", but not a hazard indicator. Photo(s) 1. There were eight lamp boxes on the pallet and three of those lamp boxes were not closed. Two of the lamp boxes were not structurally sound. I saw one box at the bottom of the pile of boxes on the pallet, that was crushed from the boxes that were on it. It appeared to me to the weight of the other boxes caused the box on the bottom of the pile to be open. The other open box was broken (and therefore open) on the side. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the cardboard boxes that contained waste fluorescent lamps had been repaired. I asked Mr. Riley what the boxes held, and he said they held waste light ballasts and that each one contained a Ni-Cad battery. At the time of the inspection these ballasts were not labeled as a universal waste battery. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the battery containers had been labeled. Although labeled, the photo documentation did not show that the containers were labeled with the words "Universal Waste." The photo showed them labeled as Non-RCRA waste. I contacted Mr. Deck by telephone on 11/22/21 to inquire about the labeling and he followed up with an email letting me know that the battery containers were in fact labeled with the words "Universal Waste." The plastic container held various types of waste batteries including lithium and NiCad. According to Mr. Riley, the batteries were picked up from other locations on campus last Friday (9/10/21) and brought to the HMF. According to Mr. Deck, the waste lithium and NiCad batteries are managed as universal waste. There were at least 13 lithium/Ni-Cad batteries in the plastic container. Neither the plastic container nor any of the batteries were labeled as universal waste. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the battery containers had been labeled. Although labeled, the photo documentation did not show that the containers were labeled with the words, "Universal Waste." The photo showed them labeled as Non-RCRA waste. I contacted Mr. Deck by telephone on 11/22/21 to inquire about the labeling and he followed up with an email letting me know that the battery containers were in fact labeled with the words "Universal Waste." The label on the tray indicated that it held an ultraviolet (UV) light bulb/lamp that contained mercury. According to Mr. Deck it had been in storage for approximately six months and would be managed as a universal waste lamp. The UV bulb/lamp was not labeled as universal waste. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, in which he stated that the UV bulb/lamp had been shipped to Total Reclaim, along with other universal waste lamps for recycling on September 14, 2021 and September 16, 2021. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, in which he stated that the universal waste-lamps had been shipped to Total Reclaim, along with other universal waste lamps for recycling on September 14, 2021 and September 16, 2021. One was labeled "Ethyl acetate waste" and one was labeled as "DCM & Chloroform waste" At the time of the inspection, neither container was labeled with the words "Hazardous Waste" or the hazard indicators. The other fume hood had a small open container that was labeled "Methanol waste." The container was not closed, and the fume hood was actively running. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the containers of hazardous waste observed during the inspection were labeled with the words "Hazardous Waste" and hazard indicators. In the email, Mr. Deck stated that the open container marked Methanol waste, in the Room 442 fume hood is an immediate use container used only while the apparatus (Vent hood) is running. Any waste collected is consolidated into the Room 448 Methanol waste container. Mr. Deck also provided a copy of an inventory of the containers identified. While inspecting the contents of the cabinet, I saw various sized containers that held (according to the container labels) wastes such as: Alcohol waste Ethyl acetate waste Acetic acid waste Acetone waste Chloroform waste Benzene, ethanol waste Toluene and ethanol waste Chlorinated waste Hexane waste Waste acidified - isopropyl alcohol KO4 waste Chloroform methanol waste HNO3 waste and Oxalic acid waste At the time of the inspection, none of the waste containers in the flammable storage cabinet, in Lab B, were labeled with the words "Hazardous Waste" or hazard indicators. Neither was the flammable storage cabinet, in Lab B labeled with the words "Hazardous Waste". The storage cabinet was labeled with the word, "Flammable." The container it was in was not closed or labeled. It was just sitting out on a table with other items and, according to Mr. Deck, required a waste determination. I saw another container across from the previous table that was labeled with a yellow piece of tape and included the words "Assay Waste." The container was not labeled with the words "Hazardous Waste" or a hazard indicator. I also saw a fifth container of waste Formalin inside the fume hood that Mr Riley told me was waste Formalin. 22 of 85 UNIVERSITY OF ALASKA FAIRBANKS Inspection Date(s): 09/13/2021 - 09/15/2021 At the time of the inspection, I did not observe a label on any of the six containers that indicated if they held a hazardous waste nor did a I see a label that indicated the hazards of the container contents. Following the inspection, Mr. Deck sent me an email, dated October 8, 2021, that included both written and photo documentation that the formalin waste containers located in the O'Neill 143 laboratory had been determined to be non-regulated. The formalin solution is a 10 percent solution containing 3.7 percent formaldehyde. While processing the samples the solution is further diluted. I then explained the follow-up process that would take place once I finished the report. I explained they would receive a copy as would a case officer. I told them that the case officer would review the report to determine if any of my areas of concern documented violations.
