I met with site representatives Nate Hernandez (Senior Superintendent), Joe Shaub (Assistant Superintendent), Madeleine Forde (Senior Project Engineer), Collin Lagree (Senior Project Manager), and Dennis Darling (Safety Coordinator) of HCBeck, Ltd (hereinafter referred to as "HCBeck") at the construction trailer off Reeder Road to conduct an opening conference. I presented my credentials to the site representatives and informed them that a random stormwater inspection was being performed to determine compliance with the Texas Pollutant Discharge Elimination System (TPDES) program and the Construction General Permit (CGP), in accordance with the Clean Water Act (CWA). The generation of this report is based on information supplied by HCBeck representatives (the permittee), observations made by the inspector, and records and reports maintained by the permittee, the state of Texas, and the U.S. EPA. FACILITY DESCRIPTION Texas Health Forney Hospital is a commercial construction site operated by HCBeck. The site is northeast of Reeder Road and West US Highway 80 in Forney, Texas (denoted in Aerial Image 1 below) with coordinates in the vicinity of Latitude 3244'31.1" North, Longitude 9625'40.4" West. The project totals 40.6 acres and discharges into a tributary of Mustang Creek and a tributary of Big Brushy Creek in the City of Forney's Municipal Separate Storm Sewer System (MS4). 2 HCBeck, Ltd. / Texas Health Forney Hospital Permit #: TXR1501TD Inspection Date: 2/5/2026 Aerial Image 1: The Texas Health Forney Hospital commercial construction site. The site is located within the approximate blue boundary lines depicted above (northeast of Reeder Road and West US Highway 80 in Forney, TX). Section II - OBSERVATIONS The following points of discussion are derived from on-site visual observations on February 5, 2026: The Construction Site Notice (CSN) was posted near the construction entrance off West US Hwy 80 (Appendix 1, Photo 1). The site map and Stormwater Pollution Prevention Plan (SWPPP) were observed inside the construction trailer (Appendix 1, Photo 2-3). Silt fence was installed along the perimeter of the site (Appendix 1, Photos 1, 4-5). The silt fence was observed in effective operating condition. The inlets throughout the site had BMPs installed (Appendix 1, Photo 5). The BMPs observed were in effective operating condition. Two (2) fuel storage tanks were observed on-site (Appendix 1, Photo 6). Both containers were plugged with no evidence of leakage. Two (2) retention structures were observed on-site. Near the southwest perimeter of the site was a retention pond (Appendix 1, Photo 4) that collects surface water. Near the northeastern perimeter of the site was a dry sedimentation basin that receives water from the on-site inlets (Appendix 1, Photo 7). 3 HCBeck, Ltd. / Texas Health Forney Hospital Permit #: TXR1501TD Inspection Date: 2/5/2026 One (1) concrete washout was observed on-site (Appendix 1, Photo 8). An Area of Concern (AOC) was identified as described in Section III of this report with photos included in Appendix 1. Section III - AREAS OF CONCERN The following areas of concern are derived from on-site observations from the February 5, 2026, Compliance Evaluation Inspection (CEI): TCEQ General Permit No. TXR150000 Part V, Section C; Part VI(B): Concrete washout liquid/waste was observed on the ground near the prefabricated metal roll-off concrete washout bin (Appendix 1, Photos 8-9). The waste was cleaned and photo evidence was emailed by Madeleine Forde shortly after the inspection ended on February 5, 2026 (Appendix 2, Photo 1). Section IV - FOLLOW UP The following information was received by the U.S. EPA after leaving the site on February 5, 2026: On February 5, 2026, a digital copy of the SWPPP, CSN, site map, inspection reports, and photos of the corrected AOC were emailed by Madeleine Forde. I completed a detailed review of the documentation provided and no concerns were identified.
