(NOTE: The nuribers you enter in the boxes should be as far to the right as possible in all cases so that they may be easily added.) Line 4 - Generated During 1985 - Biter the total weight of the waste or waste mixture that you generated during the calendar year 1985. Line 5 - Total Amount for Which to Account - Enter the sum of Lines 3 and 4. Line 6-9- Shipped to: - Enter the EPA I.D. Number and Name of the waste handler to whom the self-generated waste was sent for treatment, storage, disposal, reshipment, use, reuse, recycling, or reclamation. Enter the amount of the waste. Use a different line for each waste handler. Line 10 - Treated On-site - Enter the amount of self-generated waste treated on your property during the year. Line 11 - Treatment Code - Enter the code that best describes the manner in which the waste on Line 10 was treated.
