SHAC Meeting With Gene Tromblee to Discuss Safety Philosophy R. A. Hernandez to set up. If committee cannot make it, Glass and Hernandez to review Safety Direction Philosophy with Tromblee. 3. Plantwide Alarm System Survey "All Clear" is hard to hear in various parts of the plant even after it was moved from Power 2 to the top of CT-2. Kuehl reported his wardens had trouble hearing "All Clear" also. Kuehl will personally listen for "All Clear" (work whistle) at 4:25 PM and report findings. 4. Fire/Entry Permit Procedure There is much concern about the ambiguity of delegation of responsibility on Special Permits and on the checking of equipment before issuing permits when the job lasts for several days. R. A. Hernandez will ask Doyle Wise to submit alterations to present P-Bulletin to make it more clear. SC 17246 LAM025498 File 2- - April 6, 1979 5. Overspeed P-Bulletin Bailey reported it was ready for issue after some revisions due to a overspeed incident early this year. 6. Relief Valve Records Baily reported a program is being worked on to get records on-line and computerized with the MICS System. This will make records more meaningful as the Valve Shop can enter the data on each RV as it is tested. Addi tionally, data can be gathered to optimize frequency of testing. Currently, all RV's are scheduled for testing once-a-year. The data, along with the type of service the RV is in,could conceivably show that RV's could go longer than one year. 7. Eyebath Dust Covers Bailey has gathered data for SAC that they requested. He will forward data and recommendations to SAC. 8. Safety Calendars from Gulf Oil SHAC agreed it would be a good idea to mail these out at year's end. J. A. Glass to follow up. 9. Idle Pipelines P-Bulletin Hernandez will obtain from SAC clarification of "Owning Department" before assigning P-Bulletin No. 0409 revision Task Force. tg Attachment R. A. Glass Doug Metten described the operation of the Safety Advisory Committee, pointing out that it is a working arm of the Plant Manager's Safety Board. They provide investigation follow-up review and preparation of proposed procedure or policy changes. They work on projects assigned by the Plant Manager or the Plant Manager's Safety Board. OLD BUSINESS: 1. The Hourly Safety Committee asked the Safety Advisory Committee to consider the possibility of lengthening the term of service of the Hourly Safety Committee from six months to one year. The purpose being to reduce the start-up time allowing the members to be more effective. If terms are staggered new members could be added at six month intervals. This item will be placed on the Safety Advisory Committee agenda. 2. Bicycle Safety: The committee asked that the load limit of bicycles be investigated as a part of the bicycle safety review currently in progress. There is some concern that bicycles are being over loaded by some people using them. This will be passed along to Raoul Hernandez. NEW BUSINESS: a^>k 1. Off Loading ofwDrums from Pallets - Dept : This concern is related to the back strain caused by tilting and removing loaded drums delivered on pallets by the truck department. 2. HCN Alarm in Pumper Gauger Shack: The concern in this area is that the HCN Alarm can not be heard in the shack and these workers are closest to HCN storage tanks. 3. Brain Tumor Concerns: The committee asked about the status of the brain tumor study and a number of other cancer cases were described. Chain Saw Leg Protectors: The concern here is that the leg protectors offered no protection to the upper leg when the operator of the chain saw is stooping or bending his knees. The upper extention of the protector interferes with the proper and safe operation of the saw. (Attention: H. Jackson, A. W. Kuehl, C. M. McCullough) 5. Asbestos Insulation from Styrene Column (16D1): The Concern here is about the Asbestos Insulation that is continuing to fall off the column creating asbestos hazard below. The committee was informed about the plans in progress to scaffold this tower from the ground to the top to make repairs as soon as possible. Meanwhile, alternative interim actions are being explored to prevent additional insulation from fall ing off the column. Asbestos Insulation on Lines in Department 35f: There are some in sulated lines in poor condition and are coming apart. An MRO needs to be issued to repair these lines. (G. L. Berry) 7* Asbestos Gloves: Are they still being issued for use in the plant? .Y 8 What is being done about eliminating the need for respiratory protection in the control house at Dock 2 while loading AN? Can anything be done on a short term basis to reduce the personnel exposure of Acetic Acid vapors when loading barges and tank cars? 3A& 10 Can Loss Prevention and Safety check on a news report that three firemen lost their lives due to a gas mask malfunction in some west Texas town?
