Willis Dept. 13/14 PLAINTIFF'S EXHIBIT The Second Quarter Safety and Housekeeping Inspection for Dept. 13/14 was conducted by the team of George Pappas, Jake Llnkey, Roger Mears, and Rick Sharp on April 23, 1985. Safety 1. The interlock bypass system is not formalized. There should be documentation on the proper procedure to be used. 2. There are eleven (11) AVIP's overdue. These have all been scheduled with six (6) awaiting acoustic emission tests by PETS. 3. There is concern among the operators about the handling of NSI. There is a natural work group looking at how to improve handling procedures. 4. Plectrons are operating properly. ) 5. The Protective Device "Out of Service" board should be kept clear of miscellaneous items such as flashlights, chains, etc. 6. Scaffolding north of 13E201 improperly erected. 7. Scaffold south of 14E14 improperly erected. 8. Generally, hoses on water and air not pinned with safety pin. 9. Personal safety equipment - good. 10. General knowledge of safety procedures - good. Housekeeping 1. Door handle off fire extinguisher box 13 - 17. Door will not close. 2. Warning Danger Acid Area sign next to 100 bldg, is an eyesore recommend replacing. 3. Upper structure of distillation area covered with rusty nails, nuts and bolts. 4. Six foot piece of pipe marked asbestos laying in southwest corner of Dept. 13.
