It has been called to SHAC's attention that ladders and barricade tape are being misused in the plant. I have attached two SHAC Reminders addressing this subject. Please review these with your personnel at the earliest op portunity.

LADDER RULE 5 GUIDE REMINDER:
Use of defective ladders is prohibited.
Inspect ladders before using.
Straight ladders must be equipped with safety shoes.
Straight ladders must be either securely fastened at the top or a man stationed at the bottom to hold the ladder in place.
When extension ladders are erected, the sections must be secured together with No. 16 baling wire or 30" plastic ties (MCC #042904) on both ends of the rungs, at the level of the latches.
When taking down an extension ladder, it should first
( be untied at the top of the ladder, and then untied at the rungs.
Do not substitute stepladders for straight ladders.
Return ladders to designated location after use.
When climbing up or down, face the ladder and use both hands.
Tools should be secured on person or hauled up with a line.
Don't overreach - move ladder as work progresses.
Do not place ladders in front of unlocked door unless properly guarded.
Straight ladder should be erected so that the bottom is one-fourth the length of the ladder away from the vertical surface against which the top is resting.
Straight ladders used to climb onto a work platform should extend at least 3^ feet above the working level.
No electrical work shall be done from a metal ladder. V
Do not stand on the top two steps of a ladder.

Do not enter taped off or barricaded areas unless authorized to do so by the owning department.
Remove tapes promptly when hazard no longer exists.
