FORM B SELF-GENERATED HAZARDOUS WASTE AND ITS DISPOSITION INSTRUCTIONS Fill in your installation's name and EPA I.D. Number in the upper left hand corner of the form. Line 1 - EPA Hazardous Waste Number - For listed wastes, enter the four character EPA Hazardous Waste Number from Georgia Rules for Hazardous Waste Management 391-3-11-.07 or 40 CFR Part 261, Subpart D (see Appendix) which identifies the waste. For unlisted wastes which exhibit hazardous characteristics, enter the four character EPA Hazardous Waste Number from Georgia Rules for Hazardous Waste Management 391-3-11- .07 or 40 CFR Part 261, Subpart C (see Appendix) which is applicable to the waste. Four spaces are provided in each column. For a mixture of more than one listed or unlisted waste, enter all of the relevant EPA Hazardous Waste Numbers. If there are more than four waste numbers in a mixture, continue on the next column, and leave all other items in that column blank (see attached example)*. Line 2 - On Hand, On-site on January 1, 1983 - Enter the amount of self generated waste, regardless of when generated, (expressed in tons to the third decimal place) that was on your property on January 1, 1983. (Waste placed in a disposal unit is considered disposed, not stored, and should not be shown on Line 2.) Line 3 - Generated During 1983 - Enter "the total weight of the waste or waste mixture that you generated during the calendar year 1983. Line 4 - Total Amount for Which to Account - Enter the sum of Lines 2 and 3. Line 5-8 - Shipped to State of ( ) - Enter.the name of the state (other than Georgia) to which the self-generated waste was sent for treatment, storage, disposal, use, reuse, recycling, or reclamation, and the amount of the waste.
