Interoffice Communication To Jim Barton From Ray McGhee Date : April 25, 1978 Subject : VCM RELEASE On Saturday morning, 4/15/78, at approximately 06:45 Lead Operator, Charles Holcomb, vented 744 reactor through the recovery system. During this venting the recovery bullseye blew out of the recovery line. There was no one in the area at the time of the blow out. Harold Sanders and Reggie Jones were on the way to the control room when the bullseye blew out. Harold Sanders went back to the unit and closed the manual valve on the knock out tank to stop the leak. Harold was not wearing a Scott Air Pack at the time.
