introduction
The planning for the epidem iological asp ects of th is p ro ject was begun in 1949. The objectives as finally defined by the M edical A dvisory C om m ittee a re as follows:
1. To a s c e r t a in th e in c id e n c e and p re v a le n c e of n e o p la s tic d is e a s e am ong em ployees of the in terested petroleum com panies.
2. To com pare these ra te s w ith those of the g en eral population and other industrial groups.
3. To ev alu ate, em ploying ep idem iological m ethods, housekeeping and hygienic arran g em en ts d irected tow ard elim inating or con tro llin g exposure to possible irrita n ts .
4. To advise, utilizing the re su lts of these studies, possible im provem ents or m odifications of p resen t hygienic m easu res.
Among the various approaches to th ese goals are:
1. H is to r ic a l S u rv e y . A ss e m b lin g and re v ie w in g of th e r e c o r d s of known cases in the files of the in te re ste d com panies.
2. C u rren t C ase R e g istry . E stablishing by m utual agreem ent a cen tral file for receiving and tabulating rep o rts of new cases as they occur am ong th e ir em ployees.
3. In su ran ce R eco rd S u rv ey s. Em ploying the re c o rd s of v ario u s health benefit plans to determ in e the incidence of neoplastic d isease, not only in the re fin e rie s, but also in o ther in d u stries located in the sam e gen eral a re a and using the sam e labor pool.
4. Studies of Skin C hanges. C om paring the incidence and prevalence of k erato se s an d /o r sim ila r hypertrophic lesions in groups of petroleum w orkers exposed.
5. Slack Wax S tu d ies. R epeating the observations on em ployees exposed to p o ssib le unusual h aza rd s due to th e ir w ork in wax extraction processes.
6. A dvisory S erv ice. F urnishing epidem iological consultant serv ice to aid in planning investigation and co n tro l p ro g ram s for individual com panies.
7. L ite ra tu re Review S erv ice. A lite ra tu re review serv ice was considered but later rejected. C ritical ab stracts of pertinent m ed ical lite ra tu re w ere to be p re p a re d and c irc u la te d p e rio d i cally to the m em b ers of the M edical A dvisory C om m ittee.

Only the f ir s t two have been im plem ented to date. The study of health insurance reco rd s has been held up until satisfacto ry rela tio n ships can be estab lish ed . F ield surveys w ere abandoned early because it was quite apparent that such studies m ust be c a rrie d out by the indivi dual com pany. While the advice and help in the collection of re c o rd s continues to be given on req u est, the lite ra tu re review w as never attem pted.

S in ce la te in 1950 u n til S e p te m b e r 15, 1955, 1,877 r e p o r ts have b e e n received from sixteen com panies. Tw enty-nine have not been included in the v ario u s tabulations because the am ount of inform ation subm itted is inadequate. The acceptable c ases have been listed by y ear of onset and the reporting com pany num ber in Table la . (See la s t y e a r's com m uni cation for com pany identification. )
G eographical coverage of the industry is now alm ost com plete. An
additional com pany has decided to p articip a te and is planning to re p o rt
cu rre n t c a se s. E ffo rts a re continuing to im prove the acc u racy of the
rep o rts; to include a la rg e r num ber of em ployees in the rep o rtin g base
(ten of the sixteen particip atin g com panies re p o rte d that they w ere re c o rd
ing c a n c e r c a s e s in a p p ro x im a te ly 120, 000 em p lo y ees); and to obtain the
participation of a ll the m ajo r com panies rep resen ted on the M edical A dvisory
C om m ittee. C om pleteness of reporting is likew ise a serio u s problem .
F r o m th e 120, 000 e m p lo y e e s , only 185 c a s e s w e re r e p o r te d d u rin g th e
y e a r 1954, a r a te of a p p ro x im a te ly 150 p e r 100, 000, o r ab o u t 1/3 of th e
value found fo r the age group 50-54 in the Public H ealth S ervice Study.
W hile the age d istrib u tio n m ay account fo r som e of this d ifferen ce, it is
obvious that a deficit is p resen t.

At the s ta rt of th is phase of the p ro ject, the inform ation from the clin ica l and occupational re c o rd fo rm s w as coded and tra n s fe rre d to key s o rt c a rd s s o rte d m anually. L ate in 1954 when it becam e ev id en t th a t p articip atio n by a larg e segm ent of the industry w as a ssu re d and the num ber of re p o rts reached a point w here the n ecessary m anipulations becam e cum bersom e, this m ethod of tabulation w as abandoned. E lectronic m ethods u tilisin g IBM equipm ent w ere substituted. This change in su re s g re a te r flexibility and p erm its the study of m ore variab les in finer detail.
Throughout the f irs t nine m onths of 1955, the w ork cen tered on the p rep aratio n of a new code; tra n sfe rrin g the inform ation to m a ste r tab les; and finally punching the c a rd s. U nfortunately, difficulties w ere encountered, som e of which a re still unresolved. A dditional c le ric a l a ssistan ce was assigned, b u t th e t a s k was not c o m p le te d u n til S e p te m b e r 1, 1955. A s a consequence, no tabulations w ere p re p a re d for the Buffalo M eeting, and only a few very b asic sum m ary tab les a re ready a t th is tim e although m ore com plex analyses w ill be undertaken in the n ear future.

and d ate o f o n a e t. It w ill be n o te d th a t 1, 093, o r 59% of th e 1, 848
a c c e p ta b le r e p o r t s , c o n c e r n c a s e s w ith o n s e ts s u b se q u e n t to J a n u a r y 1,
1950, the date a r b itr a r ily ch osen to s e p a ra te h is to ric a l fro m c u rre n t
reco rd s. This m eans that a reasonable num ber of new cases a re at
hand for fu rth er study.

It is notew orthy th at 42 individuals w ere rep o rted w ith two o r th re e neoplasm s. T hese cases a re p resen ted in Table lc . In subsequent tab u lations, the totals w ill include this group, counting each tum or a s a separate o ccu rren c e. Some b etter m ethod of including th is group in the su m m a ries w ill be sought during the com ing y ear. In Table 2, th e re fo re , th e 1935 n e o p la sm s a r e lis te d acc o rd in g to se x , r a c e , and date of onset. Since the known colored group is such a sm all segm ent of the to tal, ra c ia l sep aratio n is also neglected in the following sum m ary tables.
The f ir s t two of the sim ple su m m a rie s co m p ares by sex and date of onset the rela tiv e frequency of d ifferen t fo rm s of ca n c e r, both as to p rim a ry s ite an d ty p e, w ith th e r e s u lts of the 1947 P u b lic H ealth S e rv ic e Survey. It m ust be em phasized that these values a re not incidence ra te s but re p re se n t the m easurem ent of the frequency of one form of cancer by com paring the num ber of that form of cancer w ith the total num ber of all cases.
The d istrib u tio n of tu m o rs rep o rted by the participating com panies, both p rio r and sub seq u en t to 1950 and b o th a s to p rim a ry site and type of tum or, follow very closely the p attern found in the PHS Survey. A slight e x c e ss, 31.4% a s co m p ared to 28.7% of d ig estiv e sy ste m n eo p lasm s, is n o ted am ong m a le s w ith o n s e ts p r io r to 1950."- T h is d iffe re n c e is e r a s e d , and a sm aller proportion is found in th is category among the c u rre n t c a se s. H ow ever, skin and soft tissu e lesio n s a re m ore frequent, 25.9% a s to 19. 3%, in the g ro u p w ith o n s e ts su b se q u e n t to J a n u a r y 1, 1950. A higher proportion of non-glandular epithelial tum ors w ere reported from both groups.
The question of age adjustm ent alw ays en ters the pictu re. In these p articu la r co m p ariso n s, no effo rt has been m ade to take this into account. The e rro r introduced by failu re to consider this aspect is probably sm all, but it should be co nsidered in the future.
Sum m ary T ables 3 and 4 re p re se n t an attem pt to rela te the occupation of the patient in the petroleum industry w ith the p rim ary site of the tum or. Sum m ary T ables 5 and 6 a re a sim ilar attem pt to a sc e rta in the relatio n ship of occupation to type. U nfortunately, fo r m any of the occupational categ o ries, the num ber of ca se s rep o rte d is too sm all to p erm it adequate an aly ses, and no relian ce can be placed in the differences found. Sum m ation
It is planned to continue theee studies following essen tially the original plan. Now that the inform ation can be tabulated read ily by electronic devices, m any fu rth er analyses a re possible and w ill be undertaken.
The epidem iological investigations developed slowly for sev eral good and sufficient reaso n s. The firs t was that certain of the com panies have no cen tralized m edical departm ent to collect the n e c e ssa ry data, past or future. E ven w ith m edical d ep artm en ts, som e found it difficult to p articip ate because (a) the departm ent is new and has no p ast re c o rd s; or (b) the r e c o r d sy ste m in u se does not p e rm it e asy lo c a tio n of the clinical reco rd s of the case. A nother is that th is w as a p reced en t breaking d eparture from usual relatio n sh ip s. Com panies and physicians a re reporting voluntarily to an unofficial c en tral agency. P hysicians have always rebelled against even the official requirem ent of reporting of com m unicable d isease c a se s. T his m akes it m o re difficult to secu re cooperation on a voluntary b asis. An occasional m anagem ent is still w ary and is w aiting to see what happens.
The cooperation obtained and the p ro g re ss m ade in view of th ese many difficulties is significant although, a s yet, little reliance can be placed on the re s u lts. As the p ro jec t grow s and m a tu res and m o re of the c u rren t c a se s a re found and rep o rte d , w orthw hile com parisons w ill be forthcom ing. F u rth e r, as the recognition of the need of such studies cry stallizes, other and different approaches m ay becom e feasible.
