The Lube C inspection report was not received when issued. 4. The Gas Recovery area inspection under the heading "Safety Items" are 607 complete. 5. Shutoff valves are being estimated for the Marine docks. 6. A revised permanent order was issued in Disp. Department, 2/22/74^outlining detailed procedures to be followed for taking pressure storage vessel out of service. 7. The current procedure used for drawing water from tanks in Dispatching is to limit the operation to a maximum of two storage tanks in the same general area. The railroad easement fire ant hills are being chemically treated as a part of the Refinery overall pest control program. 10. Management has been working with the Pasadena Chamber of Commerce in an effort to alleviate the traffic problems on highway 225. The State Highway Department is expediting the work in the congested area at Beltway 8 and the Tenneco Plant entrance. 11. In-line filters are available for use on all compressors used for sandblasting and/or respiratory protection. Further the compressors have temperature alarms on the air and lube oil systems. Periodic carbon monoxide test will continue to be made in order to preclude CO\exposure. week. 12. The dock sprinkler system repairs are scheduled to begin next s' 13. As you are aware, the Safety Department is fully staffed. 14. The resource materials available to the dockmen (Shell ChemCard Transportation Guide Sheets, Material Safety Data Sheets & the U.S.C.G. Data Guide for Bulk Shipment) plus the issuance of the appropriate ChemCard for all shipments and special instructions under "Remarks" on the load ing order are adequate to alert the dockmen as to health & safety precautions. Further, the monthly safety meetings, as a rule, will be denoted in part to materials handling safety,i.e., the required use of respiratory protection, etc. 15. An exiisting safety shower at the east truck loading rack is in close proximity of the filter area. 16. The reported leaking diesel filter at the loading rack has been repaired. 17. When it is necessary to use automotive equipment on or over the Refinery railroad tracks, the expressed approval must be given by the rail road operations foreman. The person in charge of the work must provide a flagman or other warning devices located in such a manner so as to alert approaching trains in time to come to a safe stop. 18. A study is being made to determine what action might be taken to resolve the problem concerning multi-track railroad crossings and sided railroad cars in the TFCH area. 19. The linesmen have been instructed to keep lead pots out of manholes. 20. The stiles and walkways in pispatching Department will be re paired as weather permits. LAM 030067 ABS-027993 21. As requested the following areas in Gas Recovery were checked for air contaminates: H2S - V-675, 695 & 702, nil; P-4693 & 4694, 3 ppm; Hydrocarbon - V-682 & 684, nil, west fractionator pumpshed and sewer by V-1359, trace; Ammonia - D-3025, 3 ppm. Pump and blower repairs have been made in the area of V-1225 & 1226, CO & SO2 tests were negative. NEW BUSINESS 1. Safety statistics for April, 1974: 294 reported first aid cases, 15 doctor cases and 2 disabling injuries. The disabling injuries and doctor cases were discussed. 2. The union submitted the attached addendum for the January 31, 1924, Health & Safety Committee Meeting Minutes. 3. The benzene Air Monitoring & Medical surveillance programs are continuing. 4. To date, 750 employees have received audiograms. The following recommendations made in previousqj&eetings are restated for appropriate responses: a. A safety recommendation form (8^ x 11"), similar to those used by operations should be provided for use at all locations. b. The carpenter shop dust problem should be resolved. c. The present main shop overhead crane inspection program was discussed. A copy of the inspection form was requested. d. The cleaning of the plastic water kegs is still a problem. Possibly the use of galvanized kegs should be considered and the previous procedure of steam cleaning should be used. Dust flowing into the main gate lockers is still a problem. f. The respiratory equipment cleaning procedures in the tool room should be redefined. g. Plastic hard hats should be provided for all crafts. h. Safe driving should be enforced in the Refinery. Contractors and outside truck'draXters should be given printed rules when they enter the plant gates. i. The Refinery electric circuit identification program should be finalized. LAM 030068 ABS-027994 k j. The guy wire survey results should be reported. k. Plastic shoe covers or boots should be provided for craftsmen working in contaminated areas. l. Tool room clothing should be provided when work is required in sour service areas. A deodbrizer should be provided also. m. The Corporate Industrial Hygienist should monitor the Refinery | Laboratory Inspection Division.(NOTE: The Safety Department test procedures are outlined by the Head Office-Department of Health & Safety). n. The fume exhaust system should be installed in the garage. 6. The DHT compressor piping revisions should be made to preclude maintenance work in a confined space with respiratory equipment. p. Relocate the tool room grease barrel to the cleaning building & consider using disposable grease cartridges. q. All operating department trucks are not backing up in the main gate parking spaces. Signs should be considered for the cab of the process unit's trucks to alert the driver. r. The Refinery trucks should be provided with defrosters. s. Closed pressure relief headers should be considered for con taining all released materials. t. The Center Street "S" curve guard rails should be striped. u. Lights should be installed ,at the parking lots, passenger un loading areas & the East lot should be - resurfaced".. v. The amount of insulation stored in the shop should be minimized * for ease of cleaning the area. w. The main shop exhaust system should include the Cincinnati ^ "c-Utomatic" machine. toilet. x. The electric cable should be fenced at the mol sieve field y. The crane operators & painters should receive annual physical examinations. z. The Dubbs 9 cooler box area should be cleaned. I g, b.l The exhaust system in the cleaning building should be improved. c.l A break or lunch area should be provided in the Hydrocracker zone shop. d. l Employees who require first aid due to exposure to toxic materials should be examined by a physician. The following items were discussed: 1. A request was made to install rear handles for getting into the covered pickup trucks. 2. The crane operators making turns at the west zone shop are com pletely blinded by the building. 3. The grating should be inspected on top of V-365 at the LOFU. 4. The following deficiencies in the power plant No. 1 area were reported: a. The roofing should be repaired over the control panels & bur ners for boilers Nos. 7, 8, & 9. b. Water leaks on the air conditioner at the north wall of the 450-lb. boilers. The air conditioner electric junction box should also be covered. c. The cover is off the electric box over the 450-lb. master pres sure gauge. . d. The wall receptacle box cover is missing at the exhaust fan in the 450-lb. boiler house. e. The junction box covers are missing at the following locations: The 450-lb. boiler alarm panel, the 200-lb. boiler room door to the generators, south side of the 200-lb. boiler control room, by the ladder at No. 7 boiler, 2300-V box by the 200-lb. boiler area fountain, & the stair leading to the 200-lb. boiler chemical room. f. The 200-lb. boiler high voltage switch gear is not fenced on the second floor. g. The basement floor of the air compressor building is oily & the covers are off the lights. A junction box cover is also missing. UAM 03007 ABS-027996 h. The trench plates at No. 1 water ttreater are uneven. 5. The docks do not have material safety data sheets for ethylene dichloride. (Note: The books were checked & the data sheet is available). 6. The leaks in chemical plant lines in the pipe rock should be immediately repaired instead of roping off. 7. Commendation letters should be given to employees who made good safety suggestions. 8. The No. 1 power plant "boiler maintenance safety form" should be strictly supervised and completed before a fire and safety permit is requested. 9. Several craftsmen are reporting to process areas without required safety equipment. ^ 10. The last electrician safety meeting was reported to be lacking relevant topics for the craft. 11. It was reported that the insulators are not having safety meetings. 12. A separate toilet should be considered in hydroprocessing for female employees. 13. The AEC requirements were discussed for radiological work per formed in the Refinery. 14. Aisle markers should be considered in the machine shop. 15. Respiratory protection equipment should be located at the main oil separator. 16. A report was requested regarding the tank farm grass freeing program. Re cOntmfenidations made by various members of the committee. 5-1. Foreign ships with no English speaking crews should have some one as a standby on board who could relay messages at the loading/unloading manifolds. 5-2. The Refinery organizational structure of Safety/Security and the Hospital should be like the Shell Chemical Plant. 5-3. The majority of the Engineering Field safety meetings should be conducted in small groups in order to improve the response of the group. law 030071 ABS-027997 5-4. Aerial lifts Nos. 1 and 4 should be overhauled. Prior to the business meeting, the committee inspected Power Plant No. 2 and the water treater. The following items were noted: SAFETY ITEMS. 1. The railroad derail was not in place for protecting acid car ACFX-6858. The derail sign is also missing. 2. The platform guard chains are missing between boilers 2-5 and 2-6, and the platforms between 2-1 and 2-3 boilers. 3. An eye protection sign and face shield should be provided for the zone shop drill press. guard. 4. The boiler control room lab exhaust fan needs a ^-inch mesh 5. Repaint the fire extinguisher at the west end of the treater control room. 6. Remove the drum cover on the water treater control room roof. 7. Install ^-inch mesh guards on the water treater deaerator fans. 8. The eye wash fountain does not have sufficient pressure at acid pump* A-22. 9. The hose should be properly racked throughout the units. 10. The discharge piping on C077 should be insulated for personnel protection and the wire guard should be bolted in place. 11. A storage container should be provided for the respirators at the treater control room. 12. The walkways should be inspected and repaired between the water treater vessels. 13. The location of the Chemical Plant sandyard causes silica dust in the water treater area. 14. The acid car unloading piping should have an acid spill container provided with a drain. 15. The eye wash fountain should be repaired at the south end of the treater. LAM 030072 ABS-027998 8 16. The treater eye wash fountains need quick opening valves and the safety shower valves need to be inspected for ease of operation. 17. Condulet covers missing at V-941, V-942, V-943, and the acid 2. Repair the stop button on acid pump, A-22. 3. Repair or replace the conduit racks at acid pumps, A-22, 23, 24, and 25. office. 4. Place reciptical covers on the wall outlets in the zone shop 5. Junction box cover is missing behind control panel (650-lb. steam yard 2-3 and phenol unit). panel. 6. A junction box cover is missing behind the water treater control 7. The lights on 2-5 and 2-6 boilers do not have bulbs, covers, or reflectors. 8. The open conduit should be capped south of T-1778. 9. A guard should be installed on the agitator coupling on SS-326. 10. The electric conduit and switches should be inspected at the wooden walkway west of 397. 11. The water treater refrigerator is not grounded. LAM 030073 ABS-027999 i 9 12. Cover off the lights on the east and west side of 2-1 boiler and on the west side of 2-3 boiler. 13. The light switch cover is missing in the battery room for the turbogenerators. 14. The treater electric breakers should be better identified. 15. The operating procedures should be specified on the treater disconnects or breakers. 16. Should the acid pumps have covers at the treaters? 17. The switches in the treater area should be identified. 18. The panels just west of P1856 are rusting. 19. Should the generator battery be enclosed? s' 20. Secure the battery rack to the floor. 21. The junction box is rusted on the west end of the gas fired turbine. 22. The air conditioner is not grounded outside of the building north of the gas turbines. 23. The Calgon plastic tank has a loose wire on the motor and the brackets are rusting. 24. The lights on the deaerator need covers and reflectors. LEAKS AND SPILLS 1. Oil and chemicals on the water treater storage room floor. The miscellaneous instruments, fittings, etc., should be removed. HOUSEKEEPING 1. Clean the area between 2-5 and 2-6 boilers, and between 2rl and 2-3. 2. Trash located east of 2-4 boiler. 3. Broken sample bottle in the treater sample box. 4. Scrap in the acid tank area and the acid rack. LAM 030074 ABS-028000 10 5. Remove the aluminum cover in the ditch west of 2-1 boiler. .6 Remove the rusted drum S.W. of 2-4 boiler 7. Rags and cardboard boxes on the north side of P-1856. .8 The unit rest rooms should be cleaned. 9. panel board, panel board. Clean the area behind the boiler control room panel and treater Proper storage should be provided for the area behind the treater 10. Remove the scaffold boards at pipe stations 296 and 298. 11. The water treatex lab bench should be cleaned and also the ftdor. 12. Remove the scaffolding on the north side of 2-3 boiler. 13. The wooden scaffolding throughout the boiler area should be removed if the work is complete. 14. The building north of the gas turbines has rags and trash on the fibddr. 2-2. 15. Remove the old sign bracket at the walkway southwest of boiler MISCELLANEOUS 1. The 3-inch line needs a brace north of S-398. 2. The water leak should be repaired over the radio in the boiler control room. 3. The kitchen trash cpn needs a cover. 4. The large size slag around the gas fired turbines could cause turned ankles. 5. The south kitchen door should be repaired. .6. A container should be provided for di'&ty shop towels at the water !txeater. 7. The west door behind the :treaterpanel does not have a door knob. 8. The steam piping is rusted out at the column northwest of Pw^662. LAM 030075 ABS-028001 u 9. The upper level piping is hung with wire on the south end of boiler 2-3. 10. The ladder and platform are not bolted down on the west side of boiler 2t1 and the east end of 2-3.boiler. 11. The horizontal valves stems at grade should have covers. 12. The grating on the east side of 2-3 boiler should be inspected-- one step is loose. The grating is loose and uneven between 2-1 and 2-3 boilers. 13. Loose piping hanging in the walkway northwest of 2-1 boiler. 14. The cable on the 2-2 boiler stack should be inspected. 15. Inspect the grating west of 2-4 boiler. s' 16. The Chemical Plant incinerator causes pollution in the treater / power plant area. 17. The boiler control room lab should have bottle racks. I'fl' 18. The air conditioner intake should be relocated at the boilers house control room. The fumes from the incinerator are drawn into the building. 19. Should the lab room be in the locker room? 20. V-1233 pressure relief vents to the atmosphere. 21. The zone shop needs more material bins, also the material should be racked on the north side of the zone building. 22. The small pipe is a tripping hazard on the concrete west of 2-4 boiler. The numbers correspond to the items as shown on those minutes. NEW BUSINESS: 13. It was reported that switch crews have been instructed to have flag men at crossings, but due to shortage of personnel this has not been done. Crewmen report that the tracks are in poor condition and repairs are needed. s' The Lube Plant and "C" yard crossings are hazardous at times and have been a continuous source of complaints. Fire ants on the rail beds are a problem for the switchmen and must be exterminated. 16. Operators at Distilling #3 report that shift foreman tied in 110 V. power supply to new instruments. All electrical power sources should be handled by qualified Electricians and should be installed by code. 18. Revisions to the dust handling equipment in the Carpenter Shop has not eliminated the problem of a dusty atmosphere in which men are required to work. Further revisions or a new system is needed. 28. Compressors supplying breathing air to Painters and Sandblasters are not believed to meet OSHA requirements. Portable compressors that are leased as well as permanent installations should be provided with filters and alarm devices to meet those requirements. 46. Frequent checks by the Safety Department for H2S gas at the L.V.I. Unit should be made and operating personnel should be furnished monitoring devices until work on this sump has been completed. 49. A systematic inspection and evaluation of all hydraulic turbins used in the refinery should be instituted. 51. The Union Committee feels that stronger emphasis should be placed on ' the procedures for tagging circuit breakers when the equipment it operates is being worked on. Breakers should be locked during these times. 52. The Chemical Plant "G" flare is still a source of almost constant pollution. HCL is emitted into the atmosphere and under certain conditions this falls down in liquid droplets. The Health and Safety Committee feels this problem should be eliminated. LAM 030077 ABS-028003 58. Phenol vapors continue to be a problem at the Phenol Plant. Correc tive measures to eliminate this hazard have not been taken.
