However, it is recognized that in some cases, for various reasons, persons desiring to participate in the program cannot do this. For such individuals, with valid reasons for missing the van, the following procedure should be followed for make-up exams. 1. The persons missing exams should be sent to a local medical facility for the following tests: pulmonary function, EKG, audiometry, vision, tonometry, x-ray, blood pressure, height and weight. These results will be sent to Dr. Massad. 2. These persons will be scheduled for a hands-on exam with the rest of the plant personnel. At the time of the hands-on exam Dr. Massad will obtain blood, urine and stool specimens, and medical/occupational histories. 3. Through arrangements with Health Examinetics the samples obtained will be sent to Met-Path laboratory for analysis. The physical exam report and other results obtained at the local medical facility will then be sent to Examinetics. To assure computer compatibility of the test results obtained at the local medical facility it is necessary to know the testing techniques used at each facility. To determine this I need you to supply me with the name, telephone number and person to contact at the local medical facility you would most likely be using for the make-up exams. I need this information as soon as possible. The procedure outlined above may vary slightly depending on local needs and facilities.
