COMMENTS ON STATISTICS Trend lines showing significant changes were reviewed and were highlighted as follows: Department 53 Department 33 Utilities Department 10 Insulators Painters Chemical Contact Lift/Push/Pull 3. OLD BUSINESS (From the Minutes of the PMSB Meeting 10/19/7S) a. Emergency Procedure Manuals -- Glass The Emergency Procedure Manual material is better than 50% complete. Some portions have been received, but not yet typed. Target date appears to be about one week behind the schedule of November 15. b. Plant-Wide Alarm System Survey -- Triplett Triplett reported that plans are underway to correct the problems that have been identified in the various areas in the plant where the alarm system cannot be heard. An addi tional testing of the alarm system can be scheduled for .the December EPP test. c. Respiratory Protection Procedure on Fit Testing -- Ryan The SAC has spent a great deal of time in developing an improved respiratory protection procedure bulletin. It has been sent out for comments and 99% of these comments have been resolved. It is expected to be completed and ready for typing within the next week or ten days. 4. REPORT OF THE SAFETY ADVISOkY COMMITTEE (SAC) -- G. T. RYAN In addition to the respiratory protection procedure, the SAC has been working on and has approved for typing the injury and illness reporting procedure, P-Bulletin (P-0 ). This has been circulated for approval and is now ready for final typing. The SAC has discussed, but not yet resolved, the tank identification question raised by the Hourly Safety Committee. A comment will be made to Triplett so that he can make a report at the next Hourly Safety Committee meeting. 5. REPORT OF SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- TRIPLETT AND GLASS The SHAC committee received a memo from the PT Safety and House keeping Committee regarding an incident in which a person was nearly hit by a passing automobile in the crosswalk between Research I and Research II. The committee reviewed the incident SC 002738 STG 2402058 LAM003945 PMSB MINUTEJ 11/16/^S Page j and pointed out the need for increased surveillance and proper reporting of traffic violations. A SHAC action memo will be placed on the bulletin boards so all people throughout the plant will recognize traffic hazards and report traffic violations to the Sergeant of the Gaurds who is equipped to deal with those situations. Our experience has been that our traffic control has improved in recent months as a result of the action by the guards. We believe that if people throughout the plant know what to do that we will get a significant improvement. It was pointed out that the card handout to be given to truck drivers entering the plant by the guards at the gate would also improve the performance of driving in the plant. 6. REVIEW OF INCIDENTS 1. PMSB Minutes, 10/19/78 -- 8(f) - No. 2 Boiler Off Gas Line Fire--Fuller There are two off-gas headers in the Power 2 plant. The HCN off-gas header is out of service and has been blinded away from several of the boilers and will be blinded away from the remainder as soon as it can be scheduled. In regard to the incident that occurred, all operating procedure changes recommended in the incident report have been implemented. All maintenance repair orders that were issued as indicated in the report have been completed. All engineering work orders (EWO's required by the report have been written follow-up with plant engineering services--Grissom) will be implemented. 7. OTHER SIGNIFICANT EVENTS . These events were described and discussed in some detail. They involved an HCL spill, an HCL leak, a pump seal leakage, caustic being pumped into the street, and sewer damages. 8. CONTRACTOR SAFETY -- SCHWAB FOR MCCULLOUGH Leonard Schwab described the efforts being made to improve con tractor safety performance through increased responsiblity on the contractor and their supervision. They have inaugrated a new employee safety orientation program for contractor workers The safety awareness programs are being used among all the con tractors. The new methods and procedures for. investioating and correcting unsafe acts and conditions is working. The results so far have shown that as far as maintenance contract workers are concerned the total number of injuries is about the same as last year; however, the number of recordable injuries has dropped from 54 to 14. The RIR (Recordable Injury Rate) has dropped from 21 to 7. Another indicator of good contractor safety performance has been that there were no Opel employee injuries during the recent Methanol shutdown. The Opel Company has a new safety supervisor who is assisting and achieving this performance through his work in the field, his surviellance of SC 002739 STS 2402059 LAIVI003946 PMSB MINUTES 11/16/7^ Page the conditions underway in the plant, and pursuit of all the injury reports and follow-up on the corrective action. 9. OTHER ITEMS 1. The Family Safety Magazine that is sent to the homes of families at the site was approved to be continued for another year. There was general agreement by the members of the board that this was a worthy, worthwhile reinforcement of safety programs off-the-job and was consistent with our overall plans for improved safety awareness. 2. 1978 Safety and Property Protection Survey -- Glass This year's S&PP survey will focus on how our plant-wide safety committees, line organizations, and safety promotion teams carry out their safety responsibilites. We want to emphasize the teamwork we use to increase safety awareness and individual responsibility for good safety performance. A propsed agenda for the S&PP survey was reviewed by the SAC, approved in principle by Phil Brubaker and PT manage ment. A memo was circulated asking for the assignment of responsibility for the tour of each of the units, shops, and laboratory areas to be visited.
