It was agreed that the goals were very difficult but achievable with outstanding effort. Previous bicycle safety and handling history was reviewed. It was agreed that most items were not a safety concern and that SHAC should not be involved. H. Conrad will discuss with Tromblee and PMSB. Dr. Daniel Lanier - General Motors - is the contact in regard to the Drug & Alcohol Abuse program for GM. Two films are available. Latimer will contact him. Baca issued a questionaire regarding vessel blinding (copy attached) H. Conrad discussed the Dept. 53 spill at orthozylene incident (copy attached) . Conrad will issue a SHAC alert in April. NEW BUSINESS Proper ladder installations were discussed. Baca will issue a letter to contract supervisors detailing the proper procedures regarding ladders. OLD BUSINESS Copy of the SHAC Action items for 1983 was issued by Hammann (copy attached). 'Survey of N2/Air tie-in was discussed (copy attached) - will be discussed next week. Please complete the following questionnaire and return to Rufus Baca at SS-18 by March 25, 1983. Do you open any vessels in your area without blinding for non-entry inspect ions? If the answer is Yes, please attach the procedure useo. If your answer is No, do you feel that vessels can be opened safely for non-entry inspections without blinding? If so, state procedure that you would follow. ( Rufus Baca /Is SC 001221 LAM002421 SPILL IX DEPT. 53 INCIDENT The weak seam roof of an orthoxylene storage tank (53T"02) was split in a small section due to overfilling of the storage tank while unloading a barge on February 25, 1983. The resulting spill was contained in the diked area without external release. There were no personnel injuries.' CAUSE AND CONTRIBUTING FACTORS The cause of this incident was the failure of Material Handling personnel to check storage space available in the orthoxylene tank prior to the start of barge unloading operations. A contributing factor was the lack of a high level alarm or overflow device on the storage tank. RECOMMENDATIONS Material Handling personnel should emphasize the proper measurement calculations of storage space availability prior to beginning of unloading operations. RESPONSIBILITY: V. P. Osburn TIMING: Immediately All units should be made aware of this potential hazard and should assess storage tanks in and out of battery limits for proper high level monitoring. RESPONSIBILITY: SHAC TIMING: April, 1983 PLANT MANAGER'S COMMENTS Failure to recognize that there was insufficient storage space available in a tank prior to unloading a barge is the primary cause of this serious incident. If we are oblivious to something as obvious as insufficient space in a storage tank, what are our chances for finding those smaller, not so obvious things, which can also result in serious consequences? We must continually think about what we are doing. We must remain alert to our surroundings.
