REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G.W. Daues A. Bypassing Interlocks Following a discussion on the bypassing of interlocks and other safety devices. Gene Tromblee revised and restated his charge to the SAC Committee as follows: SAC to recommend a philosophy on the level of authority suggested for the bypassing of various types of interlocks. Superintendents should also begin to evaluate their interlock systems and designate those systems requiring superintendent's permission. B. Safety P-Bulletins In order to improve plant personnel's knowledge of plant Safety Procedure Bulletins, the following steps will be initiated: 1. Update and re-issue Safety P-Bulletin Index--J.A.G. 2. Review the backlog of Safety P-Bulletins awaiting issue and/or updating--J.A.G. 3. Staff to review and update their list of personnel holding the P-Bulletin Manuals. 4. Each department will begin a program of reviewing at least one Safety P-Bulletin at each monthly safety meeting. 5. Review of Safety P-Bulletins should continue as an evergreen-type program. 6. Gene Tromblee issued a mandatory requirement that all new issue Safety P-Bulletins and/or revisions should be discussed in the next scheduled safety meeting for each group. C. Other LP&S to obtain and distribute copies of all "Tec-Facts" to all General Superintendents. LP&S to also obtain copies of back issues and issue to the General Superintendents. McClellan A. 1980 Safety and Housekeeping Inspection Updated status report from each PMSB member should be sent to Glass. B. 1981 Safety and Housekeeping Inspection Will be held October 13, 14 and 15. All members should check the signs in their various areas to be sure that they are adequate and properly maintained. All pedestrian crosswalks will be repainted before the inspection. What is the status on the Ladder Inspection Program?--J.A.G. to discuss with G.L.T. C. Status of AVIP (SHAC)--J.J. McClellan Group 1--94% complete Group 2--97* complete Group 3--98% complete Danner's group--100% complete Navarrete's group--96% complete Fire Protection--100% D. Status of S-T-A-R-T Safety Program--J.J. McClellan Presentation of the slides on housekeeping is nearly finished. The program on procedures is approximately 50% complete. It was suggested that an additional program on procedures be scheduled for the last quarter. Additional publicity on the S-T-A-R-T Program should be implemented--LP&S. IV. STATUS OF LOSS INCIDENT REPORTS, JULY 1981--R.T. Hammann/J.E. Fox There are no delinquent nor outstanding loss incident reports. V. S&PP FOLLOW-UP--J .A. Glass/J.E. Fox Bob Hairanann is preparing an updated status report on prior S&PP inspections. The 1981 S&PP survey will be conducted August 31 thru September 4 by Edward J. Kilper of S&PP. Ryan Installation of the new breathing air equipment is approximately on sche dule and completion is expected in mid October. VII. Fransham) Report has not been issued. Acetic Acid Spill--Instrument man received minor irritation to face when instrument valve failed; Dept. 50 (R. Steber) Report has not been issued. Near Miss--Flange gasket blew out causing stream of hot aldehyde to cover oxo area; Dept. 48/56 (J. Gaviria) Man sustained laceration to wrist when wrench slipped while closing valve causing hand to strike another valve stem; Dept. 44 (L. Vickery) Report has not been issued. Recordable Injury--Operator sustained laceration requiring six stitches to lower arm when he cut it on insulation band; Dept. 48 (J. Gaviria) Flange fire resulting in two minor injuries; Dept. 44 (J. Stal) Description X-ray technician fell puncturing his buttocks; IDA (B.C. Lancaster) SC 003333 LAW1004347 PMSB Meeting Minutes - August 19, 1981 Page 5 Mr. Tromblee requested that all future investigation committees be instructed not to include suggestive disciplinary action in their recoirenendations. Such action is the responsibility of the concerned management and labor relations. VIII. PLANT MANAGER'S COMMENTS 1. Called attention to the arrival of the new plant ambulance. 2.
