McIntyre Monthly Safety Report March, 1980 Daw: 29 April 1980 Copies: D. I Hawes F. R. Saul J. C. Novak, Trexlertown Safety Education Topic 1. The March safety topic was "Tornados and Severe Thunderstorm" procedures. This topic was selected from the Safety Manual due to seasonal weather and revisions of the procedure. Posters throughout the plant and the main billboard reemphasized this safety theme. The safety meeting contained essential information for what to do in case of a tornado, whether at home or at work* 2. Two complete shifts were trained in OSHA 1910.1017 Vinyl Chloride standards. This included training and tests on OSHA Safety and Health Standards and VCM Health Hazards. The other two shifts are scheduled for April, 1980. 3. Twelve (12) new employees received safety orientations. 4. Conducted four (4) CPR recertification classes for the Fire and Rescue Squad for four (4) hours each. (See letter on meeting and attendance.) 5. Conducted two (2) PVOH operations safety meetings for four (4) hours each on a. Fire extinguishers (types and usage) b. Slips, trips and falls c. Self-contained breathing apparatus usage AP00045046 2 6. F. R. Saul attended Four Rivers Safety Council meeting. 7. D. I. Hawes, F. R. Saul and E. K. McIntyre attended training seminars on Handling Hazardous Material Emergencies at Welbourn Hospital, Evansville, Inc. on the weekend of March 22nd and 23rd. 8. D. I. Hawes obtained necessary materials needed for two (2) hose safety displays. A procedure, handout, and an agenda have been written on hose use and application to be used in the April safety meetings. 9. D. I. Hawes attended a Communications Workshop March 19-21, 1980. Hot Work Permits There were 83 Hot Work Permits approved by the Safety department. Projects 1. PVC had four (4) additional road blocks added to the production area and control room block. 2. All plant Scott Air Paks were audited in March with two (2) new ones being added: one in the gatehouse (T-2), and one in PVC Maintenance shop (C-16). 3. Inspected all fire systems with our fire insurance representative and found no outstanding deficiencies. Reviewed the previous report with J. J. Baliker, B. D. Helms, F. R. Saul and insurance engineer and all previous recommendations have been corrected or removed from the report with the exception of the requirements to place fire protection around the Emulsion ethylene storage tanks. 4. Hazards review was conducted in Waste Treatment new expansion plus three (3) new extinguishers were added to this area. (See Hazard review report.) 5. Made final tie-in of the LEL monitoring sensors to the automatic deluge sprinkler system at the monomer recovery structure improving plant fire protection. 6. Covered PVC shutdown during two (2) days, tlo incidents occurred. Security 1. Scrap Material Procedure approved by the Central Safety Committee was implemented on the 5th of March 1980 and was audited all four Thursdays of March. 2. New 02 system installed in ambulance in upright position providing better care and more room. 3. Pneolator has been removed from the ambulance and a resuscitator placed in service. This resuscitator requires less room and the 02 tanks are adaptable to our new 02 supply system. Committee Meetings 1. Safety training in March by plant breakdown is presented in detail in Table I. 2. Activities of area safety committees in March'are detailed in Table II. 3. The Central Safety Committee was held as scheduled during the month. Lost Time and OSHA Cases Calvert City experienced one (1) OSHA recordable case for the month of March, 1980. This case was charged to PVC and involved John Smith, Jr.'s wreck on his bicycle. (See report.)
