Iam over eighteen (18) years of age and otherwise competent to testify about the matters set forth in this Affidavit. 2. I am the manager of the library of the National Safety Council located at the headquarters ofthe National Safety Council at 1121 Spring Lake Drive, Itasca, Illinois 60143-3201 and am authorized to testify about the interpretation of the Annual Membership Review Form attached to this Affidavit on behalf of the National Safety Council. fI PLAINTIFFS L EXHIBIT flSL-qZ. II I 3. Attached to this affidavit is a true and accurate copy of an Annual Membership Review form issued by the National Safety Council to the Potomac Electric Power Company, also known as PEPCO. On the first page of the invoice, there is a box entitled "Affiliation Date." This box refers to the date the organizationjoined the National Safety Council. As reflected by the entry of"06" under the "Month" column and "36" under the "Year" column, PEPCO joined the National Safety Council in June, 1936. 4. As reflected on page 2 of the form, in connection with its membership PEPCO's Human Resources Health and Safety Department received Safety Materials Series A1, seven copies of the monthly publication "National Safety News", a subscription to the newsletter for the Constmction Section of the National Safety Council, and a subscription to the publication "Traffic Safety." Additionally, PEPCO's Medical Department received the newsletter for the Occupational Health and Nursing Section of the National Safety Council.
