Heinrich Fritz) with 14 illu stra tio n s In the age of technology, professional sicknesses are moying more and more into the focal point of in te re s t. Asbestosis, which is rare among them in relation to s ilic o s is , has-been fam iliar up to now only to a small circle of professional physicians, experts and pathologists. But rapid motorization and an increased demand for h eat-resistan t materials has led to an enormous in crease in the world production of asbestos (Merewetherl and to an expansion of the asbestos processing industry in Germany. Moreover, the great fluctuation of the German population a fte r 1945 can have caused the fact th at asbestosis emerged in places where i t had been unknown previously. This provides a reason for making reference to the profes sional disease of lung asbestosis CLA)., la s t but not lea st for the reason th at a series of papers has appeared concerning the complications of lung asbestosis which require examination ac cording to the experiences of the Dresden asbestosis experts. As the center of the asbestos processing industry, Saupe has made Dresden the center of clinical-roentgenological asbestos research. The classificatio n of LA into roentgenological degrees of seriousness I - I I I , which has been conventional in Germany, and the preliminary stage 0-1 goes back to Saupe, and up to now could not be replaced by a b etter one (Worth and S c h ille r). 2 The new international classificatio n of pneumokoniosis (Sidney 1950) and i t s modification (Worth) cannot be applied to lung asbestosis. The roentgenological symptomatology of asbestosis has been exhaustively described by Saupe, so that reference can be made to his studies. Also, one can only indicate the pathological anatomy of lung asbestosis. The widespread lite ra tu re was also summarized in the monography of Worth and S chiller. The diagnosis ''lung asbestosis" is presently supported on 2 facts: 1 on the x-ray picture and 2. on su fficien t dust exposure in the professional anamnesis. The frequently required keystone in the diagnosis structure: the proof of asbestos bodies in vivo, however, is not verifiable, since the formation of such "small bodies" represents no specific reaction of the lung to inhaled asbestos needles. Obviously, with the origin of these formations resembling clubs and dumb b ells, we are dealing with a general response of the body to needle-like and fib er-lik e dusts (Literature of Rtlttnerl. The x-ray diagnosis of lung asbestosis is naturally a l l the more d iffic u lt, the smaller the pathological changes are. Each complication makes the conditions more involved, and in the normal course of the disease, that is to say one spared of additional noxious elements, asbestosis shows a fullness of roentgenological accompanying phenomena, which are causally linked to lung fib ro sis. In the following, we w ill discuss the results in the evalua tion of asbestos films with regard to complications. They have been obtained from 343 cases of asbestosis over the course of 2 decades. 3 Reports have been made about these already in another connection. The cases consist of 130 women and 213 men. In stage 0-1 there were 42 women and 61 men a t the time of the f i r s t appraisal. In stage I there were 43 women and 74 men, in stage II 32 women and 61 men, in stage I I I 13 women and 17 men. In the case of 8 men, we were also able to find changes in the sense of a s ilic o s is . The average age of the patients with the f i r s t appraisal was 51. A' survey of the x-ray picture series repeatedly shows an extraordinarily changing picture: A p art of the typical asbestosis changes are covered over the lower fields with haze, circumscribed turbidity zones or uncharacteristic increase of lung signs with grown together peribronchial structures and the recognition of lung asbestosis on the individual picture is made more d iffic u lt by th is . The diagnosis is often made on the basis of the series. The most frequent complication--characterized by surface turbidity of the lower fie ld s--is doubtlessly the blackening of the pleura, which is found with different intensity with increasing degrees of seriousness of lung asbestosis--predominantly symmet r ic a l--in 49% of our cases. I t is recognized mostly by pleura accompanying seams of the side breast wall tapering toward the top Csee also Fig. 2). Asbestos bodies are to be proven path.anatomically in these cortexes (Behrens). However, one must also reckon with tuberculosis causes (see below). Relatively often, among our patients in 5% of a l l cases, we find bizarre shaped surface calcifications in the cortexes (Fig. l a ) . In the case of localization in the mediastinal or pericardial pleura, they are to be recognized only on supplemental pictures (Fig. l b ) . Typically, emphysema is most clearly expressed over the cranial lung parts with respect to the conventional behavior (Saupe e t a l l . I t is easy to overlook because the normal thorax x-ray picture exhibits a greater radiation permeability in the upper parts. Not infrequently, however, in addition to the compensatory emphysema of the upper field s, even in the direct vicinity of asbestos changes, that is to say in the side and lower parts, near the heart or epidiaphragm, i t comes to circumscribed emphysema formations between the fibrosis focal points (Beintker, Behrens e t al.)_, which outshine the present pleura blackening points or asbestosis lung structures CFig. 2) or simulate caverns. 5 The process of such "perifocal" emphysema formations is complex in nature CGough).. Strata examination or soft special photo graphs--in the case of women possibly with raised breasts--in most cases allow us to rule out fusings by the proof of typical lung sketches in the lig h ter or brighter p a rts . By means of x-ray, we were able to determine emphysema of the upper lung parts in 22% and perifocal emphysema areas in 16% of a l l cases among our p a tie n ts. As a resu lt of the asbestos dust inhalation, i t came to chronic bronchitis whose deforming effe ct on the bronchia was increased by the features of the shrinking callous lung parenchyma. The re su lt is bronchial widenings and stenosis caused by rupture of the axis. Evidently, these bronchus changes are responsible for the multiform nature of the x-ray picture series of lung asbestosis (Fig. 3a and b ) . The rapid change of the lung pattern must therefore be understood essentially as the resu lt of p eri bronchial and peribronchiectasic inflammations, which, according to the expression and time course can spread out to bronchial pneumonia or to chronic carnificating pneumonia. 8 In a sought out part of our patient of 69 cases, for whose selection alone the presence of year long observation series is determinant, we find such more or less rapidly changing findings of the lower or middle fie ld in 9 + 3.4%. By localization in the area of the asbestosis lung structures, they can largely put into question the recognition of the professional illn e ss on individual x-ray pictures. The more focal-like and surface shadows of such unspecific in filtra tio n s CFig. 4} can easily .be imposed as asbestos lung cancer, especially since in Germany the bronchial carcinoma is the most frequent complication of lung asbestosis Csee below). The lobe pneumonia, evidently occurring only seldom in the case of those having lung asbestosis, contributes to this p o ssib ility of deception, as one case shows (Fig. 5a and b) . Photographs of 10-13-1950 and 7-30-1953. 8a Fj,g. 8b 10 In the case of the frequency and the often expressed degree of seriousness of emphysema, i t is astonishing th a t the spontaneous pneumothorax appears to be very rare in the case of asbestosis. From the lite ra tu re , we know only one case of Wood and Gloyne. A spontaneous pneumothorax was observed in Dresden in the case of asbestos lung cancer with serious metastatic in f iltr a tio n of the non primarily attacked l e f t lung CFig. 6). Probably, the event of a spontaneous pneumothorax stands opposite to the formation of the pleura blackening in a hindering manner. The a r t i f i c i a l pneumothorax for influencing the tubercular process, has been used successfully on the other hand with slig h t asbestosis (Wedler). With the exception of lobar pneumonia and of the pneumo thorax, the enumerated complications are much more frequent in the case of lung asbestosis, than has been assumed up to the present time. Saupe has already made us aware of th is and ex pressly emphasized the unfavorable influence of these unspecific inflammatory changes on the fin al course of the disease. After reviewing the causes of death of 28 cases who died in the Dresden hospital with an average age of death of 54.4, there is no doubt th at circulatory changes caused the failu re of the right side of the heart CFig. 7) and the banal inflammatory lung complications bringing on the deaths of those a fflic te d with lung asbestosis. On the other hand, i t is astonishing with the serious results of these processes how often they were endured "in an ambulant sta te ". Occasionally, neither c lin ic a l or any astonishing symptoms or subjectively more serious troubles exist even with roentgenological impressive findings. 11 T*g. 9,.--H.Sfc. --Layer photograph of the right upper field , cavernous upper fie ld . TBC in the case of asbestosis I - I I , sputum +++. 8 cm: In contrast to the frequency of the ju s t lis te d complications for which i t is hard to find arguments in the direction of local differences, since th eir cause lie s in the nature of the disease, th eir stepmother treatment exists in the lite ra tu re . However, this should in no case indicate th at we are dealing here with the most frequent complications of lung asbestosis. Tuberculosis ju stifia b ly assumes a special position under the inflammatory accompanying diseases of asbestosis. In the German lite ra tu re , active lung Tbc with asbestosis is considered very rare among the overall population, so that a t the present time in the effect of asbestos dust, one has seen a certain protec tion, but a t le a st a defensive increase against tuberculosis. 12 In particular, reference is made to the monography of Wedler, who discusses a l l the lite ra tu re in d e ta il. Up to now, useable material has not existed from Germany. Only the case communicated by Leicher of an abdominal tumor in the case of asbestosis exhibits an active lung-tuberculosis (with tubercular pleura blackness!). The Dresden patients include 15 with active tuberculosis (Fig. 8, see also Fig. 2) which are distributed as follows in the astestosis stages: 0-1:3, 1:6, 11:4, 111:2. 5 of the patients are women and 10 are men (4.7%). 5 patients exhibit caverns with positive sputum findings (Fig. 9). 3 cases could be confirmed by dissection. With respect to the entire number of patients, 4.4% of active tuberculosis is present, which are distributed evenly over the report time according to time occurrence. The value of 1.6% of those with active tuberculosis relativ e to asbestosis, determined by Wedler for Germany, lie s far below the Dresden figure. The difference between the individual cases C8.2%X and the value of 7% given by Wedler for the German asbestosis is less noticeable for the frequency of inactive lung tuberculosis. I f however the 69 picture series are considered, the fraction of active tuberculosis cases climbs to almost 10+3.6%, that of the inactive to 11+3.7%. Calcified focal points or primary com plexes are not calculated a t the same time. They are shown in almost 22% of the cases. Very different frequencies are given in the world lite ra tu re for tuberculosis in the case of lung asbestosis. According to Wed ler, the values for America lie sim ilarly low as for Germany. 13 In Finnland, Wegelius 1947 found no single combination case of tuberculosis and lung asbestosis, while s t a tis tic s are available in England with frequency figures up to 34%. The figures communicated here for Dresden are therefore by far the highest. On the other hand, they ju stify no conclusions as to the causal relationship between asbestosis and tuberculosis, which could give cause for examining compensation guidelines valid in Germany up to now. Baader et a l. see the causes for the frequency differences of active tubercular patients with lung asbestosis in the individual industrial countries in regional differences. Perhaps th is also applies for the figures up to now. A higher frequency is given for Dresden by Reichert in 1941 than for the re s t of central Germany on the basis of local insurance s t a t i s t i c s . 14 There are discrepancies according to dissection s ta tis tic s with regard to tuberculosis frequency with lung asbestosis. Among 31 German dissection cases, Wedler indicated only 4 active cases of tuberculosis--that was 3.2%. In the meantime, the number of published dissection cases in Germany has increased by 4 CBoemke 3, Leicher 11 and 1 active tuberculosis, so th at 2 active cases of tuberculosis were given among 35 cases, a percentage of 5.7%. In the Dresden dissections of 9 cases, the 3 active cases of tuberculosis would make up 33%. With the Dresden dissections, a frequency of active tuberculosis with asbestosis disections of 11+4.7% would be given for a l l of Germany. This figure largely coincides with the tuberculosis frequency figures summarized in tables by Grosse in different dissection s t a ti s t i c s , whose values lay between 2.6 and 43.2%. Grosse gives a frequency of 11.6% for Dresden from 1928 to 1951. With the increase of the s ta tis tic a lly evaluated material i t would have to be determined th at those patients with asbestosis suffer no more or no less with tuberculosis than the to ta l population. All percentage figures with the thus rare lung asbestosis could naturally have comparative values. Even the dissection material or the dissection s ta tis tic s do not work selection-free, aside from "errors of small figures". The figures of Grosse can be used here with the present question for comparison only to a limited extent, since they consider only the lethal tuberculosis, not the less serious forms. On the other hand, the morbidity s t a ti s t i c s according to clinical-roentgenological c rite r ia have th e ir error sources. The question of the effective frequency 15 of tuberculosis in the case of asbestosis leads to the same d iffic u ltie s which are known from cancer s t a t i s t i c s . Thus with most cited complications of lung asbestosis, we arrived a t asbestos lung cancer. Inexact generalizations of i t s frequency in the dissection s t a tis tic s of 12-20% and an early recognition of the compensation obligation of lung cancer in the case of asbestosis have brought asbestos lung cancer to a not completely ju s tifie d popularity. According to Wedler, Hol stein e t a l, i t represents the most frequent complication of lung asbestosis. On the basis of a s t a t i s t i c a l comparison of the Dresden cases with the world lite ra tu re , the authors believe they have verified that lung cancer among asbestos workers unjustifiably has the reputation of great frequency. The lung cancer expecta tion of asbestos workers can not be substantially larger than th at of the overall population. The expectation appears to be some what higher only for women endangered by asbestosis dust than for th eir fellow females and matches th a t of the men. Also some other special features pronounced about profes sional cancer of asbestos workers appear to be no longer tenable. Nevertheless, further clear indications of causal relationships of the origin of cancer exist with asbestos dust inhalation (JBohlig and Jacob} . The most convincing argument for causality, namely the frequent lower lobe attack of asbestos lung cancer CFig. 10) is also at the same time the only roentgenological important criterio n . 16 In contrast to the bronchial carcinoma on the whole, which prefers the upper lobe about 2:1, the lung cancer of asbestos workers shows an occurrence a t the s ite of the most severe asbestos changes, the lower lobe, in 74% of the cases known up to the present time. Moreover, an accumulation of serosa tumors is noticeable among asbestos cancer. Of 31 histologically detailed cases of world lite ra tu re , 4 were pleura cancer and one was a diaphragm tumor. The course and prognosis of asbestos cancer show no deviations from other lung cancer types. The metastasizing mode follows the pulmonal type of Walther. The skeletal metastases are not to be distinguished roentgenologically from other destruction processes. Special studies like a tomogram and a well-aimed bronchus diagnosis are to be used in assuring the cancer diagnosis with suspicion of lung cancer. A clear warning must be made against the reflex -lik e rush diagnosis "Tumor like shadow in the case of asbestosis--ergo asbestos lung cancer". Finally, le t i t be noted in the case of mediastinum shadow th at no noteworthy roentgenological changes are to be found in the case of lung asbestosis except for a dissemination of lung roots and in advanced cases a blurring of the heart contours by pleuropericardial blackening (see also Fig. l b ) , la s t but not le a st because the lung asbestosis leaves the lymph system of the lungs i n itia lly out of participation (Di Biasi, Wedler). Heart enlargements are observed in different cases in the case of lung asbestosis (Lanza et a l . ) . Detailed studies dq not ex ist to a large extent. In correlation with Dreesen, et a l . , no significant differences could be found between the Dresden patients and 17 healthy people in th is regard. Reliable conclusions concerning the heart size are hardly to be obtained from a thorax photograph. Aus diesem Grunde besteht Veranlassung, auf die Berufserkrankung der Lungenasbestosr (LA) hinzuweisen -- nicht zuletzt, weil besonders ber die Kom plikationen der LA im Schrifttum eine Reihe von Auffassungen vertreten werden, die nach den Erfahrungen der Dresdner AsbestoseGutachter einer berprfung bedrfen. Dresden als ein Schwerpunkt der asbest-verarbeitenden Industrie ist durch S a u p e frhzeitig auch zu einem Zentrum der klinisch-rntgenologischen Asbestoseforschung geworden. Die in Deutschland bliche Einteilung der LA in die rntgenologischen Schweregrade I --I I I und (Ins Vorstadium 0--I geht auf S a u p e zurck und h at bisher nicht durch eine bessere ersetzt werden knnen (W o rth und S c h ille r). Die neue internationale Einteilung der Pneumokoniosen (Sidnev 1950) bzw. ihre Modifikationen (W o rth ) sind auf die LA nicht anwendbar. Die rntgenologische Symptomatologie der Asbestose ist von S a u p e erschpfend geschildert worden, so da auf seine Arbeiten verwiesen wird. Auch auf die pathologische Anatomie der LA kann hier nur andeutungsweise eingegangen werden. Die gesamte, im Schrifttum z. T. weit verstreute L iteratur wurde soeben in der Monographie von W o r th und S c h ille r zusammen. getragen. Die Diagnose ,,Lungenasbestose" sttzt sich z. Z. auf 2 Tatsachen: 1. auf das Rntgenbild und 2. auf eine hinreichende Staubexposition in der Berufsanamnese. Der hutig geforderte Schlustein im Diagnosengefge: der Nachweis von Asbestosekrperchen in vivo, ist jedoch nicht beweisend, da die Bildung solcher ,,Krperchen" keine spezifische Reaktion der Lunge auf inhalierte Asbestnadeln dafstellt: offenbar handelt es sich bei der Entstehung dieser keulcnund hantelhnlichen Gebilde um eine generelle Antwort cles Krpers auf nadel- und faserartige Stube (Lit. bei R ttn e r ) . Die Rntgendiagnostik der LA wird naturgem immer schwieriger, je geringer die patlmlogischen Vernderungen sind. Jede Komplikation m acht die Verhltnisse unbersichtlicher, und gerade die Asbestose zeigt schon im normalen --- d. h. von zustzlichen Noxen verschonten K rankheitsverlauf eine Flle rntgenologischer Begleiterscheinungen, die kausal an die Lungen fibrose gekoppelt sind. Im folgenden sollen Ergebnisse bei der Auswertung von Asbestosefilmen hinsichtlich der Kom plikationen besprochen werden. Sie sind an einem K rankengut von 343 Asbestosen aus gut 2 Jahrzehnten gewonnen worden, ber das in anderem Zusammenhang bereits berichtet worden ist. Die Flle setzen sich aus 130 Frauen und 213 Mnnern zusammen. Im Stadium''0--I haben sich zum Zeitpunkt der ersten Begutachtung 42 Frauen und 61 M n n e r befunden, im Stadium 1 43 Frauen und 74 Mnner, im Stadium I I 32 Frauen und 61 Mnner, im Stadium I I I 13 Frauen und 17 Mnner. Bei S Mnnern konnten zustzlich Vernderungen im Sinne einer Silikose festgestellt werden. Das Durchschnittsalter der K ranken bei der Erstbegutachtung h at 51 Jahre betragen. Die Durchsicht von Rntgenbildserien zeigt immer wieder ein auerordentlich wechselndes Bild: Durch Schleierungen, umschriebene Trbungszonen oder uncharakteristische Vermehrung der Lungenzeichnung m it verwaschenen peribronchialen Strukturen wird ein Teil der typisch asbestotisehen Vernderungen ber den Unterfeldern verdeckt und die Erkennung der LA auf dem E in z e lb ild dadurch erschwert. Die Diagnose ist oft erst auf Grund der Serie zu stellen. Die hufigste Komplikation -- durch flchonhafte Trbung der Unterfelder gekennzeichnet ist zweifellos die P l e u r a v e r s c h w a r t u n g , die in unterschiedlicher Strke m it zunehmendem Schweregrad der LA immer hufiger -- berwiegend symmetrisch -- in fast 49 '0 unserer Fllt gefunden wird. Sie ist meist auch durch nach oben sich verjngende Pleurabevlcitsume di-r seitlichen Brustw and erkenntlich (s. auch Abb. 2). Path.-anatom isch sind in diesen Schwarten Asbestosekrperchen nachzuweisen (B e h re n s); es mu jedoch auch mit tuberkulsen Ursachen gerechnet werden (s. u.). Bei Lokalisation in der uinliastmalen oder perikardialen Pleura sind sie z. T. nuidauf hrteren Zusatzaufnahm en zu 'ikimnen (Abb. lb ). Doppelseitiges Auftreten von Pleurakalk sollte immer Veranlassung geben, hu Berufsanamnese zu erfragen. >/ Auch die Asbestose neigt wie die Silikose zur E m p h y s e m b ild u n g . In typischer Weise i-t 'ins Emphysem entgegen dem gewohnten Verhalten zunchst ber den kranialen Lungenteilen tmausgeprgtesten (S a u p e u. a.). Es wird leicht bersehen, weil schon das normale Thorax- 'ilgenbild in den oberen Partien eine grere Strahlendurchlssigkeit aufweist. S Durchaus nicht selten kom m t es jedoch auer dem kompensatorischen Em physem der lilinfclder auch in unm ittelbarer Nachbarschaft der asbestotischen Vernderungen, d. i. in den 'Iliehen Untergeschossen, in Herznhe oder epidiaphragnial, zu umschriebenen Emphyseml'iMiuigon zwischen den Fibroseherden ( B e in tk e r , B e h re n s u. a.), die vorhandene Pleurariuvurten oder asbestotische Lungenstruktui'en hinwegleuchten lassen (Abb. 2) bzw. Kavernen 'tuschen. Der Vorgang solcher ,,perifokaler" Emphysembildungen ist komplexer N atur [ diiugh). Schichtuntersuehung oder weiche Spezialaufnahmen -- bei Frauen evtl, m it angehoi ,,iii,n Brsten -- werden in den meisten Fllen den Ausschlu von Einsehmelzungen durch i 'vidi weis typischer Lungenzeichnung in den aufgehellten P artien zulassen. Rntgenologisch lassen sich in unserem Krankengut Emphysem der oberen Lungenpartien j ii 22% und perifokale Emphysembezirke in 16% aller Flle feststellen. Bereits S a u p e h at hierauf aufmerksam gemacht und auch ausdrcklich 'len ungnstigen Einflu dieser unspezifischen entzndlichen Vernderungen auf den Endablauf , ikr Erkrankung hervorgehoben. Nach Durchsicht der Todesursachen der vom Dresdner K ranken- siiit verstorbenen 28 Flle m it einem durchschnittlichen Sterbealter von 54,4 Jahren bestehen Afine Zweifel, da Kreislaufstrungen m it Versagen des rechten Herzens (Abb. 7) und die dmnulen entzndlichen Lungenkomplikationen die Haupttodesursachen der Asbestosekranken - Iigoben. Andererseits ist es bei dem folgenschweren Charakter dieser Prozesse erstaunlich, in oft sic ,,am bulant" berstanden werden: Auch bei rntgenologisch eindrucksvollen Befunden 1s . (s. auch Abb. 4) bestehen gelegentlich klinisch weder irgendwelche aufflligen Symptome noch | .subjektiv ernstere Beschwerden. 3 Im Gegensatz zur Hufigkeit der eben aufgefhrten Komplikationen, fr die sich schwerlich j Hwelche Argumente in Richtung rtlicher Verschiedenheiten finden lassen werden, da ihre Ursache in der N atur der Erkrankung gelegen ist, steht ihre stiefmtterliche Behandlung in der | Literatur. Dies d arf jedoch keinesfalls darber hinwegtuschen, da wir es hier tatschlich mit I 'len hufigsten Komplikationen der LA zu tu n haben. Unter den entzndlichen Begleitkrankheiten der Asbestose nimmt die T u b e r k u lo s e m it % vcht eine Sonderstellung ein. J a c o b uncdl H. B o h lig Rfo 83, 4 man z. T. in der Asbeststaub einwirkung sogar einen gewis sen Schutz, zumindest aber eine Abwehrerhhung gegen die Tuberkulose gesehen hat. Im einzelnen sei auf die Monographie von W e d le r verwie sen, die die gesamte Literatur ausfhrlich bespricht. Neues verwertbares Material liegt u. W. aus Deutschland bisher nicht vor. Lediglich der von L ei e h e r mitgeteilte Fall von Bauchfelltumor bei Asbestose weist gleichzeitig auch eine aktive Lungen-Tuberkulose (mit t u b e r k u l s e r Pleura. schwarte!) auf. : Im Dresdner Krankengut sind 15 aktive Tuberkulosen (Abb. Bezogen au f das gesamte K rankengut sind also 4,4% aktiver Tuberkulosen vorhtui- : den, die sieh dem zeitlichen Auftreten nach gleichmig auf die Beric-htszeit verteilen. Der von W e d le r fr Deutschland erm ittelte, au f die Asbestose bezogene W ert von 1,6% aktiver Tuber- , kulosen liegt erheblich unter den Dresdner Zahlen. F r die Hufigkeit der i n a k t i v e n Lungen-Tbc bei Asbestose ist die Differenz zwischen d e n eigenen Fllen (8,2%) und dem von W e d le r fr die deutschen Asbestosen angegebenen Wert von 7% weniger auffllig. W erden jedoch die 69 Bildserien allein betrachtet, so erhht sich der Anteil der aktiven Tuberkulosen au f fast 10+3,6% , der der inaktiven auf 11+3,7% . Verkalkte Herde bzw. Primrkomplexe sind dabei nicht mitgerechnet worden; sie zeigten sich in fast 22% der Flle. Im W eltschrifttum sind fr die Tbc bei LA sehr unterschiedliche Hufigkeiten angegeben. Die W erte liegen z. B. fr Amerika nach W e d le r hnlich niedrig wie fr Deutschland; in Finn land h at W e g e liu s 1947 keinen einzigen Kom binationsfall von Tbc und LA gefunden, whrend in England Statistiken m it Hufigkeitszahlen bis zu 34% vorhanden sind. Die hier fr Dresden m itgeteilten W erte sind also bei weitem nicht die hchsten. Andererseits rechtfertigen sie keim : Schlufolgerungen au f urschliche Koppelung zwischen Asbestose und Tuberkulose, die di. bisher in Deutschland gltigen Entschdigungsrichtlinien zu revidieren Veranlassung geben knnten. B o h lig : Die rntgenologischen Komplikationen der Lungenasbestose Rfo 83,4 An H and eines statistischen Vergleiches der Dresdner Flle m it dem W eltschrifttum glauben Verff. jedoch berzeugend dargetan zu haben, da der Lungenkrebs unter den Asbestarbeitcm sehr zu Unrecht den R uf einer groen Hufigkeit besitzt. Die Lungenkrebserwartung d Asbestarbeiter kann danach nicht wesentlich grer sein als die der Gesamtbevlkerung; lediglich fr die asbeststaubgefhrdeten Frauen scheint die Erw artung etwas hher zu liegen als fr ilm Geschlechtsgenossinnen und sich der der Mnner anzugleichen. Auch einige andere dem Berufskrebs der Asbestarbeiter nachgesagte Besonderheiten Schemen sich nicht lnger aufrecht erhalten zu lassen. Trotzdem bestehen weiter deutliche Hinweise auf urschliche Zusammenhnge der K rebsentstehung m it der A sbeststaubinhalation (B ohlig mul Jacob). ' V W - f '-v' / : Das schwerwiegendste Argument fr die K ausalitt, nmlich der hufige Unterlappenbcfall des Asbestlungenkrebses (Abb. 10), ist auch gleichzeitig das einzig rntgenologisch wichtige Kriterium . Im Gegensatz zum Bronchialkarzinom berhaupt, das den Oberlappen etwa 2:1 bevorzugt, zeigt der Lungenkrebs der Asbestarbeiter ein A uftreten am Ort der strksten asbcslu- tischen Vernderungen, dem Unterlappen, in 74% der bisher bekannt gewordenen Flle (Verff.). Auerdem ist eine Hufung von Serosatumoren unter den sbestkrebsen auffllig. Von 31 liisfu. logisch detaillierten Fllen der W eltliteratur sind 4 Pleurakrebse und einer ein Bauchfelltumur ; gewesen. ' , : Verlauf und Prognose des Asbestkrebses zeigen keine Abweichungen von anderen Lungen krebsarten. Der Metastasierungsmodus folgt dem Pulm onalistyp W a lth e r s . Die Skelett- : m etastasen sind rntgenologisch nicht von anderen Destruktionsprozessen zu unterscheiden. Zu sicherung der Krebsdiagnoseisind die auch sonst bei Lungenkrebsverdacht angezeigten Spezial Untersuchungen wie Tomographie und gezielte Bronchusdiagnostik einzusetzen. Vor der rcllci , artigen Anhiebsdiagnose: ,,Tumorartige Verschattung bei Asbestose -- ergo Asbestlungcnknl*' (s. o.) mu nachdrcklich gewarnt werden. Zusam m enfassung Die hufigsten Komplikationen der Lungenasbestose (LA) sind auer Pleuravorschwartungcn und koiii) satorischem Emphysem die unspezifischen Entzndungen peribronchialor, poribronehiektatischer und bi-ond.. pneumonischer Art. Besonders letztere sind differentialdiagnostiseh vom Asbestlungenkrebs abzugrenm Hierbei wird auf die Bedeutung der Bildserie hingewiesen. Eine aktive Lungentuberkulose als BeglcitkmnM* der LA hat sich im Dresdner Krankengut von 343 Fllen in 4,4% gefunden; dies ist ein wesentlich liiilm . Prozentsatz, als bisher fr Deutschland angenommen worden ist. Der Asbestlungenkrebs ist unter den Aul-, arbeiten! nicht hufiger als das Bronohial-Ca in der Gesamtbevlkerung. Une tuberculose pulmonaire active a ri-ii<-(mtre en tant que maladie accomiiagnant l 'asbestose pulmonaire dans le 4,4% des 343 cas.de l'ensemble nmlailes de Dresde; ce chiffre est notablement plus lev que le % admis gnralement en'Allemagne. Le uio-r nsbestogne pulmonaire n'est pas plus frquent chez les ouvriers travaillant l'amiante que le cancer 'toin-hirjuo pour l'ensemble de la population. Il ne diffre radiologiquement du cancer bronchique d'tiologie liHV-ii-iito que par lo fait que les plages infrieures sont relativement plus souvent atteintes. Radiolgicamente se distingue de los carcinomas de~pulmn de otra gnesis solamente por su Waliaun relativamente ms frecuente a nivel del lbulo inferior.
