Calgary Energy, LLC employee, Connor Haag was present during the inspection. Landowner, JT Hough was present during the inspection. This was an EPA follow-up inspection in response to an initial spill inspection (NRC No. 1425812) conducted 3/11/2025. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no other federal, state, or local agency representative present at the time of the inspection. FACILITY DESCRIPTION EPA conducted a follow-up inspection at the Calgary Energy, LLC, Tidal Osage 1 Lease, where a 3-inch steel flowline ruptured releasing a company estimated 350 barrels of saltwater and 2.5 barrel of crude oil onto the ground. The spilled fluid migrated south into an unnamed tributary, then west eventually impacting Little Hominy Creek. Section II - OBSERVATIONS I observed the release point for the spill. At the time of the inspection the release point had been repaired through installation of a new 3" poly flowline. See photo No. 1. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report. I observed a Total Dissolved Solids (TDS) field reading of 77,688 mg/L at the point of entry (POE) into the unnamed tributary of Little Hominy Creek. This was the highest TDS observation of the inspection. See photo No.2. In the main channel of the impacted tributary, at the confluence with Little Hominy Creek, I observed a TDS field reading of 2,960 mg/L. See photo No. 3. In the main channel of Little Hominy Creek, immediately downstream from the confluence with the impacted, unnamed tributary, I observed a TDS field reading of 342.6 mg/L. See photo No. 4. Approximately 1 mile downstream from the POE into Little Hominy Creek, I observed floating oil trapped in organic debris on the surface of the water. See photo No. 5. Section III - AREAS OF CONCERN 2 Calgary Energy, LLC / Tidal Osage 1 Lease CWA Inspection Date 03/24/2025 The primary areas of concern included the elevated TDS waters in the unnamed tributary of Little Hominy Creek as well as the oil floating on Little Hominy Creek. Calgary Energy, LLC's representative, Connor Haag, was present during the inspection and noted the GPS location of each area where contamination should be removed from the impacted waterways. Section IV - FOLLOW UP EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre spill" condition.
