These requests came as a result of confusion on how and to whom the permits are to be Issued. The variation over the past month of allowing the Issuance of permits to equipment has proved undesirable. Therefore, in the meeting of March 14, the Board clarified the method of Issue as follows: A. _The permit will be issued to the craft performing the work and it shall cover a specific Job to be done. B. The permit (carbon copy) shall be attached to the equipment or In the immediate vicinity of the work by the operating personnel in the area. C. On permits Invalidated by a fire or spill emergency, work may be resumed by the chief operator or operat ing supervisor countersigning the permit as soon as he has determined that the work can be resumed safely. On permits invalidated for other reasons (such as, changes in leadmen, expiration of permit period) new permits, marked "Extension" must be issued for continua tion of work on the same day (G:CO A.M. to 8:00 A.M.). > These changes will be Incorporated In a revision of the procedure bulletin_(0117) on fire and entry permits. ' Remember--the permit form does not, nor can It, substitute for a proper individual work performance. The safety of the Job depends on the proper preparation of the equipment, the proper inspection of the work to be done and the proper performance of work by the craft.
