Per your request, enclosed is an Application for Active Membership in the Institute.
Friction material manufacturers in the United States and Canada who are manufacturing from basic raw materials and are producing parts in commercial quantities are eligible for active membership in the Institute. Complete eligibility requirements for membership are spelled out in our constitution.

ANNUAL FEE FORMULA
The annual fee is based on four categories in which the Member participates: (1) Disc Brake linings; (2) Drum brake linings; (3) Brake Blocks; (4) Clutch Facings. The fee is calculated with a basic charge of $1,800, with an additional category charge of $250 for each of the first two categories in which a member participates, with an additional category charge of $500 for each of the next two categories. Essentially, this comes to:

COPYRIGHTS/TRADEMARKS As noted in our EXHIBIT A write-up, the FMSI Numbers and Numbering Systems are protected by copyright, and FMS and FMSI are registered trademarks. Use of these part numbers and/or trademarks without benefit of Membership for friction material manufacturers or express written permission of the Institute for customers of Members, is a violation of our copyrights and infringes on our trademarks. We believe a central gathering point for product information has been good for our industry and a common numbering system is a benefit to all. APPLICATION FOR MEMBERSHIP If you were to apply for Active Membership, you should complete the "Application for Membership" Form enclosed. As regards the question on "financial strength", copies of financial statements, or bank references, or trade references such as domestic customers/suppliers would suffice. The application should be signed by a principal of the applicant - Officer, Sales Manager, or someone in similar capacity. The application should be accompanied by a prepayment of the first year's annual fee. Assuming the application were accepted, this deposit would be applied to the fee for 12 months from date of acceptance. It normally takes about 6 to 8 weeks to process the application for Active Membership. If the application were not accepted, the prepayment would be returned. FRICTION MATERIALS STANDARDS INSTITUTE
