INSTRUCTIONS FOR COMPLETING THE GEORGIA BIENNIAL HAZARDOUS WASTE REPORT WHICH FORMS TO COMPLETE Form A - Identification requests identification information and must be completed by everyone. Form B - Self-Generated Hazardous Waste and its Disposition must be completed by the following: Hazardous Waste Handlers that generated over 2200 pounds of hazardous waste in any month in 1985 and/or had waste in storage at the beginning of 1985. NOTE: If your status is Small Quantity Generator, and no regulated quantity of hazardous waste was generated or on hand in any month of 1985, so indicate by entering zeros on Line 5 of Form B. Form C - Hazardous Waste Received From Off-Site Handlers and its Disposition must be completed by the following: TSD Facilities that receive hazardous waste from off-site handlers for treatment, storage, disposal, reshipment, use, reuse, recycling or reclamation. A separate Form C Report is required for each off-site handler from whom waste is received. Form D - Hazardous Waste Minimization must be completed by the following: liazardous Waste Handlers that generated over 2200 pounds of hazardous waste in any month in 1985 and/or had waste in storage at the beginning of 1985. It is very possible that you may need to complete a combination of these forms depending on your particular hazardous waste activities. For example: TSD Facilities that generate hazardous waste and receive waste from off-site handlers for treatment, storage, disposal, reshipoent, etc., would need to complete Form A, Form B for the hazardous waste they generated themselves, Form C for the hazardous waste they received from off-site handlers and Form D.
