(2 hr. 19 min.) While washing a weak caustic storage
tank to a recovery sump, the drain basin at the tank
plugged with solids and overflowed to an adjacent
storm runoff sewer which discharges downstream of our
pH neutralization system. A change in operating
procedures has been implemented to have the retention*
gate in the storm sewer closed during washing out of area tanks.

(1 hr. 8 min.)-Misinterpretation of instructions i . regarding the clearing of a caustic product storage tanl for essential maintenance resulted in caustic reaching Outfall 001 downstream of the pH neutralization system. To prevent future occurrences, the necessity of effective communication will be stressed in a group session with unit operators ip April.
