- 21 years - single Residence - 365 Cloverdale Avenue, Akron, Ohio since January 1930. Previously at 411 Carrol street'- not in factory district Complaint Inability to extend right hand on wrist, or fingers on hand. First noticed September 2, 1930. Numbness began in morning on rising to dress - tingling. Completely unable to use hand in extension. Worked through the day, using left hand. Tingling left after three days, and there has been no change since. The following week after illness began he had cramps with tendency to diarrhoea, weakness, fatigue. For a month prior had been tired and run down. Had lost weight (28 pounds) during month preceding. Lost 3 pounds during the attack of cramps. Industrial History Taught school - Connelsville, Pa. 1927. School State Cel. of Pa. - Col. Pa. summer 1928. Taught school till February 1929 - then got "flu" - in bed one week - came home then and has been here since February 22, 1929. Began on February 25, 1929, at filling station in Akron, where he sold gas, occasionally greased cars (some each day). Worked at station three months - no illness. . Then transferred to office until April 1930. In office was stock clerk handling books, reports and forms. No handling of gasoline or other lead compound. April 1, 1930 transferred back to filling station work (cor N. Main and York St). Very busy station. Dispensing gasoline and helping greasing, changing oil, etc. Approximately 60% Ethyl Gasoline sales (50,000 gallons per month). Nozzles on pumps leak almost uniformly. 8 hour day - sometimes 12, 15, 16 hours. Hands wet often, never washed except lunch time. Painted hoist, ladders and pits, August - 1 afternoon only. Clothes used at station laundered at home weekly - never more than two weeks ; clean shirt daily - shoes wet often with gasoline - no blisters or gasoline burns. Two months ago had abscess on foot - opened and drained - healed in few days. Previous illnesses Measles, whooping cough, chicken pox - no prior illness except "flu11 during teaching. Diet Fruits and vegetables - grape fruit - apples and grapes. 5 or 4 apples - a bunch of grapes occasionally. Peppers carrots, cabbage, celery. Milk every day - a quart. No alcohol wine once or twice in life - no grape juice - very little meat perhaps twice per week* 0003559 PHYSICAL EXAM Temperature 99.2 - Pulse 84 - Blood Pressure 110-60 Eyes - React well to light and accommodation. No paresis, strabismus, or other abnormalities. Nose - Dark red mucous membrane, somewhat dry and swollen. No obstruction but small resp. canals - turbinates prominent. Throat - Red, especially in tonsillar fossae. Anterior pillars red. Throat irritable. Tonsils injected not scarred - no attempt made to express pus. Teeth - General bright red ring of gingivitis, especially inside on molars. Teeth proper in fair condition. No suppuration of gross type. No lead line at any point. (No history of lead line at any time) Chest - Normal. Slight decrease in resonance on left upper region, but no adventitious sounds of abnormal type. Well muscled with no atrophy. Muscles not especial ly reactive to mechanical stimulation. A rash proba bly due to ingestion of iodides prescribed by doctor present on back and front of chest. Abdomen Legs Negative. No distension. No tenderness. muscled. Normal K g 00C3560 Well . - 3- The right arm shows atrophy of striking degree 'involving deltoid slightly, biceps greatly, and especially marked in extensor region of forearm. The musculature is soft and unresponsive to light pinching. The triceps react to per cussion hammer, perhaps a little more readily than the left. There is a distinct coldness to the arm, and the nail beds are cyanotic (some on both sides but more pronounced on right). There is also a distinct pallor of the skin of the right arm. A complete wrist drop is present, though a little ability to supinate remains. The hand cannot be lifted on the wrist, nor can the fingers be extended. Vihen the fingers are tested carefully with the hand supine, there is the slightest resistance to an enforced flexion of the fingers. There is no suggestion of resistance to flexion of the wrist. The flexor muscles are strong, and the adductor of the thumb is strong, hone of the flexor muscles are involved. There is complete normality of tactile and dis criminative sensibility over the entire right hand and arm, and elseivhere on the body. Painful sensibility is likewise normal.
