I. Summary of Incident
Frank Scherer's VCM for 9/17/81 was 12.1ppm. Exposure traced to cracked nipple on reactor #2 below the pressure gauge.

III. Detailed Description of Incident
F. Scherer, Semi-Pilot Plant Operator was working 4-12 shift on Thursday, September 17, 1981. He performed no unusual tasks. He filled 2 VCM "bombs" for the next days charges. He was work ing in an area adjacent to reactor #2. No respiratory equipment was used by Mr. Scherer as no VCM alarm was observed by him. Computer printouts for 9/17/81 indicate that the Semi-Pilot area
was under alarm status for more than 2h hours.
IV. Investigation
A. Primary Cause: Cracked nipple on reactor #2 pressure gauge line.
B. Contributing Cause: Cracked nipple leak was not readily de tectable.
V. Recommendations
A. What is to be done: 1. Repair or replace nipple on reactor #2 PG line - Done 9/18/81--G. Martin, R. Swedberg.
