Hamperl) Cancerogenous Effect of Sisbestos in Implantation in Rats+ By Dietrich Schmlhl With 3 text illustrations (Received August 19, 1958) The causal connection between "asbestosis11 and the appearance of lung cancer among humans is today considered to be proven (Lynch and Smith; Nordmann; Wedler, Hueper, Weiss; Bohlig and Jabob ). According to Nordmann about 20-/0 of all persons suffering from asbestosis later on have lung cancer. The latent period is supposed to be about 15 to 20 years. There are only few experimental efforts concerning any cancerogenous effect of asbestos. Nordmann ana Sorge in 1941 reported on experiments with mice which were exposed to as bestos dust. Below we shall briefly report on the results of these experiments which were begun in 1953. Methodology The experiments included a total of 39 rats of both sexes from our pure stock BD IV. At start of the experiment they were about 100 aays old. Food consisted of a mixture of kitchen waste, fortified with vitamins, proteins, fats, and salts. Water was given ad libitum. The animals were observed until their spontaneous health. After thorough section there took place a histological examination in case of suspicious findings. We implanted the asbestos in two different ways. One in volved pure asbestos fibres, like those used for filtering pur poses, and the other mineral asbestos. Initially the asbestos? fibers were loosened and cut to a length of 0.5 - 1 cm. In 21 rats 150 mg each of these asbestos fibers 'were implanted subcu taneously and intrapcritoneously. In the second experimental group mineral asbest of the same dosage was implanted in 18 animals. Tnis type of asbestos is fragile and it was implanted in small, long pieces of about 0.2 cm length. Re suits The implantation was tolerated by the animals without acute damage. Only in 2 cases there developed after the implanting of the asbestos fibers fistuals at the abdominal operation scar, but they healed with sulfonamide powder treatment. Tne medium life expectancy of the animals in the first series was 900 days and in the second 720 days. within the normal range. Thus it was roughly 3 22 months after the implantation of the fibrous asbestos there occurred in one rat a swelling which emanated from the implantation located within the network. The histological examination produced a spindle cell sarcoma which was in some areas mucuous, and forming fibers. Subcutaneous grown spindle cell sarcoma 27 months after the implan*ation of fibrous asbestos. sarcoma, which had _ . ,, &r o m around the intraperional implantation, with mtastass in the pericard and in the pleura, and further, a spindle cell sarcoma, emanating from the network, with mtastass in the liver and spleen, in the kidneys and axillary lymph glands, and finally 3 further spindle cell sarcomas which had arisen around the subcutaneous implantation (111. 1) and had not metastasized. In another rat we observed in the stomach cavity a nucuous fibroma, which had grown together with the stomach wall 111. 2. With 3 rats the swellings arose around the intraperitonal implantation, and with the 3 others in the subcutis at the implantation point. It appears especially interesting that the sarcomas which had come about, had without exception metastasized.', but not the ones grown subcutaneously. The latent period up to the appearance of tumors, after implantation amounted to 37 - 4 months, but it was so long that the swellings occurred only at an advanced age among the animals. ill. 3 Asbestos fibers enclosedin hyalineous binding tissue (a ); it in turn surrounded by a granulation tissue (u) 'with penetrating tumor tissue (T) - o - V/e had similar results with a second experimental group, as in the case of the animals who had received an implant of mineral asbestos. The first swelling in this instance came already lb months after implantation in the stomach area. This was a case of a hemorrhagic tumor, probably emanating from the pancreas v/hich had metastasized in the thymus and in the side kidneys. Histologically the swelling turned out to be a large-cell sarcoma. Among the 12 rats still living at this time, another 4 developed spindle cell sarcomas, wxiich had all originated locally, ana specifically with 2 animals around the intraperitonal and with 2 animals around the subcutaneous implant. Here, too, the implanted asbestos could be proven in the tumor. The yield of 5 malignant tumors among lo sur viving rats is practically the same as in the experimental series with the asbestos fibers, and equally the latent period of 21 - months. Frequently it was possible to prove the asbestos fibers in the midst of the tumor tissue. Histologically this provided in each instance fundamentally the same picture, regardless whether it involved fibrous asbestos or mineral asbestos; in larger deposits one finds a central hollow space which is filled by crumb-like masses of asoestos (ill. 2 Z). The granulation tissue surrounding it encloses to'ward the periphery progressively smaller foci of asbestos (ill. 2 A), until in the end it involves only individual thread-like asbestos fibers wxiich have been broken apart. In the farthest centrally located segments, the granulation tissue is poorer in vascular terms and richer in fibers (ill. 2 G). The asbestos fibers are then located in 7 individual "bundles in the low-core hyalinous binding tissue (ill. 3 A). Further toward the periphery, the granulation tissue becomes richer in cells (ill. 2, 3 Q), and it also contains num erous multi-core giant cells around the asbestos fibers. Only outside this zone there begins then the area of the swelling-type cell growth (ill. 2, 3 T). Discussion The results of the tests at hand show that in rats asbestos leads to the development of tumors after intraperitonal and subcutaneous implantation. This once again proves that a sub stance which produces cancer in humans also brings about the same effects in experiments with animals. The effect mechanism of cancer production of such implant ations is still unclear. In his investigations concerning the creation of tumors through foils of artificial substances and metals, Nothaurft came to the conclusion that it largely depends on the form and magnitude of the implanted material. Foilltype implants produced the largest yields of tumors in his experiments, whereas the same material in the form of powder or fibers pro duced virtually no cancer. Probably in these cases the origin of tumors can be based on an unspecific, mechanical effect of the implants, whereas their chemical nature appears not to play a determining role, insofar as the material cannot be metabolised within the organism. However, in some cases chemical effects may also come under consideration, for example with metals. In the meantime Druckrey, Hamperl, and Schm&hl were able to show thatafter intraperitonal injection me tallies mercury can likewise 8 lead to sarcomas, although it is formless and is divided into small and very small droplets. Thus the unspecific mechanical effect recedes in this instance. The present tests show that asbestos can cause cancer also when implanted in the form of fibers and small crumbs. In the case of human asbestosis, lung cancer is caused even by the finest asbestos dust, which had also led to lung cancer in mice in the tests of Aordmann and Sorge. On the other hand, my own comparative tests, where other silicates, such as augite and tremolites, were implanted in the same form of small crumbs in rats, led to tumors in only a very few cases. Accordingly, the creation of tumors in these experi ments with asbestos cannot be based on a solely mechanical and thus unspecific effect, but appears to contain, after all, yet other factors. Also remarkable are the purely local relationships between asbestos and tumor, which allow the drawing of certain conclusions about the hystogenesis of the tumor in~question. Around the location of the maximum "concentration1 of the asbestos fibers, i.e. the center of the implantation, we only find a hyaline binding tissue capping, which evidently represents the remainders of the earliest reaction of the organism. Toward the periphery, where the asbestos has been blown apart into individual needles and threads, it is possible to prove a reaction rich in cells, fresh or still under way, with a transition to sarcomatose swelling growth which is almost unnoticeable at man;.y points. August 1958) Der kausale Zusammenhang zwischen ,,Asbestose" und dem Auftreten v0a Lungenkrebs beim Menschen gilt heute als erwiesen {Lynch und smith; N oedmann; Wed leb ; H u e p e e ; W e is s ; B ohlig un d J acob). Sach N ordmann bekommen etwa 20% aller Menschen, die an einer Dbestose leiden, spter Lungenkrebs. Die Latenzzeit soll etwa 15 bis Jahre betragen. Experimentelle Arbeiten ber eine caneerogene Wirkung von Asbest legen nur wenige vor. N oedmann und Sorge berichteten 1941 ber Versuche an Musen, die Asbeststaub ausgesetzt worden waren. Etwa *?o der Tiere erkrankten an Lungenkrebs. Im Gegensatz zu diesem }*itiven Ergebnis verliefen Untersuchungen von I I uepee (1955) an Ratten, denen Asbest in das Femurmark implantiert worden war, nega tiv. Keines der Versuchstiere entwickelte einen Tumor. Wir haben im Verlauf unserer Untersuchungen ber die Krebserzeuungdurch makromolekulare Substanzen auch den Asbest geprft. ber die Ergebnisse dieser Versuche, die 1953 begonnen wurden, soll im folgenden kurz berichtet werden. Methodik Die Versuche umfaten insgesamt 39 Ratten beiderlei Geschlechts unseres caien Stammes B D IV . Bei Yersuebsbeginn waren sie etwa 100 Tage alt. Die Whrung bestand aus einer Mischkost von Kchenabfllen, die m it Vitaminen, Uoteinen, F etten und Salzen angereichert waren. Wasser wurde ad libitum ge sehen. Die Tiere wurden bis zum Spontantod beobachtet. Nach grndlicher vktion erfolgte bei verdchtigen Befunden eine histologische Untersuchung. Den Asbest haben wir in zwei verschiedenen Formen implantiert. Einmal handelte es sich um reine Asbestfasern, wie sie zu Filterzwecken verwendet werden, und zum anderen um mineralischen Asbest. Die Asbestfasem wurden zunchst aufgelockert und auf eine Lnge von 0,5-- 1 cm geschnitten. 21 R atten erhielten P 1-50 mg dieser Asbestfasern subcutan und intraperitoneal implantiert. Ein hnliches Ergebnis erhielten wir in der zweiten Versuchsgrupi*.-. als bei den Tieren, die den mineralischen Asbest implantiert bekommen hatten. Die erste Geschwulst entstand hier bereits 15 Monate nach der Implantation im Bauchraum. Es handelte sich um einen, wahrschein lich vom Pankreas ausgehenden, hmorrhagischen Tumor, der in de Thymus uncl in die Nebennieren metastasiert hatte. Histologisch er wies sieb die Geschwulst- als ein grozelliges Sarkom. Von den in diesem Zeitpunkt noch lebenden 12 Ratten entwickelten noch 4 weitere Spindel* zellsarkome, die smtlich lokal entstanden waren, uncl zwar bei 2 "Heren um das intraperitoneale und bei 2 Tieren um das subcutane Implant** herum. Hufig gelang es, die Asbest-de-ern inmitten von Tumorgewebe naelizuweisen. Histologisch ergab sieh dabei immer grundstzlich das selbe Bild, gleichgltig, ob es sieh um faserigen Asbest oder minerali schen Asbest- handelte: in greren Depots findet sieh ein zentraler gohlraum, der von brckligen Asbestmassen erfllt ist (Abb. 2Z). Das hn umgebende Granulationsgewebe schliet gegen die Peripherie zu immer Meiner werdende Herde von Asbest ein (Abb. 2.4), bis es sieh schlielich nurmehr um einzelne, auseinandergesprengte fadenfrmige Asbestfasern handelt, ln den am weitesten zentral gelegenen Abschnitten ist das Granulationsgewebe gefrmer und faserreiclicr (Abb. 2 G). Die Asbestfasern liegen dann in einzelnen Bscheln in kernarmem hyalinem Bindegewebe eingebettet (Abb. 3 A). Weiter gegen die Peripherie zu wird das Granulationsgewebe zellreicher (Abb. 2, 3 G), enthlt vueh ein zelne vielkernige Riesenzellen um die Asbestfasern. Erst auerhalb dieser Zone beginnt dann das Gebiet der geschwulstmigen Zeihvucherung (Abb. 2, 3 T). Diskussion Die Ergebnisse, der vorliegenden Untersuchungen zeigen, da Asbest hei der R atte nach intraperitonealer und subcutaner Implantation zur Entwicklung von Tumoren fhrt. Damit ergibt sich erneut, da eine Substanz, die beim Menschen Krebs erzeugt, auch im Experiment am Tier die gleichen Wirkungen hervorruft. Der Wirkungsinechanismus der Krebserzeugiing solcher Implantate : ist noch unklar. Xothdeft kam bei seinen Untersuchungen ber die S' Erzeugung von Tumoren durch Folien von Kunststoffen und Metallen zu dem Schlu, da sie weitgehend von der Form und Gre des im plantierten Materials abhngt. Folienirmige Implantate ergaben bei seinen Versuchen die grten Tumorausbeuten, whrend das gleiche Material in Form von Pulver oder Fasern praktisch keinen Krebs er zeugte. In diesen Fllen kann die Entstehung von Tumoren wahr scheinlich auf einer unspezifischen, mechanischen Wirkung der Im plantate beruhen, whrend ihre chemische X atur keine magebliche Polle zu spielen scheint, sofern das Material im Organismus nicht ab- gebaut werden kann. Es kommen aber in manchen Fllen auch chemische Wirkungen in Frage, z. B. hei Metallen. Dreckrey, Hamperl, und Schmhl konnten inzwischen z; .yn, da metallisches Quecksilber nach bitraperitonealer Injektion ebenfalls zu Sarkomen fhrt, obwohl es formlos ist und in kleine und kleinste Trpfchen verteilt wird.
