We met with Scot Foltz, Regional Compliance Manager, and Marci Choate, supervisor of the system's Fort Worth office. The inspection began at approximately 12:30 p.m. and concluded at approximately 3:30 p.m. on February 24, 2026. This inspection also served as on-the-job training (OJT) for an EPA staff member pursuing the drinking water credential. During the opening conference, Mimi Phillips presented her EPA identification and informed the system representatives that the purpose of the visit was to evaluate the system under the SDWA and applicable federal and state regulations. The information in this report is based on review of documents, conversation with the system representatives, requested documents, and observations made during the inspection including: x Review of PWS records including daily-logs and operator credentials. x Evaluation of monitoring reports and monitoring plans. x Field inspection of water treatment plants (WTPs) and pump stations: for each of the active treatment plants we visited the following elements: o Water Wells o Chlorine Room o Ground Storage Tanks o Service Pumps o Pressure Tanks o A backup generator at Plant#1 Photos taken during the inspection are included in Appendix 2, Photograph Log. FACILITY DESCRIPTION The Foxbane Combine WS (PWS ID: TX 0610164) is a community water system owned and operated by Aqua Texas, Inc., located in Justin, Denton County, Texas. The system serves approximately 1,455 people through 485 service connections, based on Texas Drinking Water Viewer data. The public water system obtains water from six active groundwater wells on a single pressure plane. The Trinity Aquifer is the water source for the system's wells located within the plants. The system is organized into four water plants, each with specific wells, onsite chlorination, ground storage tanks, service pumps that send water to the hydropneumatic (pressurized) tanks, and then to the distribution system. Each plant has an ZZZZZ 2 Zdistribution system (Appendix 3, system schematic). Foxbane Combine WS PWS ID No. TX0610164 Inspection Date: 02/24/2026 There are four plants (Appendix 3, system schematic). x Plant 1 (Foxbane Plant): Houses Well 1 (G0610164A), rated at 18 gallons per minute (GPM), and Well 3 (G0610164C), rated at 160 GPM (combined 178 GPM). Disinfection is provided via sodium hypochlorite injection prior to the ground storage tanks. The plant has two ground storage tanks, each with a capacity of 33,000 gallons. Water is pumped from storage by service pumps rated at 200 GPM to a 3,500-gallon hydropneumatic (pressurized) tank, which maintains pressure throughout the distribution system. x Plant 2 (Double Tree Plant): Houses Well 6 (G0610164F) rated at 9 GPM. This plant has been out of service since February 18, 2026, and was not available for inspection. When in service, disinfection is via liquid sodium hypochlorite. A 15,200-gallon storage tank feeds 150-200 GPM service pumps, then a pressurized (hydropneumatic) tank with a capacity of 3,000 gallons, and then to distribution. x Plant 3 (Sage Meadows Plant): Houses Well 7 (G0610164G) rated at 70 GPM. Utilizes gas chlorination prior to storage (Appendix 2, Photograph 1). This plant has two ground storage tanks, each with a capacity of 33,000 gallons, which feed 200-300 GPM service pumps, then to a pressurized (hydropneumatic) tank with a capacity of 10,000 gallons, and then to distribution. x Plant 4 (Riggs Place Plant): Houses Wells 4 (G0610164D) rated at 55 GPM and 5 (G0610164E) rated at 50 GPM. Disinfection is provided by liquid sodium hypochlorite prior to storage. This plant has one storage tank with a capacity of 21,000 gallons, which feeds 100 GPM service pumps, then a pressurized (hydropneumatic) tank with a capacity of 3,000 gallons, and then to distribution. Interconnection According to the system representative, the interconnection of the four plants ensures service continuity. When one plant is offline, there is no service disruption to the water provided to the customers. During planned or unplanned outages, the two emergency interconnections located at plant 3 (Sage Meadows) and plant 2 (Double Tree) help to maintain service (Appendix 4, System Emergency Interconnection Diagram). Section II - OBSERVATIONS Operation and Staffing x The inspection team inspected the Foxbane water system's water wells, chlorine rooms, ground storage tanks, service pumps, pressure tanks, and the backup generator at Plant 1 (Foxbane Plant) x The team observed locked gates with signage indicating Aqua ownership, contact information for customers to use was also on display, and minimum Personal Protective Equipment (PPE) requirements at the entrances to Plant 1, Plant 3, and Plant 4 (Appendix 2, Photographs 2-4). 3 Foxbane Combine WS PWS ID No. TX0610164 Inspection Date: 02/24/2026 Monitoring x Monthly bacteriological samples are sent to the Texas health department laboratory. x Public Notification (PN) is provided to customers via Reverse 911 alerts, direct mail, or email. x The system has one primary operator for the Foxbane Combined system. x Emergency callouts are addressed immediately; non-emergencies are scheduled. x Ten staff are available for emergency response and address issue quickly such as leaks. Supervisory Control and Data Acquisition (SCADA) According to the system representative, the SCADA system is operated by a third party and provides remote monitoring only. There are no on-site alarms; however, staff receive text-message alerts when issues arise, such as when the free chlorine residual at an active well falls below the system's target of 0.8 mg/L. Water Wells The team observed one active water well at Plant 1 (Foxbane Plant), one water well at Plant 3 (Sage Meadows), and two water wells at Plant 4 (Riggs Place). Offline Well: Plant 1 (Foxbane Plant): The site schematic indicates two active wells--Well 1 (G0610164A) and Well 3 (G0610164C) (Appendix 3). However, during the inspection, the team observed only Well 1 (G0610164A). Further inquiry with the system representative revealed that Well 3 (G0610164C) is located near Plant 1, but is offline. Chlorine Room x Chlorine residuals are measured daily, with a target free chlorine residual of 0.8 mg/L for active plants and an overall company goal of 1.5 mg/L. x Chlorine storage duration does not exceed 30 days; monitoring is performed by a contract lab. Service Pumps x According to system representatives, service pumps are programmed to operate on an alternating lead/lag basis. x Service pumps were observed at the three operating plants with the following capacities: Plant 1--200 GPM; Plant 3--200-300 GPM; Plant 4--100 GPM. Ground Storage Tanks Two tanks, each with a capacity of 33,000 gallons, were observed at Plant 1; two tanks, each with a capacity of 33,000 gallons at Plant 3; and a 21,000-gallon tank was observed at Plant 4 (Appendix 2, Photographs 5-7). Pressure Tanks The inspection team observed three hydropneumatic (pressure) tanks with the following capacities: Plant 1--3,500 gallons; Plant 3--10,000 gallons; Plant 4--3,000 gallons (Appendix 2, Photographs 8-10). 4 Foxbane Combine WS PWS ID No. TX0610164 Inspection Date: 02/24/2026 Backup Generator The system has an emergency backup generator located at Plant 1, which is tested monthly by a contractor (Appendix 2, Photograph 11). Operational Capital x According to Mr. Foltz, the system utilizes project capital (reserved for plant projects) and corrective capital (for repairs such as leaks). x During emergencies--e.g., low-pressure events--the system issues pressure notices to customers, typically via email or web postings. Requires facilities and equipments to be kept in good working condition. Protective devices (e.g., vents, screens, caps, seals/grommets) must be intact and maintained to prevent contamination. 30 TAC 290.41(c) (Groundwater wells--construction and protection). Wellheads must have an atmospheric casing vent that is downturned and fitted with corrosion-resistant insect/rodent screening -mesh). The vent must terminate where it cannot be flooded and must prevent entry of surface water or other contaminants. The wellhead must maintain sanitary integrity with a tight sanitary cap/seal and all openings properly protected. Area of Concern 1. Plant 1 (Foxbane Plant): Air vent part of the water well 1 (G0610164A) was broken/ disconnected (Appendix 2, Photograph 12). 30 TAC 290.43 (Water storage): Storage facilities and their appurtenances (including inlet/outlet/connecting piping) must be designed and maintained to be watertight and protected from contamination. Leaks must be corrected to preserve sanitary integrity. Area of Concern 2. Plant 1 (Foxbane Plant): deteriorating seal creating gaps around the wellhead Appendix 2, Photograph 13). Area of Concern . Plant 4 (Riggs Place Plant): Leaking of the check valve or pressure release valve of the pressure tank (Appendix 2, Photograph 14). Area of Concern . Plant 3 (Sage Meadows Plant): unlabeled chemical bottle in the treatment room (Appendix 2, Photograph 16). Area of Concern . Plant 3 (Sage Meadows Plant): No chlorine gas monitoring sensor is installed to detect leaks. Scot Foltz stated that management plans to eliminate the chlorine gas feed; however, a timeline for when this will occur was not provided (Appendix 2, Photograph 17). 30 TAC 290.46(m) (Maintenance and housekeeping): Public water systems must keep all facilities and equipment in good working and presentable condition. The grounds and facilities must be maintained to minimize the presence of rodents, insects, and other disease vectors, and to prevent any conditions that could contaminate the water. Area of Concern . Plant 1 (Foxbane Plant): pressure gauge indicated 16 PSI in the treatment room. It is unclear if the pressure gauge was connected to a water pressure tank in distribution (Appendix 2, Photograph 18). Area of Concern . Plant 1 (Foxbane Plant): No concrete pads to prevent overflow around the ground storage tanks which can lead to erosion potential damaging to tank foundation (Appendix 2, Photograph 19). Area of Concern . Plant 1 (Foxbane Plant): The support underneath pressure tank has severe rusting (Appendix 2, Photograph 20). Area of Concern . Plant 1 (Foxbane Plant): Severe rusting around well head (Appendix 2, Photograph 21). Area of Concern 1. The team verbally reviewed observations and areas of concern with system representatives. Section IV - FOLLOW UP x Following the onsite closing conference, EPA received the system schematic (Appendix 3) from the system representatives on February 27, 2026. x An additional document request was sent on March 5, 2026, to obtain updated information on the Plant 1 (Foxbane Plant) water well (G0610164C). EPA received a message on 3-17-26 stating that the water well has been offline.
