Section I - INTRODUCTION
PURPOSE OF THE INSPECTION
EPA Region 6 Inspector, Matthew Allen, arrived at the Calgary Energy, LLC / Tidal Osage 1 Lease (EPA ID No. OKU000893) at 10:00 AM on 03/11/2025 for an unannounced inspection. Calgary Energy, LLC employees, Toby Jones, Mick Raab, Blaine Haag and Connor Haag were present for portions of the inspection. This was an EPA inspection in response to a landowner complaint as well as a National Response Center Spill Report (NRC No. 1425812) which was received by EPA Region 6. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no other federal, state, or local agency representative present at the time of the inspection.
FACILITY DESCRIPTION
EPA conducted an inspection at the Calgary Energy, LLC, Tidal Osage 1 Lease, where a 3-inch steel flowline ruptured releasing a company estimated 350 barrels of saltwater and 2.5 barrel of crude oil onto the ground. The spilled fluid migrated south into an unnamed tributary, then west eventually impacting Little Hominy Creek.
Section II - OBSERVATIONS
I observed the Release Point of the spill which was a 3-inch steel flowline which ruptured due to corrosion. See photo No. 1. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report.
I observed the overland flowpath of the spill. Beginning at the release point, spill impacts migrated southeast approximately 50 feet along a drainage feature, toward an unnamed tributary of Little Hominy Creek. See photo No. 2.
I observed the Point of Entry into the unnamed tributary. At this same location I observed a TDS field reading of 106,363 mg/L, this was the highest TDS level observed during the inspection. At this same location, I also observed crude oil floating in the main channel of the impacted tributary. See photos No. 3 & 4.
On the west side of county road 5305, I observed oil which had passed through the cement culvert under the road. I also observed the unnamed tributary continuing toward the west, which was fully covered with oil at the time of the inspection. See photos No. 5 & 6.
Below (south) the confluence of the impacted tributary and Little Hominy Creek, in the main channel of Little Hominy Creek, I observed a TDS field reading of 26,702 mg/L. This was the highest TDS level

observed in Little Hominy Creek during the inspection. See photo No. 7. This TDS observation and all others throughout the inspection were observed using the EPA issued, properly calibrated, YSI Pro 30 conductivity meter.
At a cement low water bridge crossing Little Hominy Creek, I observed a TDS field reading of 338.9 mg/L in the water overflowing the bridge. At the same location, I observed a floating, oil absorbent containment boom which had been placed to prevent further downstream migration of crude oil. See photos No. 8 & 9.
Immediately downstream from the low water bridge, I observed a TDS field reading of 584 mg/L. See photo No. 10.
From the west bank of Little Hominy Creek, I observed the confluence of the unnamed tributary and Little Hominy Creek. At the time of the inspection a floating oil absorbent containment boom was installed to capture crude oil coming into Little Hominy Creek. Oil was floating on the unnamed tributary and Little Hominy Creek at this location. See photo No. 11.
I observed the end of spill impacts, which was oil and oil sheen floating on the waters of Little Hominy Creek, this observation was approximately 1.55 miles downstream from the POE, if following the main channel of the creek. At the time of the inspection, there was no containment measures in place to prevent downstream migration of spill impacts. From the end of spill impacts, I observed the upstream and downstream view of Little Hominy Creek, capturing the bed and bank features of the waterway. See photos No. 12, 13 & 14.
I observed a "background" TDS field reading of 299.8 mg/L. This measurement was observed upstream from the POE in the main channel of Little Hominy Creek where spill impacts could not reach. See Photo No. 15.
Section III - AREAS OF CONCERN
EPA Region 6 inspector Matthew Allen conducted a field meeting with Calgary Energy, LLC representative, Blaine Haag, at the conclusion of the inspection. The primary area of concern was the unsecured spill impacts migrating down Little Hominy Creek.
Additional areas of concern included the ruptured flowline, brine/oil impacted soil and vegetation along the overland flowpath, the impacted waters and banks of the unnamed tributary and the oil/brine impacts in the main channel and along the banks of Little Hominy Creek.
Section IV - FOLLOW UP
EPA inspector scheduled a follow-up inspection for 11/13/2025.
