I also performed two x-ray of the chest, an electrocardiogram, laboratory studies, computerized vital capacity studies, and I am submitting the following report and opinion to you: HISTORY: The patient states he has worked for the above respondent for thirty-eight and one-half years as a hi-lo operator. He is exposed to dust and fumes, which irritated the chest. The patient has an occasional cough but has lost no time from work nor had medical treatment for any pulmonary condition. The p atient is presently working on the same job. His chief complaints at this time are: (1) Occasional cough (2) Pains in the low back. PAST HISTORY: Born and raised in the United States; married; three adult children; compensation for injuries to the low back, a right toe, right elbow and right shoulder; gastrectomy (twice) about eight years ago; does not smoke; drinks beer moderately; takes Tagamet and Iron Tabs. SYSTEMIC REVIEW: Gastro-intestinal: Respiratory: Cardio-vascular: Genito-urinary: Muscular skeletal: Nervous system: Appetite is good; no nausea or vomiting; bowels regular. See chief complaints; no shortness of breath;uses no pillow. No pre-cordial pain; no edema. No frequency, nocturia or dysuria. Maintains a constant weight. Height: Weight: Head: Eyes: Nose: Ears: Mouth: Neck: Chest: Cardio-vascular Abdomen: Extremities: 5 feet; 10 inches. 188 lbs. Scalp free and clear. Sclerosis of the retinal arterioles Septum in the midline; no obstruction. Drums visualized and are normal. Pharynx negative. Supple; no adenopathy. Thyroid not enlarged. Symmetrical; expansion is equal. Percussion note and breath sounds are equal throughout. No rales; no rhonchi. Blood pressure 140/76 P.M.I. diffuse; regular sinus rhythm. No thrills or murmurs. A2 is greater than P2 Soft. Liver, spleen and kidneys not felt. No hernia; no masses. No edema. LABORATORY STUDIES: Hemoglobin: 13.9 grams or 89%. VITAL CAPACITY STUDIES: See enclosed computerized vital capacity study. ELECTROCARDIOGRAM: 1. Regular sinus rhythm 2. Normal ECG. X RAY OF THE CHEST: (Taken on inspiration and expiration) The bony cage appears normal. The costo-phrenic sinuses are free and clear. The broncho-vascular markings are increased at both bases. Two X-rays of the chest, taken on inspiration and expiration reveal that the diaphragms are rounded with no evidence of emphysema. A computerized vital capacity study was performed and shows only a slight abnormality due to the bronchitis.
