CORCYCA POTENTIAL ASBESTOS EXPOSURE: SEQUENCE OF EVENTS ON 4/20/82 A valve operator mechanism on the block valve separating #1 and #2 adipic acid cooling towers (C/1) riser water was in need of repair. The valve operator sheared off the valve. To make the repair. Mechanical required the valve body to be exposed (see attachment). A 14 ft. hole was dug which required a vessel entry permit to be is sued. On 4/20/82 a work crew from adipic acid was sent to the excavation to ready the pit .or vessel entry (0800 hrs.). The crew consisted of two adipic acid maintenance men, three Zachry construction men, two laborers and one carpenter from Arthur Brothers, and one man from adipic acid production (to be used for standby). At the same time, Zachry insulators were directed to remove insulation from the pipe bridge between #1 and #2 C/1 (see attachment). DCB supervision authorized the insulation work with a permit requiring Industrial Hygiene. (I/H) Section 10 to be followed (Asbestos Handling). On the 20th, a Iight wind was blowing (5-10 mph) out of the southwest (plant). Between the two C/T's, the wind was noticeably stronger. This was probably due to a `funnel effect* caused by the two C/T's and wind direc tion. At 0900 hours, the Op. 2-3 Production Supervisor and Day Foreman were at the excavation to authorize the vessel entry permit. The insulators were in the pipe bridge already at work. No airborne dust was noticed at this t ime. e At 0945 hours a maintenance man at the pit noticed a light cloud of dust coming in the direction of the work. He evacuated everyone from the work area and called for the Day Foreman. When the foreman arrived, he saw no "SAFETY IS MY RESPONSIBILITY" DUP 0821249 airborne dust. The insulators had come down from the pipe bridge, cleaned the area, and were leaving the site. At approximately 1100 hours, the Day Foreman notified Medical that an employee under his supervision might have been exposed to asbestos (l/H: 10-0). At 1130 hours, the production employee requested to go to Medical. The Day Foreman gave his permission. About 45 minutes later Medical called the Day Foreman wanting all the names of the individuals at the excavation and to have them report to Medical. By 1300 hours, all personnel that were at the excavation had-been contacted and sent to Medical. DC8 supervision I had been notified of the situation.
