Kaldenberg Paul Siems Bubba Bean TO Glen Vogtman Frank Bales Charles Loney Rusty Wilson MON-1979 The safety inspection for the Materials Handling area was conducted by Don Kaldenberg, Paul Siems, Bubba Bean and Bob Hammann on 1/23/84. Overall the department was found to be in satisfactory condition. Comments on the inspection follow. Please answer these recommendations in writing to G. W. Bostick with a carbon copy to Steve Moncla by 2/21/84. General Comments It is recommended that drums located west of Dock 2 (determined to contain napthalene) be labeled immediately after filling. A question was raised on the nitrogen hose stations at Dock 1. After some deli beration, it was determined that each station should comply with the plant proce dure instead of having a common block and bleed valve for the purpose of determining the operability of the check valve. Need to establish a procedure with the CO2 supplier for checking flexible hoses used in unloading tank trucks. Personal Knowledge of Safety 1. The safety procedure book contained two out-of-date procedures, P-0105 and P-0132. The latest procedure bulletin on bicycle safety, P-0108, was missing. In addition these procedures were not available in the super shack for pumper gaugers. This committee recommends that a copy of the procedures be made available to the pumper gaugers. 2. In lieu of the fact that some safety showers are alarmed in the super shack, we feel that the decision to not establish a procedure for bypassing interlocks and alarms needs to be reconsidered. Personal Safety Equipment Several reel type air hoses at Dock 1 did not have plugs on the end. Condition of Safety Equipment 1. No safety chain at the top of the ladder at Dock 1. LAM001834 2. Need to relocate unloading hose restricting access to fire extinguisher at Dock 5. SC 000636 Glen Yogtman Page 2. Safety Hazards 1. No toe plate on scaffold between Docks 1 and 2. 2. Ladder not racked at Dock 1. 3. Rubber grommets in poor condition at many utility stations. 4. Barricade tape down around hole for septic tank at Dock 2. Work on this sep tic tank needs to be completed as it has been going on for a long period of time. 5. Need new fitting on the nitrogen line south end of Dock 4. 6. Eyebath screens were off at Dock 4. 7. No check valve on the nitrogen line at #1 spot west on consolidated truck rack. 8. Nitrogen line on the second level at spot #2 west at the consolidated truck rack has open pipe - no fitting. Housekeeping 1. Trash by slop tank at Dock 1. 2. Capital Projects has some non-standard ladders between Docks 1 and 2. 3. Docks 2 and 4 need a general cleanup. 4.
