Calgary Energy, LLC employee, Toby Jones was present during the inspection. This was an EPA inspection in response to an NRC Spill Report (NRC No. 1429257) which was received by EPA Region 6. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There was no other federal, state, or local agency representative present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection at the Calgary Energy, LLC, Tidal Osage 1 Lease, where the Tidal Osage 1 well 1-2 released oil to the surface from a subsurface casing leak. Section II - OBSERVATIONS I observed the release point for the spill which was the Tidal Osage 1 well 1-2. The well was temporarily abandoned and was not in production at the time of the spill and inspection. This was the second occurrence of the well surfacing oil from a subsurface casing leak. See photo No. 1 and 2. For additional GPS location information and photo metadata, please see Appendix 1 (photo log) attached to this report. I observed a pool of oil off the west edge of the well location along the flowpath of the spill. See photo No. 3. I observed the flowpath of the spill from the point of entry (POE) into the unnamed tributary of Little Hominy Creek back toward the release point. The flowpath varied between 1 foot and 15 foot in width. See photo No. 4. I observed the POE into the unnamed tributary of Little Hominy Creek. At the time of the inspection there was no active flow of oil into the tributary. See photo No. 5. Downstream from the POE, I observed floating oil, absorbent pads and an oil absorbent boom which had been placed to prevent downstream migration of oil. See photos No. 6-8. I observed the confluence of the impacted tributary and Little Hominy Creek. At the time of the inspection, oil had not reach Little Hominy Creek. See photo No. 9. 2 Section III - AREAS OF CONCERN Calgary Energy, LLC / Tidal Osage 1 Lease CWA Inspection Date 04/23/2025 The primary areas of concern included the recurrent purging of oil from the compromised well casing of well 1-2, the oil impacted overland flowpath, and the oil impacted section of the unnamed tributary of Little Hominy Creek. A temporary solution was applied to the well in 2021 but failed to prevent recurrence of the purging event. A permanent solution to the failed well casing should be achieved to prevent additional recurrence of the spill event. Section IV - FOLLOW UP EPA inspector conducted a follow-up phone call with Calgary Energy, LLC representative, Blaine Haag, at the end of the inspection. Mr. Haag stated he planned to test the well casing for integrity, then convert the well to production or plug and abandon the well. EPA inspector will conduct follow-up calls, field meetings and inspections to ensure the impacted area is returned to "pre spill" condition. EPA inspector will continue follow-up with Calgary Energy, LLC to observe the permanent solution to the recurrent purging problem.
