In general, most items are on schedule. Conrad discussed the Hourly Safety Committee meeting. ( NEW BUSINESS Howe discussed a near-miss injury and suggested that a SHAC alert be issued regarding water entering radio. He will issue the SHAC alert. Fritz discussed a concern of shelf life of amyl nitrite. The shelf life is about 2 years max. Riddle agreed to review and institute a proper program to insure good material is maintained. Howe discussed a concern about utility stations. Gatewood PMSB SHAC Central Files File General Comments The 1st Quarter Competitive Safety and Housekeeping Inspection of the Shops was made on March 22. There appeared to be a high level of safety awareness and personal knowledge of safety procedures among all craftsmen. The following are recommendations and concerns of the inspection team. Lockout Procedure Two infractions of the Lockout Procedure were found during the inspection. Pipefitters were working on the NE overhead crane without the main breaker being locked out. There was confusion as to where the main breaker was located. The Marvel bandsaw in the Pipefitters Shop was being worked on with a "tag" on the breaker instead of a lock. A review of the Lockout Procedure and breaker location concerning shop equipment is recommended. Safety Training On Shop Equipment Through interviews with the craftsmen, it was indicated that most safety training on equipment came during apprenticeship, which in many cases I SC 001209 LAM002409 2- - (' was years ago. Even though craftsmen are told to be safe when operating shop equipment, seldom specific hazards are pointed out and "things that can go wrong" clearly identified. Some shops have excellent manuals written by leadmen covering hazards and the use of shop equipment, but very few of the craftsmen knew they existed. Considering the injuries we've had using common shop equipment such as grinders, drill presses, and lathes, a review of safe practices using shop equipment might be in order. Safety Log Books Showed little use. Some up to two years without entries. P-Bulletins (. -------------------- Review of P-Bulletins was found to be inconsistent. Feedback ranged from little or no review of P-Bulletins to monthly reviews by the Instru ment and Electrical Departments. Safety Meetings Feedback concerning the bi-monthly craft safety meetings was inconsistant. Except for the Electrician, the impression was received that we were only "going through the motions" every two months. The effectiveness of the bi-monthly decentralized safety meetings should also be reviewed. Some craftsmen normally assigned to the field indi cated that they have never attended a meeting, while some craftsmen remembered attending two. ( SC 001210 LAM 002410 -3- STOP Program Craftsmen that knew about the program thought that it was good. Observations Noted Per Craft Electric Shoo No unsafe acts observed. High level of safety awareness. Safety logbook not used in approximately 2 years. Format for safety meetings appeared to be very good with weekly and monthly meetings. P-Bulletins reviewed on regular basis. Housekeeping could have been better considering the small number of people working in the shop. Ladder needs to be racked instead of leaning against wall. Conduit needs to be stored in racks. Clutter on window sill. ' Not clear as to where safety glasses need to be worn in the shop. ' Clutter on drill press table. Items on top of hydraulic press. Items stored on building purlin. Inspection tag missing on one ladder. Eye bath dirty. General housekeeping was poor. Paint Shop No unsafe acts observed. Painter indicated he felt that Management's committment to safety the last two years was "real" and at a high level. SC 001211 LAM002411 -4- Safety logbook indicated recent use. P-Bulletings not reviewed regularly. Good Housekeeping Eyebath dirty and eyepiece needs to be replaced. ' Drum of solvent stored next to heater. Machinist Shop Man found standing on milling machine table. Safety logbook not used in one year. There existed a high level of safety consciousness among the craftsmen. P-Bulletins were reviewed. Housekeeping could have been better. Machinists felt that the hydraulic press needs to be repaired or replaced. The return spring is broken in the cylinder. Hoses not racked. Oil on floor under stairs. Eyebath dirty Tools left out. Wrench found altered to accept larger nut. Boilermaker Shop No unsafe acts observed. High level of safety awareness. Safety log book had entry in January with no written answer. Feedback from crafts men indicated that P-Bulletins were not being reviewed. Housekeeping SC 001212 LAM002412 -5- was adequate. Some Boilermakers interviewed indicated safety rules are more apt to be bypassed for fear of their jobs due to present economic condition. Safety chain was down behind shear. ' Hose not racked. Cigarette left burning near cutting rig. Pipe Shop Two unsafe acts involving Lockout Procedure were observed: Pipefitters were working on the NE overhead crane without it being properly locked out, and the Marvel bandsaw was being worked on with a tag instead of a lock on the breaker. No positive feedback on P-Bulletin reviews. Safety log book was up-to-date. Satisfactory housekeeping. Pipe storage was very neat. * Oil spilled on floor. * Tarps blocking doorway in storage area. ' Conduit lying on floor. Instrument Shop No unsafe acts observed and safety awareness appeared to be very high. Safety logbook was being used and up-to-date. P-Bulletins were reviewed on a regular basis. Did not have any written procedures on the safe use of shop equipment, but the Shop Chief seemed to be in control of 001213 LAM002413 6- - equipment use. Housekeeping was a mess in the south part of the shop and storage area. ' Vales blocked egress to eyebath. Drill press table was cluttered. ' Clutter on cabinets and floor of south shop. Carpenter Shop No unsafe acts observed and safety awareness appeared very high. Very positive responses from craftsmen. Safety log book was up-to-date. Shop was very neat and clean even though work was in progress. Little indication that P-Bulletins were being reviewed. Craftsmen were very knowledgeable of Lockout Procedures. ( ' Good housekeeping. Two craftsmen did not know exactly where the nearest eyebath was located. Insulator Shop No unsafe acts observed and there existed a high level of safety awareness among the craftsmen. Safety logbook was up-to-date. The shop was active and very neat and clean. Little indication that P-Bulletins were being reviewed. Good housekeeping. sc 001214 LAM002414 -7- Rating Scheme Used Safety accounted for 75% of the rating and housekeeping accounted for 25%. Catagories in the attached table was given a score of 1 to 10 by mutual agreement of the inspection team.
