Section I - INTRODUCTION

Date 12/23/2024.
PURPOSE OF THE INSPECTION
EPA Region 6 inspector David Esparza, P.E., along with Nafis Fuad, Compliance/Enforcement Supervisor and Issac Martinez- Monitoring Team-Scientist of the New Mexico Environment Department (NMED) arrived at the City of Rio Rancho Wastewater Treatment Plant (WWTP) located at 100 Industrial Park Loop NE, Sandoval County, New Mexico at 9:00 AM (MST) on December 23, 2024, for an unannounced inspection with regards to a failure of a force main from Lift Station-21 (LS-21) resulting in a continuous sanitary sewer overflow (SSO) event.
We met with Mr. Steve Gallegos, City of Rio Rancho Acting-Director Utilities Department, Mr. Dennis Gonzales, Project Manager and Mr. Bill Jacquez, Compliance Coordinator, both being from Jacobs Rio Rancho's contract operator.
I presented my credentials and informed the group that this was an EPA inspection to determine compliance under the Clean Water Act (CWA). This inspection was conducted under the authority of the National Pollutant Discharge Elimination System (NPDES) permit program, in accordance with the CWA. The generation of this report is based on information supplied by Rio Rancho representatives, observations made by the EPA inspector, and records and reports maintained by the permittee (Rio Rancho) and the EPA. Before leaving the facility, an exit briefing was held with Mr. Jacquez to explain areas of concern noted at the time of the inspection. NOTE: Mr. Gallegos was available by telephone as he had a prior commitment.
FACILITY/SITUATION DESCRIPTION
The 14-inch force main from LS-21 (1650 Riverside Drive), adjacent to WWTP #3 (NPDES permit NM0029602, issued on June 6, 2016, and terminated on March 24, 2021) had ruptured on December 8, 2024, at approximately 7:15 AM (MST). Responding crews constructed unlined earthen berms or impoundments to stop the flow from continuing to enter a nearby arroyo and the Rio Grande located East and down gradient. The December 19, 2024, SSO notification states the estimated event volume was 168,000 gallons, inclusive of the estimated 72,000 gallons that reached the Rio Grande. SSO flows were ceased at approximately 11:00 PM (December 8, 2024) after line repairs were completed. Rio Rancho estimated an impacted land surface area of approximately 97,500 ft2. Efforts to mitigate the impacted soils contained in the impoundments, included excavation to a depth of 4-inches (97,500 ft2 x 4-inches = 32,500 ft3 (approximately 1,200 yd3) and the spreading of lime (December 9, 2024). The berms or impoundments were removed when remediation efforts were completed on December 13, 2024. The excavated impacted soils were transported to the Rio Rancho WWTP #2 at 100 Industrial Park Loop NE for further disposition. E. coli analytical samples were collected upstream and downstream from where the SSO spill entered the Rio Grande.
Section II - OBSERVATIONS
I observed the following and/or the following information was provided and/or stated:

Lift Station -21- experienced 14-inch line failure on December 8, 2024. SSO flows were ceased at approximately 11:00 PM (December 8, 2024) after line repairs were completed. SSO flows continued for approximately 15+ hours (approximately 0700 to 2300 hrs.). Backfill was completed at approximately 00:18 AM (December 9, 2024).
Responding crews constructed earthen berms or impoundments to stop the flow from entering a nearby arroyo and the Rio Grande located East and down gradient. The Rio Rancho December 19, 2024, SSO notification states the estimated event volume was 168,000 gallons, inclusive of the estimated 72,000 gallons that reached the Rio Grande. Rio Rancho estimated an impacted land surface area of approximately 97,500 ft2. The Southern Sandoval County Arroyo Flood Control Authority (SSCAFCA) only requires verbal, or electronic mail (e-mail) notification should an event occur within their jurisdictional boundaries. Additionally, Mr. Edmonson the SSCAFCA Facilities Operations Director, was on site after the initial response and the city, had constructed an earthen berm prior to his arrival during the cleanup or containment efforts.
The adjacent WWTP #3 (NPDES permit NM0029602, issued on June 6, 2016, and terminated March 24, 2021) was observed to confirm, previous treatment basins were not utilized as equalization (EQ) basins.
When asked about isolation valves Mr. Gallegos stated "no valves" existed (Appendix 2 Request for Information- December 26, 2024).
A previously provided Rio Rancho spreadsheet for the period of August 2022 through June 2024 indicated SSO events related to LS-10, LS-15, LS-20, LS-22, LS-24, and LS-28. Of significance both LS-15 and LS-22 collect comingled flows and experienced events on August 27, 2022, July 6, 2023, and June 29, 2024, respectively. Of note, during this onsite visit LS-#15, LS-#21, and LS-#22 were observed by EPA and NMED personnel. Additionally, LS-#10 and LS-#24 were visited by NMED and Rio Rancho personnel. In the interest of time, it was decided LS-#28 (the newest LS) and LS- #20, located on the opposite side of the city were not visited.
Though some of the Lift Stations observed during this December onsite inspection-maintained perimeter fencing various areas of the fencing exhibited damage, others lacked secure site fencing at all, exhibited vagrant vegetive growth, had visible evidence of previous storm event erosion and/or lacked visible flashing lights or audible alarms. Additionally, visible notification signage was lacking.

The following areas of concern (AOCs) were identified and/or discussed with the City of Rio Rancho:
1. Lift Station -21- experienced 14-inch line failure on December 8, 2024. SSO flows were ceased at approximately 11:00 PM (December 8, 2024) after line repairs were completed. 3 The requested electronic copy of the Sanitary Sewer Overflow (SSO) event, inspection reports and associated work orders/work sheets related to LS-21 for the previous two (2) years indicates more than 3-dozen Operating and Maintenance (O&M) issues related to pump failure, communications loss, alarm notices and force main piping (Appendix 5 LS-21 Related Work Activities for the Previous 2-Years).

EPA Region 6 inspectors David Esparza, PE conducted a closing conference at Rio Rancho WWTP #2 at approximately 3:30 PM on December 26, 2024, for the inspection. During the closing conference, David Esparza reviewed the several Areas of Concern noted during the inspection. Additionally, several AOCs were determined after the conclusion of the inspection and were not included in the closing conference.
Section IV - FOLLOW UP
The following information was received by EPA on January 6, 2025, and January 15, 2025, after the inspection on December 23, 2025, and review of items submitted for the Information Request needed to finalize this report. Note: The information/clarification request is in bold:

Request, and Response (Appendix 2 Request for Information- December 26, 2024):
1. Clarification of the flow paths (comingling) related to LS-8, LS-15, LS-21, and LS-22, inclusive collection line size (diameter), distance between collection points and location of appurtenant isolation valves.
All Lift Stations in the City of Rio Rancho are designed with two pumps. Each pump is designed to handle maximum peak flows. The lift stations have a low-level alarm, lead pump start, lag pump start/alarm, high level alarm. Lift Station 8 Collects Rivers Edge 1 subdivision and pumps via 6-inch force main to Lift Station 21 Wet Well (approximately 700 to 900 linear feet, As-Built 972 pages 29 thru 31). Lift Station 8 pumps to the Lift Station 21 wet well. Lift Station 21/13 sits adjacent to Wastewater Treatment Plant 3 and the Rio Grande River. Lift Station 21 pumps via 14-inch force main (16,800 linear feet) from Wastewater Treatment Plant 3 to Wastewater Treatment Plant 2 (As- Built 454). Lift Station 22 (Approximately 220 feet of 6-inch PVC) connects to the 14-inch force main (approximately 8,900 linear feet) at Corrales Rd/Lomitas Negras. Lift Station 15 Pumps to a 18-inch HDPE (approximately 4,500 LF) line up to Willow Creek and is reduced to the 14-inch force main. The 14-inch force main runs from Willow Creek and connects to the original 14-inch force main on Riverside Drive. The overall force main length is approximately 26,000 linear feet. No isolation valves. Only valves were for pigging stations. An InsertaValve-Romac Industries is being planned to be place upstream of LS21 to avoid or mitigate any future force main breaks.
2. Electronic copies of the associated "as-built" drawings related to LS-21.
As-Built 454 Force Main from Wastewater Treatment Plant 3 to Wastewater Treatment Plant 2.
3. Clarification/delineation of impacted surface area resulting from the recent LS-21 Sanitary Sewer Overflow (SSO) event.
See LS_21 Gravity and FM on shared drive (Appendix 4 LS-21 Gravity- Force Mains and Impacted Area). The surface area was calculated through google maps, please see attached document (LS_21 Gravity and FM_4). A total area of 3,251 Ft was determined and multiplied by the arroyo size towards the entry into the Rio Grande, 30 Ft. This results in a surface area total of 97,530 Sq Ft.
4. Clarification of the disposition of the excavated impacted soils associated with the recent LS21 SSO event.
The third-party contractor removed approximately 4 inches of soil of the affected area. The soil was disinfected with lime and hauled back WWTP #2 where we will dispose of at the local landfill.
5. Clarification/confirmation if the on-site impoundments related to the LS-21 SSO event were earthen or poly-membrane lined.

Appendix 5 LS-21 Related Work Activities-Previous 2-Years
7. A complete electronic copy of your Standard Operating Procedures (SOPs) and clarification how often it is updated, specifically related to SSO events.
Please see attached SOP. This is updated yearly, we will also add Corrales, Albuquerque Bernalillo Water Utility Authority and the Isleta pueblo to the list for future events or failures.
8. An electronic copy of the 30%/60%/90% plan set drawings of the stated upgrades to LS-22.
I attached the 90 percent drawings along with Huitt-Zollars Letter 10/29/24 (Project R31 on shared drive.). I did not see a 30% or 60% in the project file folder. We can reach out to H-Z to see if they turned any in to the city.
9. Clarification of the City of Rio Rancho's Public Works Department versus the Jacobs Engineering Group with respect to the collection system.
The City of Rio Rancho owns the water, wastewater, and reuse systems. Jacobs/OMI operates the facilities. Jacobs/OMI is responsible for maintaining the wastewater collection system, lift stations, force mains, and wastewater treatment plants by contract.
10. Confirmation the Emergency Response Plan (ERP) has been updated consistent with the NPDES permit.
ERP has been updated to correlate with the NPDES permit and include all personnel.

11. In reviewing the various documents Rio Rancho submitted regarding the Sanitary Sewer Overflow (SSO) event I request clarification of the time frame. The letter to Ms. Nancy Williams on December 12, 2024, states the event occurred "at approximately 07:14 on Sunday, December 8, 2024". The following is known or stated in the letter:
a . A local contractor was called out to repair the line. b. The estimated SSO volume was 168,000 gallons and that an estimated volume of 72,000 gallons
reached the Rio Grande. c. Overflow was ceased on 12-8-2024 at 1111. Is this AM or PM as the "line repair was completed
on 12-9-2024 at 0018." d. Clarification how the overflow was ceased.

The event occurred at approximately 07:14 AM Sunday December 8, 2024. The line repair was made on the cracked force main on Sunday December 8, 11:11 PM (23:11) which stopped the flow of the sewage. Final backfill was completed on Monday December 9, 2024, at 00:18 AM. The overflow (cracked force main) ceased when the 14-inch force main was repaired on December 8, 11:11 PM (23:11) 2024. I wanted to let you know the Lift Station 8 6-inch force main (SSO) broke this last weekend. No sewage reached the river. Jacobs reached out on the contact list and is currently preparing the letter for Ms. Nancy Williams. I believe this is the first break in the 6-inch force main since around the 2005-time frame. The lift station 8 as-built was part of the shared folder. It is my understanding the City of Rio Rancho Wastewater Treatment Plant (WWTP) is contactor operated by Jacobs Engineering Group. The following information is requested to make a complete and accurate assessment of the applicable parameters associated with the permit. Additional information might be requested after my review of the received information. The specific request for information (RFI) will be delineated below. Therefore, please forward by email the requested information as soon as possible, but no later than close of business January 6, 2025.
Clarification of the flow paths (comingling) related to LS-8, LS-15, LS-21, and LS-22, inclusive collection line size (diameter), distance between collection points and location of appurtenant isolation valves. All Lift Stations in the City of Rio Rancho are designed with two pumps. Each pump is designed to handle maximum peak flows. The lift stations have a low-level alarm, lead pump start, lag pump start/alarm, high level alarm. Lift Station 8 Collects Rivers Edge 1 subdivision and pumps via 6-inch force main to Lift Station 21 Wet Well (approximately 700 to 900 linear feet, As-Built 972 pages 29 thru 31). Lift Station 8 pumps to the Lift Station 21 wet well. Lift Station 21/13 sits adjacent to Wastewater Treatment Plant 3 and the Rio Grande River. Lift Station 21 pumps via 14-inch force main (16,800 linear feet) from Wastewater Treatment Plant 3 to Wastewater Treatment Plant 2 (AsBuilt 454). Lift Station 22 (Approximately 220 feet of 6-inch PVC) connects to the 14-inch force main (approximately 8,900 linear feet) at Corrales Rd/Lomitas Negras. Lift Station 15 Pumps to a 18-inch HDPE (approximately 4,500 LF) line up to Willow Creek and is reduced to the 14-inch force main. The 14-inch force main runs from Willow Creek and connects to the original 14-inch force main on Riverside Drive. The overall force main length is approximately 26,000 linear feet. No isolation valves. Only valves were for pigging stations. An InsertaValve-Romac Industries is being planned to be place upstream of LS21 to avoid or mitigate any future force main breaks.

Clarification/delineation of impacted surface area resulting from the recent LS-21 Sanitary Sewer Overflow (SSO) event. See LS_21 Gravity and FM on shared drive. The surface area was calculated through google maps, please see attached document (LS_21 Gravity and FM_4). A total area of 3,251 Ft was determined and multiplied by the arroyo size towards the entry into the Rio Grande, 30 Ft. This results in a surface area total of 97,530 Sq Ft.
Clarification of the disposition of the excavated impacted soils associated with the recent LS-21 SSO event. The third-party contractor removed approximately 4 inches of soil of the affected area. The soil was disinfected with lime and hauled back WWTP #2 where we will dispose of at the local landfill.
Clarification/confirmation if the on-site impoundments related to the LS-21 SSO event were earthen or poly-membrane lined. The on-site impoundments are earthen lined.
A complete electronic copy of your Sanitary Sewer Overflow (SSO) event, inspection reports and associated work orders/work sheets related to LS-21 for the previous two (2) years. Spreadsheet from Jacobs attached.
A complete electronic copy of your Standard Operating Procedures (SOPs) and clarification how often it is updated, specifically related to SSO events. Please see attached SOP. This is updated yearly, we will also add Corrales, Albuquerque Bernalillo Water Utility Authority and the Isleta pueblo to the list for future events or failures.
An electronic copy of the 30%/60%/90% plan set drawings of the stated upgrades to LS-22. I attached the 90 percent drawings along with Huitt-Zollars Letter 10/29/24 (Project R31 on shared drive.). I did not see a 30% or 60% in the project file folder. We can reach out to H-Z to see if they turned any in to the city.
Clarification of the City of Rio Rancho's Public Works Department versus the Jacobs Engineering Group with respect to the collection system. The City of Rio Rancho owns the water, wastewater, and reuse systems. Jacobs/OMI operates the facilities. Jacobs/OMI is responsible for maintaining the wastewater collection system, lift stations, force mains, and wastewater treatment plants by contract.
Confirmation the Emergency Response Plan (ERP) has been updated consistent with the NPDES permit. ERP has been updated to correlate with the NPDES permit and include all personnel.

Good afternoon Mr. Esparza,
The event occurred at approximately 07:14 AM Sunday December 8, 2024. The line repair was made on the cracked force main on Sunday December 8, 11:11 PM (23:11) which stopped the flow of the sewage. Final backfill was completed on Monday December 9, 2024, at 00:18 AM.
The overflow (cracked force main) ceased when the 14-inch force main was repaired on December 8, 11:11PM (23:11) 2024.
I wanted to let you know the Lift Station 8 6-inch force main (SSO) broke this last weekend. No sewage reached the river. Jacobs reached out on the contact list and is currently preparing the letter for Ms. Nancy Williams. I believe this is the first break in the 6-inch force main since around the 2005-time frame. The lift station 8 as-built was part of the shared folder.

In reviewing the various documents Rio Rancho submitted regarding the Sanitary Sewer Overflow (SSO) event I request clarification of the time frame. The letter to Ms. Nancy Williams on December 12, 2024, states the event occurred "at approximately 07:14 on Sunday, December 8, 2024". The following is known or stated:
A local contractor was called out to repair the line. The estimated SSO volume was 168,000 gallons and that an estimated volume of 72,000 gallons reached the Rio Grande. Overflow was ceased on 12-8-2024 at 1111. Is this AM or PM as the "line repair was completed on 12-9-2024 at 0018." Clarification how the overflow was ceased.
