The Quality Assurance Lab will be given the outstanding award. Quality Assurance Lab Research I Research III Research II c GENERAL COMMENTS Outstanding Excellent Excellent Satisfactory o All lab vent hoods' air velocity should be set between 100 fpm (min.) to 125 fpm (max.). Lab vent hoods ranged from 96 fpm to 178 fpm. 0 Housekeeping in all areas was very good. 0 Use of cloth strap (car seat belt type) to secure gas cylinders needs to be reviewed with Plant Safety Department for compliance. QUALITY ASSURANCE LAB Second floor wet lab vent hood - last inspection was 1984. Safety shower and eye batch in south lab not checked since 7/31. RESEARCH I Need to review use of combination locks (1st floor) with Plant Safety Department - concern over off-hour entry by Guards/Night Supt./Fire Department during emergency. 0 Need to provide secondary (emergency) escape ladder from roof (ex. QA Lab stair platform extension to include roof area). L SC 000302 LAM001499 2- - r \ Hose inspection - roof area. Vacuum tank in penthouse - is it on AVIP list? Ladder not inspected in penthouse area. 0 Bottom plate on men's restroom door (1210) needs repair. 0 People/equipment space problems in lab 1206/1207 - should review. RESEARCH III 0 Hose inspection south side of PETS Lab. o All hoses (small) should be inspected to be consistent with plant policy. 0 Clutter in storage area. RESEARCH II 0 Chain around cylinder valves in lab 2136C (this item was pointed out in 1983 Annual Inspection) - need to resolve problem. c 0 Air mask hose inspection - catalyst preparation area. Hose inspection - A/C room roof. General housekeeping - A/C room roof. Wood ladders not inspected - lab 2128B. Should air surge tank be on AVIP list? 5-Minute escape mask - last inspection 5/22 - 1st floor west. (If one person did all safety equipment inspections, this may solve the problem.) Halon system needs consistent inspection tag system (lab 2,3). Elevator should be designated for personnel or freight - no inspection, fire extinguisher, etc. Clutter in storage area at 2134. 5-Minute escape mask next to 2136C below 4k psi. 0 Four small unsecured cylinders in lab 2136C, L SC 000303 LAM001500 -3- Need to specify smoking/non-smoking areas in back lab areas.
