Inc.. Waiau Station Unit No. 8. Honolulu. Hawaii Purchaser: American Factors Ltd. (AMFAC) Inc. Honolulu, Hawaii CE furnished (1) Type "RRP" Radiant Reheat Pressurized Steam Generating Unit having a capacity of 575,000 lbs./steam per hour maximum continuous - no peak. Furnace width 19'-11" and furnace depth 28'-8". Unit is an exact duplicate of Unit No. 7 supplied under Contract No. 5863. Purchaser supplied necessary field labor to install the CE supplied boiler and auxiliaries and CE supplied setting materials. ^ Purchaser supplied and installed insulation materials and CE supplied setting materials. During the life of the contract Hawaii became the 50th state. During the life of the job "American Factors Ltd." name was changed to "AMFAC Inc.". Export Sales Agreement 1/15/59 CE/American Factors Ltd. CE/AMFAC Sales Representative Agreement 7/1/67 under which AMFAC is paid a commission based on contracts sold involving EXHIBIT C DOOOl 'G. Kittle 2 March 1, 1993 AMFAC purchase order for the boiler includes CE Proposal No. 15763A 4/3/63 as part of their purchase order. Proposal under Settings includes refractories and no insulation material by CE and under Insulation not- by CE - Purchaser supplies on a' delivered and erected basis. Estimate includes under Setting refractory materials only and no insulation material by CE. Manufacturers Data Report indicates Boiler Serial and National Board No. 20694 and year built 1967-68 with Certification of Field Assembly Inspection - Post Hydro by Royal Indemnity Co. of New York, New York on 10/9/68. Certification that erection was performed in accordance with requirements of ASME Code by Customer's Erection Subcontractor BWK Joint Venture 8/20/68. CE Erector E. King Curry. Unit went into commercial operation in November 1968. Attached are the following CE purchase orders issued on suppliers involving Settings part of CE supply: P.0. No. 1702177 5/8/67 issued to Refractory & Insulation Corp. ordering refractory setting materials including 150 lbs. super stic-tite plastic insulation, 40 lbs. Mix "A" packing material. No. 1702179 5/8/67 issued to Refractory & Insulation Corp. ordering weatherproofing for walls and doors consisting of 5,400 gallons of Insulkote "ET" as manufactured by Johns Manville Corp.
