ROSE cc: RE: ROD MILL DRIVE LOCK OUT, BUILDING R-25 As requested, this letter is to confirm the desired procedure to be used when it is necessary to safely immobilize the rod mill. This letter deals only with the rod mill immobilization and does not cover other necessary safety procedures such as blinding of pipe, entry permits, locking of the pan conveyor drive, etc. Any time maintenance is performed to the rod mill feed chute, internally, feed drum, rod mill, rod mill drive, rod mill discharge tube or slurry mixer, a yellow equipment tag, a white personnel danger tag and lock is to be affixed to the open main power switch for the rod mill drive motor. The locking apparatus should provide for installation of multiple locks. Departmental locks should be provided for each maintenance person who will maintain possession of the key while performing maintenance to the above mentioned mechanical equipment. A master key for all area de partments should be provided only to the responsible mechanical maintenance area supervisor. This procedure was agreed upon at a meeting attended by yourself, N. E.
