Sharp PLAINTIFF'S EXHIBIT The Third Quarter Safety and Housekeeping Inspection for Departments 44, 45 and 46 was conducted by R. A. Forsthoffer, Pinky Cole, Charles Anders, Bob Hammann and J. V. Cavender on September 11, 1985. The overall rating of the department is "Outstanding". The following items were noted: General Comments 1. The new protective bypass procedure is in hand and they are ready to start training. 2. JSA training is about 84Z complete. 3. Ladder inspections and fire monitor checking is up to date. 4. Hurricane procedures have been reviewed. 5. Chlorine storage. Training is 100Z complete. Cylinder storage area near CT-3 in excellent shape. Well marked, safety showers and equipment readily available. Safety 1. No. 1 CO detector needs new filter. 2. Chainfall at pump P103-3 has an out-of-date color code on it. 3. Fire extinguishers in good shape. 4. A chain valve at compressor C201 does not have a safety chain on the wheel. 5. A number of utility stations did not have safety pins. 6. Need to replace safety slips on air hoses near the cold box. 1.85.0514 SC 000297 LAM001494 Ken Gore -2- September 19, 1985 Housekeeping 1. Cabinet behind control panel containing Scott SKA-Paks Is messy and SKA-Pak face pieces should be cleaned. 2. Trash around scaffold material located north of the control room. 3. Old extinguisher box by oil drum storage area -- door missing and trash inside. 4. Need to clean up platform under reformer steam drum. 5. Remove ladder rack braces at top level of reformer if not needed. 6. Badly frayed ground cable in drum handling area. 7. Need to clean up insulation on 46C602DT. 8. Need to repair Insulation around 44P103. 9. Need to clean up under skirts of tower. 10. Need to clean out weeds and trash near 45T408-1, 2 and 3. 11. Need to clean up at drop off Station 44-H. 12. The ladder to the A/C unit on top of the analyzer building does not have a safety gate on It. 13. Several of the ladders on the column were not chained or had missing chains.
