Bobbie showed me a copy of a contract for a part-time physician, however, I believe I would prefer something much simpler. I believe the main purpose of my visit was accomplished in agreeing on a contract physician. Everything else seemed to be under adequate control. DHG:mm Attachment David H. Treatment will be provided in your offices or at Valley Community Hospital. Specialists will be used in consultation when appropriate to provide the best available treatment. The contract physician will be responsible for evaluating disability and determining whether the employee is capable of performing restricted work. He will be responsible for communicating and consulting with a designated representative of Union Carbide regarding availability of restricted work and the condition and progress of the patient. The contract physician should make himself available for telephone or other consultation with the plant nurse to provde guidance and assistance in special problems. The contract physician should visit the plant so as to familiarize himself with materials and operations. When the contract physician is unavailable by reason of vacation or other reasons, he must designate an acceptable physician to cover his duties and responsibilities. Box 471 Texas City, Texas 77590 Jose Kuri, M.D., P.A. 844 Central Blvd. #1800 Brownsville, Texas 78520 Dear Dr. Kuri: The following guidelines are provided to designate the duties and responsibilities of a contract physician for the Brownsville Plant of Union Carbide Corporation. A separate contract or letter of agree ment will cover examinations on a fee for service, retainer fee and duration of agreement. Examinations as follows will be ordered by the company. Forms will be furnished by the company to be used for specific exams. Basic height, weight, vital signs, sight screener, audio, spirometry, required laboratory, chest x-ray and EKG (when necessary) will be performed by the Union Carbide nurse and furnished for evaluation to the examining physician. A. Pre-employment physical exams to help fit applicants to the job and screen for potential problems. Restrictions should be designated when appropriate in evaluating handicapped applicants. Applicants should be informed of abnormal find ings and results should be furnished to the company. Bi Periodic clinical health exams to evaluate the health status of employees. Communication of abnormal findings should be made to the employee. Special forms are provided so that data may be keypunched into the confidential medical files of Union Carbide. C. Medical surveillance and special exams as may be required by OSHA or corporate policy: 1. Heavy equipment and forklift operators to determine ability to safely perform their jobs. Minimum phy sical standards will be furnished separately. 2. Tankerman's license exam. Minimum physical standards will be furnished separately. 3. Medical clearance for wearing a respirator. Only those employees with abnormal spirometry or special problems need to be personally examined. 4. Medical surveillance exams for chemical exposure. Specific information will be furnished when required.
