(Altoona District) Dec been a truck driver for above company past twenty-two vear:'.' ~Y/ork consists of loading, and unloading trucks at destination, generally six loads of gasoline daily of 1000 gallons each, g a u g i n g tanks, and part of the time in unloading, tank cars of gasoline. Work includes general work at a bulk terminal. States that present illness began about a year ago becoming noticeably worse in January last. It started with headaches mostly frontal but at times' involving entire head and states "head felt tight and like it was going to burst" . Also at onset noticed a pain in right ankle and heel. Over tibia anteriorly to knee and over thigh up to hip and around the buttock particularly on right the skin was hyperaes'thesic. - The left leg was practically uninvolved. At no time did he have a definite sciatic neuritis it being more a "tingling"'of the skin and pain when the skin was touched or pinched. This hyperaesthesia has persisted along-with the headaches but the pain in the ankle and heel has practically disappeared. At present his complaint is "constant headache and tingling, numbness or pain in right leg" . His appetite is improv ing and fairly good at present. during the past year he has contacted a number of doctors. According to his history he first visited a Dr. Brant, ~hellsburg, Pa.; then one visit to Dr. Enfield at Bedford, Pa. who sent him to the local hospital (Everett Hospital) where Lr. Everett xrayed his head and made a diagnosis of sinus disease. He was not satisfied and then visited his brother in Pittsburgh who is a naturopath and he -was given electrical treatments over sinuses and placed on a diet of eight oranges daily for 55 days. Luring this time he became quite weak, lost JO pounds in weight, end his systollic blood pressure dropped to 9'5/' * -At the end of this treatment he felt worse than before - had above mentioned hvadaches, and leg involvement which appeared to be worse, then he went to the Timmons Hospital in Bedford for more xrays of his sinuses and was told he had no sinus disease. He next visited the 7/Imber Hospital at Wimber, Pa. (JO miles from Bedford - hospital, apparently owned by a coal company), here he had his head xrayed and no sinus disease was found. Pie then had his chest and lower spine xrayed, numerous blood tests made and a spinal puncture done, he states his spinal fluid was normal and both spinal and blood kassermans were negative. A neurologist from Johnstown, Pa. (hr. heyers) was consulted and saw him at the 'Timber Hospital four times. At - this time -eight samples of blood were obtained at thirty minute intervals and numerous smears were made from his finger. (.Vas told they were unable to analyze samples for lead.) A Dr. Snowden, is in charge of the Pittsburgh Clinic ana Hr. H.A. Slesinger of the Wlmber -Hospital suggested the visit and made the appointment. He came to Pittsburgh on May 19 and remained five days. During this time he was seen daily excepting Sunday during which time blood examinations were made and further xray studies done including xrays of his teeth. (He states his blood is to be tested here for lead). 'When seen this morning .(May Pig) he is now finished here at Pittsburgh and is returning to Bedford today.' ' ` Eis symptoms are those of constant headache and hyperaesthesia of upper thigh and right buttock (the hyperaesthesia of lower right leg has cleared, up). - Ct.II. nothing abnormal, no pain, burning, nocturia, hematuria, or System history of venereal disease. Prostrate gland apparently elm- recently examined. . f C.R. Ho cough, no expectoration, no post nasal discharge and no System early morning cough of chronic sinusitis, and no chest pain, ho symptoms referable to heart excepting: did have weakness and so::e shortness of breath on orange diet when he lost fO pounds in 1 weight in one month. 9.1. He seems to think the le involvement is gradually cleansing up. Hot easily excited but has been and is still worried as to the seriousness of his illness past year. Had eight brothers ana two sisters (three pairs of twins) six of which are dead ("flu", typhoid, and four in infancy). He had been healthy in past until onset of present illness. Ho operations or serious injury. Ho tuberculosis or family history of same, ho mental disease in family. He had five teeth extracted sometime ago because oi? pyorrhea. . Lairoat tongue in midline, heavily coated, and has coarse tremor. 4 lumber of teeth extracted. There are two marginal fillings in lower left molars and one in upper right molar. Teeth appeal* viable although discolored. Gums show a gingivitis, particularly about upper molars. Ho lead line present. Tonsils small. ,o exudate expressed, pharynx is clear and reflex and musculature ok. Trachea In midline and thyroid not enlarged. . ..lands Ho adenopathy - very snail anterior cervicals palpable - not tender. Chest Expansion is equal - chest is clear on P and A. Whispered and spoken voice transmitted normally. te art Hot enlarged. bounds are of good quality. Ho murmurs present. Below the knee on the right nothing abnormal noted. Over 'the right thigh anteriorly there is a hyperaesthesla extending up and around on to right buttock.
