Interview Date: 4/22/25 SECTION I - INTRODUCTION Site Entry/Opening Conference and Inspection Objectives EPA Region 5 Inspector(s) Newton Ellens and Tatiana Frontera arrived at Welsch Ready Mix, Inc. (the "Facility"), located at 10175 Vans Dr. on March 7, 2025, for an unannounced inspection. There, Mr. Ellens presented his credentials to Facility representatives. The Facility representatives stated that David Johnson, the environmental contact, was not present at the time. Mr. Ellens later made arrangements to interview Mr. Johnson via telephone on April 22, 2025. On April 22, 2025, at 2:00 p.m. CT, Mr. Ellens called Mr. Johnson to conduct the interview. EPA Region 5 Lead Inspector Newton Ellens Informed the Facility that this was a US EPA Region 5 Industrial User Screening Interview to gather Facility information, including on its industrial process and wastewater management as authorized by Clean Water Act (CWA) Section 308 and implementing regulations. Discussed the right to claim Confidential Business Information (CBI) on any document or information shared with EPA during or after the inspection. Mr. Ellens conducted an opening conference with the Facility representatives (See table below) explaining the information gathering checklist. This report is based on information gathered prior to, during, or subsequent to the interview.
