The following information was obtained verbally from Angie Augenstein, Ron Hardesty, and/or Heidi Suhoski, unless otherwise noted.
Company Ownership: Miba Bearings US LLC
Background: The facility is located on the Muskingum River in McConnelsville, Ohio. There are three permitted outfalls into the receiving stream, one for the industrial operations (Outfall 001) and the second is for the onsite sewage treatment plant (Outfall 002). Outfall 001 includes untreated wastewater from the Reverse Osmosis (RO) treatment system, once through cooling water, stormwater runoff, and treated process wastewater through Outfall 601 (internal monitoring station). Some additional stormwater runoff from the facility is conveyed through an outfall owned and operated by Gould's Electronics (OH0135178).
Process Description: Miba Bearings, LLC (Miba) produces bearings for locomotion and transportation equipment. The facility operation includes presses and machining, electroplating, heat treating, a foundry with spin casters, and a lead and tin liquid metal coating (babbitt) operation.

Per Heidi Suhoski and the walkthrough of the facility, the following wastewater wastestreams
are tributary to the wastewater treatment plant and Outfall 601:

Outfall 001 has storm water runoff, RO reject and non-contact cooling water.
At the time of the inspection, some of the Outfall 601 and Outfall 001 wastestreams could not be verified by facility staff during the walkthrough. A current drawing of the process wastewater sewers was requested during the walkaround but was not available.
Staff Interview(s): The information in this section comes from Ron Hardesty (Wastewater Operator), or Angie Augenstein (Human Capital Manager), unless otherwise stated. Heidi Suhoski was previously responsible for environmental compliance activities, but has moved a different position at the facility. Heidi Suhoski was present for the majority of the inspection and provided relevant information for this report.
The facility has approximately 230 employees, who are present 5 days a week, for 3 shifts a day. Some staff are present on the weekends for overtime and security purposes.
Per Heidi Suhoski, the schematic included as the line drawing in the 2021 National Pollutant Discharge Elimination Systems (NPDES) permit application is a schematic for the old wastewater treatment system, which is no longer in use. The new wastewater treatment system was installed in 2016-2017.
During the inspection, it was identified that oily water from floor cleaning activities and a parts washer is dumped into a sump, shown in Photo 1.
The treatment system for Outfall 601 runs twenty-four hours a day, seven days a week, regardless of staffing. Level measurement located in a dosing tank upstream of the filters, controls flow through wastewater treatment plant. Backwashes on the filters and regeneration on the wastewater treatment system ion exchange system are only performed manually, as needed, at the discretion of the operator.

During the walkaround, Travis Harra, a facility representative responsible for the Babbit unit, shared the babbit process involves melting lead and tin ingot and pouring the melted metal into a spin caster, shown in photo 30. Contact cooling water is sprayed into the caster during the operation, and the excess water is sent to a drain. At the time of the inspection, the location where the drain is connected in the facility's system could not be verified.
EPA asked for additional information about the letter from Miba to Ohio EPA, dated January 17, 2025, which was in response to a Notice of Violation (NOV) from Ohio EPA dated December 16, 2024. The facility representatives provided the following responses:
Regarding the acute toxicity exceedance, Ron Hardesty thought the sample was collected from 001 during a period of snow melt when de-icing salt may have influenced the sample. EPA obtained a copy of the Alloway laboratory report for the December 2023 acute toxicity monitoring.
The facility has been self-reporting zinc, copper and lead exceedances from the facility's NPDES permit. The facility staff were retrained in January 2025 on the facility's NPDES permit. A a copy of the training roster, slide deck, and the "EHS procedure for Electroplating Dept. SOP" Revised 10/31/2013. Copies were given to EPA during the inspection.

Data Collected and Observations: All times are in Eastern time, unless otherwise stated.
On May 6, 2025, at 8:39 AM EPA observed the oily wash dump (Photo 1), which is where floor wash water and oily parts wash water are dumped into a sump, being pumped to an oil water separator prior to a drain (Photo 41)
At 8:44 AM EPA observed the electroplating chemicals storage area. Some chemical containers were present on spill containment pads, and some were on wood pallets or the ground. There were three drums with PCE labels. Per Robin Brown, coatings business unit manager, those are for used drums waiting to be refilled with waste for disposal. The PCE drums were on a wooden pallet.
At 9:01 AM EPA observed the Pero PCE degreaser system. The system has an internal oil and water separator, which generates a small volume of wastewater that is separated from recovered PCE. The wastewater accumulates inside the Pero unit and is manually transferred to a waste drum using a funnel.

At 9:32 AM EPA observed the water tanks for the Autoplater and Side Arm scrubbers (Photos 25 and 26). Makeup water is added to these tanks constantly and blowdown is discharged to the drain shown in Photo 27. At the time of the inspection, the facility representative said the makeup water flow for tank #1 (Photo 25) was 4 gallons per minute (gpm), and the makeup water flow to tank #2 (Photo 26) was 2 gpm.
At 10:10 AM, in the boiler room, EPA was shown a white PVC pipe that runs through the boiler room. Per the facility representative, this white PVC pipe serves as the drain line to Outfall 001 (Photo 39 and 40). Inside the boiler room, this line collects flow from four separate drains within the building. The specific sources of the four lines could not be identified during the inspection. Photo 39 shows the white PVC pipe and the connecting lines.
The RO system is located in the wastewater treatment building. During the inspection the RO system reject line could not be verified as one of the four lines contributing flow to the white PVC pipe shown in Photos 39 and 40. Outfall 001 is permitted to receive both stormwater runoff and reject water from the RO system.
At 2:30 PM EPA observed the exterior portions of the facility. The facility stores metal tubes or bearing pipes outdoors on racks and wood pallets. This condition is shown in Photos 49 through 99. The dust collector for the foundry is located outdoors and shown in Photo 56. The foundry dust collector particulate is collected in a sealed drum. At the same location there is a second dust collector for a carbon machining operation. The carbon dust collector particulates are collected in an open sack shown in Photo 58, and there was a dark granular material on the pavement near the collector.
There were two large roll-off boxes storing hazardous waste as shown in Photos 62 through 67. One container held wastewater treatment sludges/filter cake (F006) and the other held cadmium (D006) and lead (D008) solids.
There were three other roll-off boxes containing non-hazardous waste as shown in photos 68 through 70 and 99.
There were six totes, marked `USED ACID', stored outdoors near the southwest corner of the facility. Ron Hardisty said these totes are used to move certain waste acids from the facility to the wastewater treatment building for disposal.
There were two above-ground tanks in containment and one elevated tank along the south edge of the wastewater treatment building. The containments for both tanks were full of water and overflowing. These conditions are documented in photos 67 through 91, The elevated tank had large chips of peeling paint as shown in Photo 92.

According to the facility representatives, the stormwater runoff from the area of the site containing the above-ground storage tanks, the elevated storage tank, and the non-hazardous roll-off boxes is the responsibility of Gould's Electronics.
On the west side of the wastewater treatment building there was a three-sided storage space. There was a gap in the curb on the north end of the storage building (Photo 71). The gap allowed stormwater runoff to pass through the storage space as shown in Photos 72 through 80. Per Ron Hardesty, this storage space held used containers awaiting disposal and some unused equipment.
Wastewater Treatment Building for Outfall 601: On May 7, 2025, at 8:54 AM EPA conducted an inspection of the wastewater treatment process. There are three 10,000-gallon equalization tanks which run in parallel. The working capacity of the tanks is 28,000 gallons per Ron Hardesty.
There were two pH readout/controllers located at the reaction tanks T4 and T5. The units were Liquidtron DP5000s. Both units had alarm codes which could not be read during the inspection. Both units also had error message "E6". According to the operations manual for those units the E6 code is for Pump A `LOCKOUT', and pump A is a dosing pump dedicated to acid addition. The pH reading at tank T4 at the time of the inspection was 8.73. Per the wastewater training manual, "WWTP & DI Training Document.docx", provided by the EPS wastewater, "Tank T 4 is dosed with acid to lower the pH to a pH of around 3 to break any surfactants." Ron Hardesty said that he does not keep tank T4 acidic.
Filter cake is collected in a hopper as shown in Photo 103.
At 9:25 AM EPA observed the pH readout/controller located at tank T8. The readout was also displaying an E6 error for Pump A `LOCKOUT'. The documentation for the Liquidtron DP5000 controller calls Pump A the acid pump. At the time of the inspection the reading pH reading was 8.23. The instruction sheet affixed to tank T8 included a statement "Average pH 7 to 8" (112)
Water from tank T8 is pumped into three multimedia filters which run in parallel. At 9:48 AM EPA observed several pump cycles and determined that the manual pressure gage on the outlet for the multimedia filters was stuck at 10 psi (Photo 114) and did not move even when the inlet pressure gage showed a reading less than 10 psi. Additionally the tap line from the outlet side of the filters to the pressure transducer was closed (Photo 113). Per Ron Hardesty, the autobackwash functions have been disabled and manual backwash is conducted as needed.
After the multimedia filters water flow through two ion-exchange (IX) units. The IX units are in series. Per Ron Hardesty, the automatic regen function has been disabled and the IX units are regenerated as needed.

Compliance samples for Outfall 601 are collected from tank T11 which is immediately downstream of IX units. Water from tank T11 is pumped to Outfall 001. A Liquidtron DP5000 is used for pH compliance monitoring for pH at Outfall 601. Per Ron Hardesty, the meter is calibrated weekly using a one-point calibration with pH 7 buffer and that the Oil and Grease sample is collected from this location using the pump and tubing on the composite sampler. At 10:00 AM EPA observed the sample collection line for the composite sampler at tank T11. The line had a sag or low spot where water can collect between sample draws (Photo 119). At 10:37 AM EPA observed the flow measurement flume for Outfall 001. There was a composite sampler at this location for collecting compliance samples. Per Ron Hardesty, the system is set to collect composites every 15 minutes. The readout for the pH meter showed a summary of flow volume, pH min, max, excursions and length of excursion for the May 1st through the time of the inspection. The min values for several days were less than permit min value of 6.5, however the length of excursion was zero. For example the summary for May 1st had a minimum pH of 6.314228 and a 0 for the length of excursion. See the screenshot below.
Per Ron Hardesty there is no pH calibration schedule for the pH meter at 001, but it is verified against a handheld device every week and will be recalibrated as necessary. There is no record of that periodic calibration, and the handheld device used for verification is calibrated with a single point.

At 11:24 AM EPA observed the condition of the overflow weirs at the secondary clarifiers at the sewerage treatment plant. At the time of the inspection there was significant buildup of material on the weir plates and, in the trough, downstream of the weir plates.
At 12:00 PM EPA observed the containment structures around two exterior tanks which were part of the old and unused wastewater treatment facility. The containment structures were flooded and full of water.

When the backwash button is pressed on the control panel, when the adjustable differential pressure between inlet and outlet is met, and when the accumulated 24 hour run time condition is met...Filters backwash when the differential pressure between inlet and outlet reaches 16PSI"

EPA reviewed the continuous pH monitoring data for Outfall 001 for May 2025 for this report. This data was provided by email from Angie Augenstein. The NPDES permit defines the pH

range as 6.5 to 9.0. A measurement is collected each minute for flow and pH. There were 8 measurements below 6.5 and no measurements over 9.0. Each pH excursion was associated with a sharp increase in flow at Outfall 001. Data for Outfall for 601 could not be provided.

Requested documents: Prior to the inspection, in an email from Jonathan Moody on 4/30/2025, EPA requested a copy of the most recent NPDES permit application. Miba provided a version of the permit application in an email from Angie Augenstein on 5/2/2025.

AREAS OF CONCERN
1. Form 2C for the NPDES Permit No. OH0048372: a) "Section II.B. For each outfall, provide a description of (1) all operations contributing wastewater to the effluent." The permit application included the following operations tributary to Outfall 601: Plating rinse water, regeneration waste from DI bldg., Babbitt rinse water, boiler blowdown, lab, filter press reject water, and Ion exchange regeneration/backwash
The application in section III. B. states there are no limitations in the applicable effluent guideline expressed in terms of production (or other measure of operation).
The permit application did not include the following wastewater streams: Contact cooling water from the quench water used in the foundry for the spin caster, Scrubber blowdown for the three scrubbers; autoplater scrubber, side arm scrubber, and aluminum scrubber, Wastewater from deburring operations, which can also be called tumbling, Salvage operation which includes grinding, The oily washwater, oil and water separator and filtration equipment shown in Photos 1 and 41. There were three versions of the NPDES permit application. The version of the application available on the Ohio EPA eDocument site and the signed version provided by Miba for the inspection did not contain this wastestream. A draft unsigned copy of a permit application provided by Miba prior to the inspection did include an "oil/water waste" stream.

b) "Section II.A. Attach a line drawing showing the water flow through the facility..." Line Drawing is defined in 40 CFR 122.21(g)(2), "A line drawing of the water flow through the facility with a water balance, showing operations contributing wastewater to the effluent and treatment units... The water balance must show approximate average flows at intake and discharge points and between units, including treatment units...."
The line drawing included with the permit application is a schematic of the treatment system from the historic and no longer used wastewater treatment system. The line drawing did not include the operations contributing wastewater, nor a water balance.
Sewer drawings for the process wastewater flows leading to Outfalls 001 and 601 were unavailable at the time of the inspection. Some drains could not be verified during the inspection, including those contributing to Outfall 001 from the four drains shown in Photo 39, the quench water systems. The continuous flow measurement at Outfall 001 indicates there is a periodic batch discharge through 001.
c) Section II.A. "additives.docx" Miba provided a list of four chemical additives used in the treatment process. The additives list did not include a defoaming agent used at the wastewater treatment system tributary to Outfall 601.
2. No Exposure Certification #0GRN00393
During the inspection, EPA inspectors observed raw materials stored outside; these materials are unfinished metal tube sections (various photos 49-81).
Particulate collection systems with material stored in super sacks. Carbon material from the collectors was visible on the ground surrounding the super sacks (photos 57 and 58).
Hazardous waste roll-off boxes stored outside (potential material handling activity) (photos 62 through 67).
Uncovered roll off boxes with non-hazardous waste (photos 68-70, 93, 99) Flooded containment structures surrounding above ground storage tanks. Per the
facility representatives the tanks were unused. However containment around the tanks were full of water. This was at two locations, one south of the wastewater treatment building and the other location at the former wastewater treatment building (Photos 82-91) Storage and activity inside a three-sided storage structure which was not storm resistant (photos 71-80)

The NPDES permit issued to Gould's Electronics for the pump and treat operation (OH0135178) does not contain stormwater requirements, as of the writing of this report, Gould's Electronics is not listed in the Ohio EPA general permit for storm water discharges from industrial activity.
3. NPDES Permit No. OH0048372, PART III - GENERAL CONDITIONS, 3. FACILITY OPERATION AND QUALITY CONTROL "At all times, the permittee shall maintain in good working order and operate as efficiently as possible all treatment or control facilities or systems installed or used by the permittee necessary to achieve compliance with the terms and conditions of this permit."
Several pH control units which control the addition of chemicals into the wastewater treatment plant had error codes for equipment faults.
Differential pressure gage at the multimedia filters was offline, and backwashes are not performed on the recommended schedule based on time or head loss.
Discharge side pressure gage on the multimedia filter was not functioning. The multimedia filter backwash and IX regeneration schedule are not based on the
recommendations in the operations manual. Compliance meter for pH monitoring is calibrated with a single pH buffer (pH 7).
Calibration does not bracket the measured pH values. There was a sag in the intake tube for the composite sampler at Outfall 601. The sag
can allow volumes of sample from prior aliquots to remain in the line. The Oil & Grease sample is collected from the composite sampler hose at Outfall
601. Oil & Grease samples must be collected directly into the container used for the sample. No record of 001 pH calibration, and the equipment used for verification is a single point pH calibration which does not bracket the expected pH range. There was growth or material accumulation on the of sewage plant weir.
4. NPDES Permit No. OH0048372, PART II, OTHER REQUIREMENTS R. Total Toxic Organic (TTO) Compliance Options "The permittee may elect to monitor TTO at outfall 0IC00000601 in accordance with subparagraph 1. below or, in lieu thereof, adopt and implement a certified toxic organic management plan (TOMP) and submit certifications in accordance with subparagraph 2. hereof."
At the time of the inspection the facility was storing PCE drums on wooden pallets and not on spill containment pads.

The Toxic Organic Management Plan (TOMP) provided by the facility included several maps with descriptive callouts which were illegible. In the narrative description section on page 3, the TOMP states: "The closed Pero degreaser evaporates all Tetrachloroethylene from the parts before removal. This keeps all Tetrachloroethylene contained in the closed degreaser." The Pero system does have a waste PCE stream and a wastewater PCE stream. The wastewater PCE stream is physically removed from the Pero, as needed, and transferred to a waste drum using a funnel. This procedure is not discussed in the TOMP.
5. NPDES Permit No. OH0048372, PART I, A. Final Effluent Limitations and Monitoring Requirements Table - Final Outfall - 001- Final d. pH Range Excursions - the pH range for compliance is 6.5 to 9.0. With continuous pH monitoring, a violation is established if an excursion event continuous uninterrupted for more than 60 minutes and/or the total minutes of excursion for a month exceeds 7 hours and 26 minutes. EPA reviewed the May 2025 continuous monitoring data for Outfall 001 for this report. The data showed 8 minutes of a pH excursion. Each excursion was associated with a sharp increase in flow. The 8 total minutes of excursion were not reported on the May 2025 DMR. Per Angie Augenstein an email on July 2, 2025, the continuous pH monitoring system was set to report excursions when the pH was outside of 6.0 - 9.0 instead of the current permit range which is 6.5 - 9.0.

As of the time of the inspection, the facility was not able to provide records of the continuous pH monitoring data for Outfall 601.

APPENDIX B: FIELD MEASUREMENT DATA
Dates and times used in this appendix are in the timezone local to the inspection site.
Jonathan Moody conducted field monitoring using an In-Situ AquaTROLL 600 (AT600) S/N 1079129. The AT600 was equipped with a temperature/conductivity sensor, pH/ORP sensor, Rugged Dissolved Oxygen (RDO) sensor and a turbidity sensor. Table 1 summarizes the methods used by the AT600 sensors.
Table 2 below shows the summary of the monitoring. The AT600 was calibrated on May 1, 2025, at 2:00 PM using a 3-point pH calibration with pH 4, 7, and 10 buffers. These calibrations were performed in the Region 5 office in Westlake, Ohio. The RDO sensor was calibrated using the 100% saturation method on May 7, 2025, onsite at the facility and prior to monitoring on that day. The turbidity and temperature sensors both have a factory calibration. Calibration information is documented in the field notebook for this compliance inspection and in the Calibration Report generated by the VuSitu App and appended to the end of this appendix.
A sensor verification was conducted on the pH sensor on May 5, 2025, at 5:30 PM. The summary of the verification is included in Table 4. Table 3 summarizes the sensor verification results.
