This is the first month in 1986 that Construction has not had a Recordable Injury. All Construction employees should be commended for a job well done. The Services Department had the only Recordable Injury at the Technical Center. On October 8th, a Services Materials Handler suffered a back injury from loading and unloading heavy bags of mail while he was assigned on the Technical Center mail truck. At the end of October, Construction has 1 LWC, 30 RI*s, and 16 OTJ-DI's. The Technical Center site now has 0 LWC's, 14 RI's, and 29 OTJ-DI's. The Technical Center reported one Recordable Illness in October. An R/D-SCD employee suffered a vision problem after being exposed to a NIAX catalyst blend. The Construction Department reported two Recordable Illnesses In October. All three illnesses are summarized in the attached BLS/OSHA Injury and Illness Summary. Year-to-date Construction has a total of 4 Recordable Illnesses and the Technical Center has 12. OFE-THE-JOB INJURY SUMMARY Again, the Technical Center had a poor month for OTJ-Disabling Injuries. A total of 7 OTJ-DI's were reported In October. Construction reported 3, Data Process, Distribution, Services, and R/D-SCD each reported I. As of October, the Technical Center has 29 OTJ-DI's and Construction has 16. The following is a brief summary of this month's OTJ-DI's. Please note that one of the Construction OTJ-DI's and the Services OTJ-91 actually occurred in September but were not reported in time to be included in the September monthly report. o 09/26 - Services While on a family camping trip, an employee's dog fell in a lake. While running toward the lake to rescue the dog, the employee slipped on a sloped rock and injured his right leg and ankle. He tore ligaments in his leg and was placed in a cast for 4 weeks. o 09/30 - Construction The employee was carrying a television up some stairs inside his home and slipped and fell. The television landed on his left leg. The employee suffered a strain and bruise to the leg. o 10/10 - Construction The employee suffered multiple injuries in a one vehicle accident. The employee hit gravel in the roadway, lost control of his car and went over an embankment. She suffered injuries to her right knee and back. o 10/21 - Construction While using a wheel barrow to move gravel, the employee pulled ligaments in his left leg. o 10/24 - R/D-SCD The employee fractured his little finger while playing in a pick-up basketball game at a local high school gym. o 10/29 - Data Proc Employee was assaulted with a club and suffered multiple injuries. COLD WEATHER SAFETY In the past couple of years we have had several Injuries due to cold weather conditions. In fact since January, 1985, we have had 9 OTJ-DI's, 6 OSHA RI's and approximately 5 medical visits directly resulting from cold weather conditions, i.e, slips and falls on ice and snow. This year we want to prevent as many cold weather related accidents and injuries as we can. We can do this by being alert to hazardous conditions developed from cold weather and taking preventative action to avoid accidents and Injuries, In the future we will be providing you with safety literature and other information related to cold weather safety. BE ALERT and PLAN for COLD WEATHER NOW. FORK LIFT TRUCK TRAINING The Safety Department will be conducting training for New fork lift operators on December 4, 1986. If you have employees who have never had this training and now need fork lift truck certification, contact P. L. Graham on Ext. 4778. This training is In two parts: Part I is a 3-hour classroom session conducted by the Safety Department; Part II is a Performance Test and this is conducted by the employee's supervisor. This is the last new fork truck operator training session to be conducted this year, so if you have any questions, be sure to call the Safety Department. RESPIRATORY TRAINING The Safety Department will issue a schedule for the upcoming Respiratory Protection training in the next two weeks. If you have employees who need this training and have not completed the attached questionnaire, you should contact the Safety Department immediately. Employees who have not completed this form and consequently have not been approved by the Medical Department will not be trained. 1379A L. R. The employee was performing manual excavation work with a shovel at a small excavation at the east tank cleaning rack In the Institute Plant. The employee reported that an odor In the ground made him nauseated. He became 111 after working In the excavation all morning. The employee did not require medical treatment or restricted work.
