Dear D r. Kehoe:
D r. Bock has asked me to furnish you w ith the inform ation requested in your le tte r of June 21, T968.
In reference to the case of the gasoline "s n iffe r" re ce n tly reported in the J . A . /V i. A . , the most appropriate source o f inform ation would seem to be the a n a ly ti cal values reported for men who have been placed on lead re s tric tio n . A lis t covering the blood (and simultaneously - obtained urin) values for all individuals a t the Baton Rouge p lant who have been restricted a t any tim e from T944 u n til the present date is enclosed. Values for specimens obtained during the 3 -4 months prior to restriction and after restoral are included in this lis t.
There are a few scattered lead values in this lis t that are w ith in the range reported for the patient in the J . A . M . A . a rtic le , but none o f the restricted workers have shown persistently elevated blood lead values comparable to those reported for the gasoline "s n iffe r".
In reference to the possible relationship between the concentration o f lead
in the urine and that in the blood, the above lis t furnishes some d a ta . A listin g o f
a ll determinations on coupled blood and urine specimens recorded for my long-term
group o f workmen is also enclosed. A third source o f inform ation w hich m ight have
some bearing on this point consists o f the Q u a rte rly Reports: A copy of the pertinent
portion o f each Report from T944 through the firs t quarter o f this year is enclosed.
M uch of this inform ation is grouped, o f course. The in d ivid u a l values from w hich
the Q u a rte rly Reports were com piled cannot be recovered from the records kept in
the M edical departm ent. A ll three lists overlap to a greater or lesser extent, o f .
course.
'
As a q u ic k , rough check o f a possible constant relationship between the concentration of lead in the urine and in the blood, I have plotted the distribution of the "ratios" obtained by dividing the milligrams of lead per lite r of urine by the m illigram s of lead per TOO grams o f (a simultaneously obtained sample of) blood for
Robert A . Kehoe, M .D ., J u ly TT, T968 Page 2__________________
those groups in w hich in d ivid u a l values are shown. The distributions so obtained are diown in the graphs attached to the back of the lis t of each such group, I recog nize that a true ratio would necessitate conversion o f values to a common denominator, but the re la tive relationships should not be in great error from the omission of this additional step.
Considering the vagaries o f relationships in time o f sampling to portion of work day and work week, the range/exposure conditions involved, and the problems inherent in the random sampling o f u rin e , I was surprised that the values obtained by the above evalua tion were as closely grouped as they proved to b e . It was also interesting that, in my long-term study group, the results were sim ilar for "pure" organic lead exposures and for "m ixed" organ ic-in org anic lead (furnaces, m elt pot, a llo y) exposures.
A very hurried rough calculation of weighted values for the an a lytica l results shown in the Q u a rte rly Reports indicates the mean " r a tio " (as previously defined) is about 2 .2 ; this is q u ite close to the means of the other groups.
I la ck the tim e to attem pt a more d e fin itiv e analysis o f these data, but, to me, further study would appear to be in d ic a te d . U n fo rtu n a te ly, I have no p ra ctica l experience in matters o f this nature to guide my judgm ent. I shall be most interested to hear whether or not you consider further evalua tion along these lines to be warranted and to learn the results o f any such study that you m ight undertake.
If I can provide further inform ation or assistance, please le t me know .
Sincerely,
TRR/hc A ttach.
