1. James Mixon, Lead Warehouseman, ABERDEEN CHEMICAL PLANT, Aberdeen, MS, 5/2/75, was raising door to loading dock when the spring came loose and the door fell. He attempted to catch door to keep it from falling on other employee and sustained muscle spasms.

2. Hughie Opferkuch, Apprentice Electrician, LAKE CHARLES CHEMICAL PLANT, Lake Charles, LA, 7/3/75, jumped from the bed of a moving truck. He sustained a fractured ankle.
3. James 0. Barr, Compounder, ABERDEEN CHEMICAL PLANT, Aberdeen,
MS, 8/28/75, got leg caught between blender ribbon and hot
blender wall. Injury required amputation of foot just
below ankle.

4. Robert Pettiford, B Operator, NEWARK, 10/16/75, Newark, NJ, was splashed with phenol when disconnecting hose from tank truck nozzle and sustained a superficial burn to face and neck.

1. Reggie L. Jones, "A" Operator, 1/17/76, ABERDEEN CHEMICAL PLANT, Aberdeen, MS, was struck on index finger of left hand when a Lenape manhead cover fell. Injury resulted in amputation of distal and middle phalanges of index fin ger.

1. Geraldine Beall, Operator "C", SAU, BALTIMORE CHEMICAL PLANT, 1/18/77, slipped on patch of ice and wrenched her back. She suffered strained back muscles and ligaments.
2. Vernie L. Bryan, Shift Supervisor, 1/13/77, LAKE CHARLES CHEMICAL PLANT, Westlake, LA, stepped into concrete line ditch while tracing out instrument air line. Sustained second degree thermal burns to both ankles and feet from hot water that had collected in ditch. Steam had dis charged into ditch from liquid N2 vaporizer.
